LCC 분석을 통한 도시가스와 지열 냉 난방설비의 경제성 평가

The purpose of this study is economic analysis of heating and cooling systems that are using gas, geothermal energy through life cycle cost analysis. The adapted research method selected the office building in Seoul region. The building is 15 stories and have total floor area of 270,000㎡. And the capacity of heating and cooling system applied in new building is 700RT to 750RT as a scale of heating and cooling system that can provide same capacity. The Adapted LCC Analysis period on 40 years. Comparing and analyzing life cycle cost about each heating and cooling system in cases, economical evaluation is estimated. The results are as follows: 1) The total life cycle cost of gas heating and cooling system is approximately 7,979million won, the total life cycle cost of geothermal heating and cooling system is approximately 8,073million won. 2) The cost of the geothermal heating and cooling system is higher than gas heating and cooling system about 1.18%. Thus, optimal plan is the gas heating and cooling system.

<P>[Purpose] We investigated the effects of individual strengthening exercises for subdivisions of the gluteus medius in a patient with sacroiliac joint pain. [Subject] A 32 year-old female who complained of pain in the posterior area of the left iliac crest and sacroiliac joints over a period of 6 months was the subject of this study. [Methods] She performed individual strengthening exercises for subdivisions of the gluteus medius over 3 weeks. Pain-provocation tests and VAS scores were evaluated before and after the intervention. [Results] After individual strengthening exercises for subdivisions of the gluteus medius, the subject showed no pain in the Gaenslen, Patrick, or REAB tests for the left sacroiliac joint. The VAS score was less the 3/10, compared with 7/10 initially. [Conclusion] Individual strengthening exercises for the subdivisions of the gluteus medius were effective at reducing SI joint pain for this patient.</P>

<P> [Purpose] This study compared the forward head angle and the lumbar flexion and rotation angles of computer workers using routine and fixed computer workstations. [Subjects] Ten male workers voluntarily consented to participate in the study. [Methods] A 3-D motion analysis system was used to measure the angles of the forward head and lumbar flexion. All subjects performed computer work for 30 minutes using both types of workstation. [Results] When working at the fixed workstation, the forward head angle was less than that observed when the routine workstation was used. At the fixed workstation, the lumbar flexion and rotation angles were less than that at the routine workstation. [Conclusion] The computer workstation individually fixed for standard posture may have prevented poor sitting posture.</P>

<P> [Purpose] This study examined the effects of a ball-backrest chair combined with an accelerometer on the pain and trunk muscle endurance of a computer worker with low-back pain (LBP). [Subject and Methods] A 36-year-old male with a flat back who complained of LBP at the L3–5 level was the subject. He used the ball-backrest chair when working at a computer for 1 week. [Results] After using the ball backrest, the trunk extensor and flexor muscle endurance times had increased compared with the baseline and the VAS score had decreased from 7 to 4. [Conclusion] Combining exercises with information on motion is a useful common treatment approach for improving trunk muscle endurance in LBP.</P>

<P> [Purpose] This study assessed the use of different exercise speeds and isokinetic feedback on the middle and lower serratus anterior muscles during push-up exercises. [Subjects] Ten male workers voluntarily consented to participate in the study. [Methods] The subjects performed push-up exercises under three conditions. Surface electrodes were placed on the dominant right side of the middle and lower serratus anterior muscles. [Results] The middle and lower SA muscle activities under condition 2 were significantly decreased when compared with those under conditions 1 and 3. The middle SA activity under condition 3 was significantly increased when compared with that under condition 1. [Conclusion] This study suggests that proper selection of push-up exercise speed may be necessary for selective strengthening of the SA and that isokinetic feedback information obtained using an accelerator can help in selective strengthening of the middle SA.</P>

Purpose: To identify prognostic factors for disease progression and survival of patients with extracranial oligometastatic breast cancer (EOMBC), and to investigate the role of radiation therapy (RT) for metastatic lesions. Materials and Methods: We retrospectively reviewed the medical records of 50 patients who had been diagnosed with EOMBC following standard treatment for primary breast cancer initially, and received RT for metastatic lesions, with or without other systemic therapy between January 2004 and December 2008. EOMBC was defined as breast cancer with five or less metastases involving any organs except the brain. All patients had bone metastasis (BM) and seven patients had pulmonary, hepatic, or lymph node metastasis. Median RT dose applied to metastatic lesions was 30 Gy (range, 20 to 60 Gy). Results: The 5-year tumor local control (LC) and 3-year distant progression-free survival (DPFS) rate were 66.1% and 36.8%, respectively. High RT dose (≥50 Gy10) was significantly associated with improved LC. The 5-year overall survival (OS) rate was 49%. Positive hormone receptor status, pathologic nodal stage of primary cancer, solitary BM, and whole-lesion RT (WLRT), defined as RT whose field encompassed entire extent of disease, were associated with better survival. On analysis for subgroup of solitary BM, high RT dose was significantly associated with improved LC and DPFS, shorter metastasis-to-RT interval (≤1 month) with improved DPFS, and WLRT with improved DPFS and OS, respectively. Conclusion: High-dose RT in solitary BM status and WLRT have the potential to improve the progression-free survival and OS of patients with EOMBC.

<P> [Purpose] We developed a multi-air-cushion biofeedback device (MABD) to assist the dynamic hug exercise, and investigated the effects of MABD on the shoulder muscles during the dynamic hug exercise. [Subjects] Twelve males aged 22–32 years were recruited. [Methods] We measured the right side serratus anterior, lower trapezius, and upper trapezius muscle activities during the dynamic hug exercise with and without MABD. [Results] The serratus anterior and lower trapezius muscles activities significantly increased during the dynamic hug exercise with MABD compared to without it. The upper trapezius muscle significantly decreased during the dynamic hug exercise with MABD compared to without it. [Conclusion] The results suggest that the dynamic hug exercise with MABD is an effective scapular stability exercise.</P>

<P> [Purpose] The purpose of this study was to compare the isolation ratios of scapular retraction muscles between protracted scapular and asymptomatic groups. [Subjects] Seven males with protracted scapular and seven asymptomatic males aged 20–30 years were recruited. [Methods] We measeured the rhomboid, middle trapezius (MT), and lower trapezius (LT) muscles activities, and calculated the isolation ratio. [Results] The rhomboid and MT isolation ratio of the protracted scapular group was not significantly different from that of the asymptomatic group. The LT isolation ratio of the protracted scapular group was significantly lower than that of the asymptomatic group. [Conclusion] We suggest that a proper retraction exercise, for patients with protracted scapular posture is one that includes exercises for selectively strengthening the lower trapezius muscle.</P>
