The purposes of this study are to examine the psychological factors which make us predict the possibility of relapse into alcohol dependence, and to develop and verify the Structural Equation Modeling(SEM) about relapse. This study also examines if the psychological factors can discriminate the recovery of the patients from alcohol dependence.
In study 1, relative effects of the dysfunctional attitude, alcohol expectancy, alcohol facilitating belief, and depression of the patients with alcohol dependence on relapse were considered. Mediating effects of depression on the relationship between alcohol-related belief and relapse were also studied. The subjects were 147 inpatients at alcohol hospitals. The result was that the reliable predictors for relapse of the patients with alcohol dependence were depression, alcohol facilitating belief and dysfunctional attitude. The positive alcohol expectancy was not a significant predictor for relapse. According to the analysis of its mediating effects, depression played a mediating role between dysfunctional attitudes and relapse and between alcohol facilitating belief and relapse.
In study 2, the predictive psychological factors for relapse of the patients with alcohol dependence were identified theoretically as alcohol-related belief, negative affect, emotion-focused coping, problem-focused coping, abstinence self-efficacy. First, the relative effects of these variables toward relapse were studied. Also, to identify how these psychological variables function on relapse, we developed the hypothetical model presupposing a causal relationship and a mediating path among psychological factors for relapse and tested it. The subjects were 213 inpatients or outpatients with alcohol dependence. The reliable predictors for relapse of the patients with alcohol dependence were depression, abstinence self-efficacy, emotion-focused coping, surrounding people's drinking, alcohol problem severity and alcohol facilitating belief. The results of the structural equation analysis showed the partial mediating model suggested in this study was valid, but a fully mediated model, except the path of alcohol-related belief directly predicting relapse, was the best one. It was also revealed that negative affect, emotion-focused coping, problem-focused coping and abstinence self-efficacy played a mediating role in the relation between alcohol-related belief and relapse.
In study 3, both the recovering group finishing the therapy and continuing abstinence and the group under treatment were compared in psychological characteristics. Psychological factors predicting recovery of the patients with alcohol dependence were identified. In the recovery group were 57 people, and in the treatment group were 213 patients. The result was that the two groups showed significant differences in alcohol facilitating belief, dysfunctional attitude, negative affect, depressive mood, problem-focused coping, emotion-focused coping, abstinence self-efficacy, and relapse scale. However, there was no significant difference in surrounding people's drinking. The result of the discriminant analysis, the reliable predictive psychological factors for recovery of the patients with alcohol dependence were denial, perfectionism, and abstinence self-efficacy. This study is expected to provide a guideline for relapse prevention and recovery programs for the patient with alcohol dependence.
