Plasma Type IV Collagen and Fibronectin Concentrations in Diabetic Patients with Microangiopathy

정상인과 당뇨병환자의 혈장에서 type IV collagen과 fibronectin의 농도를 측정하였다. 정상대조군으로서 40명의 건강인(Mean土SD, age; 50.3 土 5.5 yrs)과 미세혈관합병증을 동반하였거나 혹은 동반하지 않은 94명의 당뇨병환자(age; 52.4 土 13.5 yrs)를 실험대상으로 하였다. 당뇨병환자의 혈장중의 type IV collagen의 평균농도(5.3±2.9 ng/ml)와 fibronectin 평균농도(474.4 土 119.4 ug/ml)는 정상대조군(3.7 土 1.3 ng/ml, 319 ± 50.9㎍/ml)에 비하여 유의하게(p<0.01) 높은 것으로 나타났다. 당뇨병환자중에서 합병증을 동반한 환자의 혈장내 type IV collagen 평균농도(6.6 土 3.7ng/ml)는 합병증을 동반하지 않은 경우(4.3 土 1.7 ng/ml)보다 유의하게 (P<0.01) 높았으며, 합병증의 정도가 심할수록 혈장내 type IV collagen의 농도가 높은 것을 알 수 있었다. 혈장내 type IV collagen 농도는 또한 혈중 BUN, creatinine농도와 단백뇨, creatinine clearance(r=-0.31, P<0.001) 등과 밀접한 관계가 있는 것으로 나타나 신장이상의 정도를 잘 반영하였을 뿐만 아니라, 망막증의 경중에 따라서도 의의있는 차이를 보였다. 그러나 혈장내 fibronectin농도는 합병증의 유무나 그 정도에 따른 차이를 보이지 않았다. 이상의 결과로 볼 때, 혈중 type IV collagen은 당뇨병 환자에 있어서 미세혈관합병증의 발병 및 진행과 밀접한 관련이 있으며 이를 인지하는 예민한 척도로서 임상적으로도 유용하겠으나 fibonectin은 그렇지 못한 것으로 사료된다. Plasma type IV collagen and fibronectin levels were measured in 40 healthy controls(Mean ± SD, age ; 50.3 ± 5.5 yr) and 94 diabetic patients(age ; 52.4 ± 13.5 yr) with and without microvacular complications. The mean plasma levels of type IV collagen(5.3 ± 2.9 ng/ml )and fibronectin(474.4 ± 119.4 ug/ml) in diabetic patients were significantly higher(P<0.01) than in healthy controls(3.7 ± 1.3 ng/ml and 319 ± 50.9 ug/ml ). The mean plasma level of type IV collagen in diabetic patients with complications(6.6 ± 3.7 ng/ml) was significantly higher(P<0.01) than those without complications(4.3 ± 1.7ng/ml) and became higher in more complicated patients. Futhermore, the severity of retinopathy and several indicators of nephropaty such as serum BUN and creatinine and proteinuria were closely associated with plasma type IV collagen level and a significant correlation was found between plasma type IV collagen and creatinine clearance(r=-0.31, P<0.001). There was no significant difference in plasma fibronectin concentrations, however, between the diabetic patients with complications and those without complications. The results of this study suggest that plasma type IV collagen seems to be a sensitive guide for the occurrence and progression of diabetic microangiopathy but plasma fibonectin does not
