        Effects of postoperative parenteral nutrition with different lipid emulsions in patients undergoing major abdominal surgery

        Purpose: This study was designed to investigate the effects of total parenteral nutrition (PN) using different lipid emulsions in patients undergoing major abdominal surgery. Methods: Fifty-two patients were randomized to receive soybean oil + medium chain triglycerides (MCT) (group I), soybean oil + olive oil (group II), soybean oil + olive oil + fish oil (group III) as a lipid source. PN was started on postoperative day 1 and patients were maintained on PN for a minimum period of 4 days. Laboratory variables (CRP, prealbumin, transferrin) were measured before surgery and on postoperative days. Results: Three treatment groups were included in the study. Patients in group I received long chain triglycerides (LCT) + LCT/MCT emulsion (%75 LCT + %25 LCT/MCT); Patients in group II received olive oil based emulsion (80% olive oil + 20% soybean oil, ClinOleic); Patients in group III received fish oil in addition to olive oil based emulsion (%85 ClinOleic + %15 Omegaven; Fresenius Kabi). The following 14 parameters were assessed: body weight, CRP, prealbumin, transferrin, tumor necrosis factor-α, interleukin-6, total antioxidant status, thiobarbituric acid reactive substances, oxidized low density lipoprotein-2, complete blood cell, international normalized ratio, D-dimer, activated partially thromboplastin time, prothrombin time. All other parameters showed no differences among the groups. Conclusion: The results of our trial demonstrate a potential beneficial effect of soybean oil/olive oil based lipid emulsions for use in PN regarding inflammatory response and oxidant capacity in the treatment of patients.

        Utilization of Interesterified Fat in the Production of Turkish White Cheese

        The chemical, physicochemical, proteolysis,sensory, and texture characteristics of white cheeses made from interesterified fat were examined throughout ripening for 90 days. The water-soluble nitrogen based ripening indexes of cheeses increased throughout the ripening period. However, there were not large quantitative differences between the peptide profiles of the all cheese samples. Cheeses produced by using fully interesterified fat had higher values for hardness, chewiness, and gumminess than that of control cheese (p<0.05). The polyunsaturated to saturated fatty acid ratios of cheeses were increased due to the presence of interesterified fat. The cholesterol values of cheeses decreased at the rate of between 58.83-89.04%depending on interesterified fat addition. In the sensory analysis, similar scores were obtained for both the control cheese and the other cheeses. The results showed that interesterified fat in cheese production could be used to fully or partially replace the milk fat in cheese.

      • The effect of sweep angle on the limit cycle oscillations of aircraft wings

        This study focuses on the limit cycle oscillations (LCOs) of cantilever swept-back wings containing a cubic nonlinearity in an incompressible flow. The governing aeroelastic equations of two degrees-of-freedom swept wings are derived through applying the strip theory and unsteady aerodynamics. In order to apply strip theory, mode shapes of the cantilever beam are used. The harmonic balance method is used to calculate the frequencies of LCOs. Linear flutter analysis is conducted for several values of sweep angles to obtain the flutter boundaries.

      • Effects of Progressive Relaxation Exercises on Anxiety and Comfort of Turkish Breast Cancer Patients Receiving Chemotherapy

        Background: Breast cancer is the second most common cancer in the world and by far the most frequent cancer among women. Objective: This study was conducted to observe the effect of progressive relaxation exercises on anxiety and comfort level of breast cancer patients receiving chemotherapy. Materials and Methods: A control group pre-test/post-test quasi-experimental model was applied with experimental (30) and control (30) groups, who agreed to participate in this study. Data collection was with the "Personnel Information Form, State-Trait Anxiety Inventory and General Comfort Scale". Results: The average age of the patients that participated in the study was $49.1{\pm}7.96$ years. Eighty-three point three percent (n=25) of the patients in the experiment group and 86.7 (n=26) percent of patients in control group were married. Patient state of anxiety post-test mean scores were $36.2{\pm}8.21$ in the experimental group and $43.4{\pm}7.96$ in the control group, the difference being statistically significant (p<0.05). The general comfort scale post-test mean scores were $149.5{\pm}13.9$ in the experimental group and $137.7{\pm}15.0$ in the control group, again statistically significant (p<0.05). Conclusions: Progressive relaxation exercises positively affect patient comfort and anxiety levels in Turkey.

        원칙에 의한 형벌규범의 정당화 그리고 법익개념

        독일의 형법학에서 법익 (개념)은 근래에 이르기까지 매우 큰 기대를 받아왔다. 체계초월적·체계비판적 법익을 통해서 실정형법을 비판적으로 검토하는 것이 가능할 뿐만 아니라 이러한 법익개념을 이용할 때에만 비로소 형법의 정당화논의가 진행될 수 있을 것이라는 믿음이 그 바탕을 이루고 있다. 그러나 기존의 법익개념만으로는 법익의 범위를 가장 좁게 설정하는 인격적 법익론에서부터 법익의 범위를 훨씬 더 유연하게 바라보는 입장, 실정법을 해설하는 데에 법익의 역할을 국한시키는 견해, 법익개념을 포기하고 법체계 그 자체의 보호를 목적으로 보고 있는 시각 등에 이르기까지 폭넓은 규범정당화의 대화를 이끌고 갈 수가 없다. 중요한 것은 법익의 뜻이 아니라, 무엇이 정당화될 수 있는 법률의 내용인가의 문제이며, 이에 답하기 위해서는 다른 (영미법) 원칙들의 도움이 필요한 상황이다. 특히 파인버그의 ‘해악원칙’과 이를 보충하는 ‘불쾌원칙’이 기존의 법익의 역할을 대체하거나 그 맹점을 보완할 수 있을 것이다. 그동안 형법학에서 이질적으로 생각되었던 ‘후견주의’나 ‘도덕주의’의 사고를 통해 기존 법익개념으로 근거지워질 수 없는 실정법규정들을 이해할 수 있다는 사실을 확인하는 것도 의미가 있다. Der Rechtsgutsbegriff wurde in der einschlagigen strafrechtlichen Debatte zuviel zugemutet. Unter Berucksichtigung einer prinzipiengestutzten Legitimationsdebatte wird seine Entlastung vorgeschlagen. Die hier vorgestellten und exemplarisch angewandten Prinzipien sind z. B. das Schädigungsprinzip und das Storungsprinzip, das aus Hilfe von paternalistischer Normsetzung oder des Moralismus entwickelt wurde. Sie ermoglichen zwar eine fundierten Diskurs uber die inhaltlich umstrittenen Legitimationsfragen; aber sie scheinen ganz ohne einen Rechtgutsbegriff auszukommen. Dennoch behaupte die Schrift, dass vom Rechtgutsbegriff Abschied zu nehmen sei, sondern sieht ihn gerade in seiner systemkritischen Gestalt als sinnvoll an. Die herkommliche Rechtsgutsdebatte versucht, den Nachweis zu erbringen, dass ein bestimmtes Etwas“ ein Rechtgut sei. Ist dieser Nachweis gelungen, soll sich daraus die Legitimation strafrechtlicher Normen zum Schutz dieses Etwas“ ergeben. Die hier behauptete Ansatzpunkt meint, dass man umgekehrt verfahren muss. Wird vorschlagen, einen bestimmten Gegenstand (ein Interesse, eine Ressouce, einen Wert o. a.) strafrechtlich zu schutzen muss, gestützt auf kritisch-ethische Prinzipien, plausibel gemacht werden, dass dieser Gegenstand solchen Schutz verdient. Ist dieser Nachweis erbracht, ergibt sich-als Schlussfolgerung aus den prinzipiellen Erwagungen -, dass es sich bei diesem Gegenstand im strafrechtlichen Sinne um ein Rechtsgut handelt.

        Determination and classification of intraoral phosphor storage plate artifacts and errors

        Purpose: The aim of this study was to determine the reasons and solutions for intraoral phosphor storage plate (PSP) image artifacts and errors, and to develop an appropriate classification of the artifacts. Materials and Methods: This study involved the retrospective examination of 5,000 intraoral images that had been obtained using a phosphor plate system. Image artifacts were examined on the radiographs and classified according to possible causative factors. Results: Artifacts were observed in 1,822 of the 5,000 images. After examination of the images, the errors were divided into 6 groups based on their causes, as follows: images with operator errors, superposition of undesirable structures, ambient light errors, plate artifacts (physical deformations and contamination), scanner artifacts, and software artifacts. The groups were then re-examined and divided into 45 subheadings. Conclusion: Identification of image artifacts can help to improve the quality of the radiographic image and control the radiation dose. Knowledge of the basic physics and technology of PSP systems could aid to reduce the need for repeated radiography.
