        Step-Cut Subtrochanteric Osteotomy Combined with Total Hip Arthroplasty for Neglected Traumatic Hip Dislocations

        Background: Total Hip Arthroplasty remains the standard treatment protocol for patients with neglected traumatic dislocations of the hip with arthritis. A total hip arthroplasty needs to be frequently combined with a subtrochanteric shortening femoral osteotomy to aid in the reduction of the hip joint in such cases. Still long-term stable implant fixation, rigid construct, and favorable functional outcome remain a challenge. In respect to subtrochanteric shortening osteotomy, various techniques have been described in the literature, including the step-cut, double chevron, transverse, and oblique osteotomies. Out of these types, a subtrochanteric step-cut osteotomy provides a better rotational stability and a larger surface of contact to aid in union. As there is a paucity in the literature regarding the step-cut osteotomy for traumatic dislocations of the hip, we designed this study to evaluate the outcomes of this procedure. Methods: We prospectively evaluated 24 patients with neglected traumatic dislocations of the hip, who underwent total hip arthroplasty with a step-cut subtrochanteric shortening osteotomy using a long modular stem within a span of 4 years. The indications were severe pain and difficulty in walking and performing activities of daily living. Patients fulfilling the inclusion criteria were evaluated in terms of Harris Hip Score, leg length discrepancy, neurological status, union of the osteotomy, and implant stability. Results: The mean Harris Hip Score significantly improved from 33.4 preoperatively to 89.2 postoperatively at the latest follow-up. At the final follow-up, all patients showed union at the osteotomy site and there were no cases of implant loosening or instability. No neurological complications were reported. Conclusions: Total hip arthroplasty combined with a step-cut subtrochanteric femoral shortening osteotomy in patients with neglected dislocations of the hip was associated with good functional outcome and higher success rates in terms of stable implant fixation and union at the site of osteotomy.

      • Particle Filter-based Localization of a Mobile Robot by Using a Single Lidar Sensor under SLAM in ROS Environment

        One of the most popular issues in autonomous mobile robots is mapping, localizing and autonomous navigation. In this paper, Adaptive Monte Carlo Localization (AMCL) as particle filters method is presented to show how effectively it localizes the mobile robot in an indoor environment. During the simulations, the robot is localized and autonomously navigates in Gazebo and Rviz environment. The simulation results demonstrated that the particles in the filter quickly converge on the pose and the robot was successfully able to follow the path to reach its goal position. Simulations for a mobile robot to be localized in two different environments, static and dynamic, were carried out. The robot was successful in reaching its goal every time. Simulation results thus point out that AMCL performed effectively in these environments.

        Evaluation of In Vitro Antioxidant and In Vivo Analgesic Potential of Terminalia paniculata Aqueous Bark Extract

        The Terminalia genus includes plants that are used in a variety of food, nutritional products, and traditional medicines. Aqueous bark extract of Terminalia paniculata (TPW) was screened for its antioxidant and analgesic potential. The major polyphenols were identified by high-performance liquid chromatography. In vitro antioxidant potential of TPW was investigated by 1,1-diphenyl-2-picryl hydrazyl (DPPH), 2,20-azinobis-(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS2 -) radical assay, nitric oxide (NO) scavenging, superoxide scavenging (O2 - ), Fe2 + chelating (O-phenanthroline), and ferric reducing/ antioxidant power (FRAP) assay. We evaluated the effects of TPW on cell viability, lipopolysaccharide (LPS)-stimulated reactive oxygen species (ROS), nitrite, and cytokines (interleukin [IL] 6 and tumor necrosis factor alpha [TNF-a]) in RAW264.7 murine macrophages. Evaluation of analgesic activity of TPW was performed using acetic acid-induced writhing and hot plate test in mice. Phytochemical analysis showed the presence of four polyphenols, namely, gallic acid, ellagic acid, rutin, and quercetin. TPW showed maximum superoxide, ABTS2 - , NO, DPPH inhibition, and Fe2 + -chelating property at 400 lg/mL, respectively. FRAP value was 4.5– 0.25 lg Fe(II)/g. TPW, per se, did not affect RAW264.7 cell viability. In LPS-induced RAW 264.7 cells, TPW attenuated the elevation in ROS, nitrite, IL-6, and TNF-a levels. TPW (100–400mg/kg, orally) significantly reduced the number of writhes in a dose-dependent manner compared with the control. Similarly, TPW (400mg/kg, orally) evoked a significant increase in the maximum percentage effect in the hot plate test. The study suggests the efficacy of aqueous bark extract of T. paniculata as a potential antioxidant and analgesic agent.

        Reciprocal Edge Transmission Based Topological Indices of Graphs

        The reciprocal edge transmission of a edge e = uv in a connected graph G is defined as the sum of reciprocal of distances between the edge e and all other edges of a graph G. In this paper we introduce and study new topological indices based on the reciprocal edge transmission, such as reciprocal edge transmission sum-connectivity index, recip- rocal edge transmission atom bond connectivity index, reciprocal edge transmission arithmetic-geometric index, reciprocal edge transmission geometric-arithmetic index, reciprocal edge transmission augmented Zagreb index and reciprocal edge transmission inverse sum indeg index. Further obtain bounds for reciprocal edge transmission based indices of any graph and also give explicite expression for some class of graphs.

        Pulmonary Functions and Inflammatory Biomarkers in Post-Pulmonary Tuberculosis Sequelae

        Background: Post-tuberculosis (TB) sequelae is a commonly encountered clinical entity, especially in high TB burden countries. This may represent chronic anatomic sequelae of previously treated TB, with frequent symptomatic presentation. This pilot study was aimed to investigate the pulmonary functions and systemic inflammatory markers in patients with post-TB sequelae (PTBS) and to compare them with post-TB without sequelae (PTBWS) participants and healthy controls.Methods: A total of 30 participants were enrolled, PTBS (n=10), PTBWS (n=10), and healthy controls (n=10). Pulmonary function tests included spirometry and measurement of airway impedance by impulse oscillometry. Serum levels of matrix metalloproteinase (MMP)-1, transforming growth factor-β, and interferon-γ were estimated.Results: Slow vital capacity (SVC), forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV1/FVC, and peak expiratory flow were significantly lower in PTBS as compared to controls. SVC and FEV1 were significantly less in PTBS as compared to PTBWS. Total airway impedance (Z5), total airway resistance (R5), central airway resistance (R20), area of reactance (Ax), and resonant frequency (Fres) were significantly higher and respiratory reactance at 5 and 20 Hz (X5, X20) were significantly lower in PTBS as compared to PTBWS. Spirometry parameters correlated with impulse oscillometry parameters in PTBS. Serum MMP-1 level was significantly higher in PTBS as compared to other groups.Conclusion: Significant pulmonary function impairment was observed in PTBS, and raised serum MMP-1 levels compared with PTBWS and healthy controls. Follow-up pulmonary function testing is recommended after treatment of TB for early diagnosis and treatment of PTBS.

        Comparison of priming versus slow injection for reducing etomidateinduced myoclonus: a randomized controlled study

        Background: Etomidate injection is often associated with myoclonus. Etomidate injection technique influences the incidence of myoclonus. This study was designed to clarify which of the two injection techniques—slow injection or priming with etomidate—is more effective in reducing myoclonus. Methods: This prospective randomized controlled study was conducted on 189 surgical patients allocated to three study groups. Control group (Group C, n = 63) received 0.3 mg/kg etomidate (induction dose) over 20 s. Priming group (Group P, n = 63) received pretreatment with 0.03 mg/kg etomidate, followed after 1 min by an etomidate induction dose over 20 s. Slow injection group (Group S, n = 63) received etomidate (2 mg/ml) induction dose over 2 min. The patients were observed for occurrence and severity of myoclonus for 3 min from the start of injection of the induction dose. Results: The incidence of myoclonus in Group P (38/63 [60.3%], 95% CI: 48.0–71.5) was significantly lower than in Group C (53/63 [84.1%], 95% CI: 72.9–91.3, P = 0.003) and Group S (49/63 [77.8%], 95% CI: 66.0–86.4, P = 0.034). Myoclonus of moderate or severe grade occurred in significantly more patients in Group C (68.3%) than in Group P (36.5%, P < 0.001) and Group S (50.8%, P = 0.046), but the difference between Groups P and S was not significant (P = 0.106). Conclusions: Priming is more effective than slow injection in reducing the incidence of myoclonus, but their effects on the severity of myoclonus are comparable.

        Innovative approach of in-situ fixed mode dual effect (photo-Fenton and photocatalysis) for ofloxacin degradation

        Novel composite materials composed of clay, foundry sand (FS), and fly-ash (FA) have been employed to immobilize TiO2 for incorporating in-situ dual effect for the degradation of antibiotic ofloxacin. The in-situ generation of iron from the composite beads with surface active TiO2 induced the dual effect of photo-Fenton and photocatalysis. FA/FS/TiO2 beads illustrated the best results (92% removal) at optimized conditions in the batch reactor experiments. The increment in the rate constant along with a decrease in treatment time for the dual effect has proven the credentials of the in-situ dual effect. Synergy in first-order rate constant using dual process was 51% over the single processes of photo-Fenton and photocatalysis. After 35 recycles the viability of the composed beads was observed through SEM/ EDS, UV-DRS and FT-IR analysis, which further justified its use industrially. Estimation of nitrate, nitrite, and ammonia as its by-products was performed for the confirmation of mineralization. Generation of the intermediate products was also identified through GC-MS analysis, and a degradation pathway was proposed. Toxicity test confirming the nontoxic nature of the treated solution was performed on E. coli grown in Miller’s Luria Bertani Broth nutrient medium.
