        Apicoectomy of maxillary anterior teeth through a piezoelectric bony-window osteotomy: two case reports introducing a new technique to preserve cortical bone

        Two case reports describing a new technique of creating a repositionable piezoelectric bony window osteotomy during apicoectomy in order to preserve bone and act as an autologous graft for the surgical site are described. Endodontic microsurgery of anterior teeth with an intact cortical plate and large periapical lesion generally involves removal of a significant amount of healthy bone in order to enucleate the diseased tissue and manage root ends. In the reported cases, apicoectomy was performed on the lateral incisors of two patients. A piezoelectric device was used to create and elevate a bony window at the surgical site, instead of drilling and destroying bone while making an osteotomy with conventional burs. Routine microsurgical procedures - lesion enucleation, root-end resection, and filling - were carried out through this window preparation. The bony window was repositioned to the original site and the soft tissue sutured. The cases were re-evaluated clinically and radiographically after a period of 12 - 24 months. At follow-up, radiographic healing was observed. No additional grafting material was needed despite the extent of the lesions. The indication for this procedure is when teeth present with an intact or near-intact buccal cortical plate and a large apical lesion to preserve the bone and use it as an autologous graft.

        Over the past two decades, Singapore has steadily become a popular destination for migrants. While the reasons for migrating to Singapore are many and contextual, labour and education have been the primary driving factors for attracting migrants from around the world to Singapore. Although a popular migrants’ destination, education and migration policies in Singapore are often gendered, and are negotiated along and across other axes of identification and differentiation such as ethnicity and ideas of‘modernity’. This article analyses gendered educational migration from Malaysia to Singapore focusing particularly on how educational migration leads to female self-transformation. Specifically, I argue that social actors negotiate educational migration within their gendered family constellations. The article first contextualises the empirical material by illustrating socio-historical processes in Singapore and Malaysia. In the next sections, I discuss my ethnographic methods and examine a brief history of the state of research in gender and educational migration. In conclusion, I offer a significant contribution to the growing and important body of scholarship on gender and transnational families by illustrating how gender is negotiated in migration using the case of a single Chinese woman’s migration journey to becoming a ‘modern woman’.

        Plasma Levels of K18 Fragments Do Not Correlate with Alcoholic Liver Fibrosis

        Background/Aims: Noninvasive markers of liver fibrosis in alcoholic liver disease (ALD) are crucial to establish early intervention. Previous studies have suggested that plasma levels of cleaved keratin-18 (K18; M30) fragments can predict the severity of liver disease. The aim of this study was to correlate plasma M30 levels with stages of liver fibrosis in ALD. Methods: Patients with ALD (n=139, 79.1% males) and liver histology were included, and plasma samples were collected to quantify plasma M30 levels. Patients were stratified into five groups by fibrosis stage (F0=14; F1=15; F2=35; F3=17; and F4=58) according to the Kleiner score. Differences between groups were evaluated using the chi-square test or analysis of variance. Trends by fibrosis stage were calculated by logistic regression analysis, and sensitivity, specificity and positive and negative predictive values were determined. Results: There were no significant differences in M30 levels among fibrosis stages. The correlation between plasma M30 levels and fibrosis was poor (Pearson’s correlation coefficient= 0.13, Spearman rho=0.20 [p=0.02]), and M30 levels did not correlate with alcohol-specific histological features. However, significant correlations of M30 levels with aspartate aminotransferase (Spearman rho=0.653, p<0.001) and alanine aminotransferase (Spearman rho=0.432, p<0.001) were found. M30 levels of >200 U/L reveal a sensitivity for predicting cirrhosis of 84.5% with a negative predictive value of 73.5%. Conclusions: Plasma M30 levels are often elevated in ALD and correlate with serum transaminases but do not reflect fibrosis. The usefulness as a prognostic marker awaits evaluation in prospective studies.

        The G. D. Q. method for the harmonic dynamic analysis of rotational shell structural elements

        This paper deals with the modal analysis of rotational shell structures by means of the numerical solution technique known as the Generalized Differential Quadrature (G. D. Q.) method. The treatment is conducted within the Reissner first order shear deformation theory (F. S. D. T.) for linearly elastic isotropic shells. Starting from a non-linear formulation, the compatibility equations via Principle of Virtual Works are obtained, for the general shell structure, given the internal equilibrium equations in terms of stress resultants and couples. These equations are subsequently linearized and specialized for the rotational geometry, expanding all problem variables in a partial Fourier series, with respect to the longitudinal coordinate. The procedure leads to the fundamental system of dynamic equilibrium equations in terms of the reference surface kinematic harmonic components. Finally, a one-dimensional problem, by means of a set of five ordinary differential equations, in which the only spatial coordinate appearing is the one along meridians, is obtained. This can be conveniently solved using an appropriate G. D. Q. method in meridional direction, yielding accurate results with an extremely low computational cost and not using the so-called "delta-point" technique.

        Static analysis of shear-deformable shells of revolution via G.D.Q. method

        This paper deals with a novel application of the Generalized Differential Quadrature (G.D.Q.) method to the linear elastic static analysis of isotropic rotational shells. The governing equations of equilibrium, in terms of stress resultants and couples, are those from Reissner-Mindlin shear deformation shell theory. These equations, written in terms of internal-resultants circular harmonic amplitudes, are first put into generalized displacements form, by use of the strain-displacements relationships and the constitutive equations. The resulting systems are solved by means of the G.D.Q. technique with favourable precision, leading to accurate stress patterns.

        Effect of boundary conditions on the stability of beams under conservative and non-conservative forces

        This paper, which is an extension of a previous work by Viola et al. (2002), deals with the dynamic stability of beams under a triangularly distributed sub-tangential forces when the effect of an elastically restrained end is taken into account. The sub-tangential forces can be realised by a combination of axial and tangential follower forces, that are conservative and non-conservative forces, respectively. The studied beams become unstable in the form of either flutter or divergence, depending on the degree of non-conservativeness of the distributed sub-tangential forces and the stiffness of the elastically restrained end. A non-conservative parameter ${\alpha}$ is introduced to provide all possible combinations of these forces. Problems of this kind are usually, at least in the first approximation, reduced to the analysis of beams according to the Bernoulli-Euler theory if shear deformability and rotational inertia are negligible. The equation governing the system may be derived from the extended form of Hamilton's principle. The stability maps will be obtained from the eigenvalue analysis in order to define the divergence and flutter domain. The passage from divergence to flutter is associated with a noticeable lowering of the critical load. A number of particular cases can be immediately recovered.

      • A segment containing a G-box and an ACGT motif confers differential expression characteristics and responses to the Arabidopsis Cytc-2 gene, encoding an isoform of cytochrome c

        <P>Sequences required for the expression of Cytc-2 (At4g10040), one of two cytochrome c genes from Arabidopsis thaliana, were characterized using plants transformed with deleted and mutagenized promoter fragments fused to gus. These studies indicated that a region containing a G-box and an ACGT motif is essential for expression. Mutation of the ACGT motif causes a complete loss of expression, while mutation of the G-box causes decreased expression in aerial parts and abolishes expression in roots and induction by environmental factors. Upstream located site II elements are required for maximal expression, mainly in reproductive tissues, and maximal induction by different factors. One-hybrid screenings allowed the identification of transcription factors from the bZIP and bHLH families that interact mainly with the G-box. Four of these factors were able to bind to the Cytc-2 promoter in vitro and in transactivation assays in Arabidopsis. Analysis of available microarray data indicated that the bZIP transcription factors share expression characteristics with the Cytc-2 gene, suggesting that they act as mediators of its response to tissue-specific, environmental, and metabolic conditions. Site II elements interact with a TCP family protein and may co-ordinate the expression of the Cytc-2 gene with that of other respiratory chain components. A model is proposed for the evolution of the Cytc-2 gene through the incorporation of a segment containing a G-box and an ACGT motif into an ancestral gene that contained site II elements. This may have reduced the importance of site II elements for basal expression and conferred new responses to environmental factors.</P>
