Effect of the number of splinted abutments on the accuracy of zirconia copings

<P><B>Abstract</B></P> <P><B>Statement of problem</B></P> <P>Splinting periodontally or structurally compromised abutments is a common procedure. However, studies that investigated the effect of splinting on the accuracy of zirconia copings are lacking.</P> <P><B>Purpose</B></P> <P>The purpose of this in vitro study was to evaluate the magnitude of marginal and internal gaps of zirconia copings based on the number of splinted abutments and digital scan technique.</P> <P><B>Material and methods</B></P> <P>Four maxillary acrylic resin incisors were prepared for ceramic restorations. Epoxy resin master models were fabricated by duplicating the acrylic resin model. An intraoral scanner was used to build 40 digital models (direct digital scan group [DD]); an additional 40 digital models were constructed by scanning 40 maxillary definitive casts using a laboratory scanner (indirect digital scan group [ID]). During computer-aided design, the DD and ID groups were subdivided into 4 groups; single unit (group SU); 2-unit splinted (group 2S); 3-unit splinted (group 3S), and 4-unit splinted (group 4S). Zirconia copings were then fabricated. Using the replica technique, marginal, axial, and occlusal gaps were measured and statistically evaluated (α=.05).</P> <P><B>Results</B></P> <P>In DD, group 4S exhibited larger marginal gaps than groups SU, 2S, and 3S (<I>P</I><.001). In ID, group SU exhibited smaller marginal and occlusal gaps than group 4S (<I>P</I><.001). Group 4S exhibited significantly greater axial gaps than groups SU, 2S, and 3S (<I>P</I><.001).</P> <P><B>Conclusions</B></P> <P>As the number of splinting units increased, the magnitude of the marginal gap tended to increase; however, the differences were less than 5 μm, which is clinically insignificant. Axial gaps in the ID group demonstrated a significant difference (>35 μm) among the groups.</P>
Hip Fracture in the Elderly Aged 80 Years and More

목적: 80세 이상의 노인에서 고관절 골절의 발생률과 골절 후 활동력 및 사망률에 대해 알아보고자 하였다. 대상 및 방법: 광주광역시와 전라남도에서 2001년에 발생한 고관절 골절에 대하여 역학적 조사를 하였다. 환자의 일차정보는 건강보험 심사평가원에 등록된 환자를 대상으로 하였으며, 이들을 대상지역의 정형외과 병ㆍ의원의 등록환자와 비교 검토하였다. 결과: 2001년에 80세 이상에서 341명의 환자에서 고관절 골절이 발생되었으며, 인구 10,000명당 63예(63/10,000)의 발생률을 보였다. 이러한 발생률은 백인종의 발생률보다는 낮았으며, 다른 아시아 국가의 발생률과는 비슷하였다. 341명을 대상으로 골절 후 사망률과 활동력에 대해 조사하였다. 150명(44%)이 침상 생활(grade Ⅳ)만 가능하였고, 66명(l9%)이 옥내생활(grade Ⅲ)이 가능하였으며, 125명(37%)이 골절 전과 동일하거나 약간 감소된 활동(grade Ⅰ, Ⅱ)이 가능하였다. 골절 후 1년 내 사망률은 26.1% (89사망/341명)였으며, 이는 일반 사망률의 2.1배였다. 결론: 골절 후 많은 환자에서(63%) 기능적 활동력을 회복하지 못하였으며, 사망률은 일반 사망률보다 2.1배 높았다. 또한 환자의 활동력 감소가 사망률 증가의 가장 중요한 인자이었다. Purpose: To determine the incidence, vitality (postinjury activity level), and mortality after a hip fracture in elderly patients aged 80 years and over. Materials and Methods: This study examined patients aged 80 years and over, living in Gwangju city and Jeonnam province, and who sustained a hip fracture during 2001. The primary patient information was obtained from the Health Insurance Review Agency (HIRA). As a crosscheck, all hospitals to which patients suspected of having a hip fracture might have been referred were visited. Results: A hip fracture occurred in 341 patients in this age group during 2001, with an incidence of 63 per 10,000 of the population (341/54,266). This incidence is lower than those reported in Caucasians, but is similar to that reported in other Asian countries. The postinjury activity levels were bedridden (grade Ⅳ) in 150 (44%), housebound with severe discomfort (Grade Ⅲ) in 66 (19%), and full or moderate activity similar to the preinjury level (Grade Ⅰ and Ⅱ) in 125 (37%). The one-year mortality rate of the fracture patients was 26.1 % (89 deaths/341 patients), which was 2.1 times higher than that of the general population (12.6%). Conclusion: Many patients (63%) did not achieve a high level of functional ability. The one-year mortality rate was 2.1 times higher than that of the general population, and level of activity had the most significant effect on the mortality rate.

<P>In this work, oxyfluorination treatments on carbon fiber surfaces were carried out to improve the interfacial adhesion between carbon fibers and polarized-polypropylene (P-PP). The surface properties of oxyfluorinated carbon fibers were characterized using a single fiber contact angle, and X-ray photoelectron spectroscopy. The mechanical properties of the composites were calculated in terms of work of adhesion between fibers and matrices and also measured by a critical stress intensity factor (K(IC)). The K(IC) of oxyfluorinated carbon fibers-reinforced composites showed higher values than those of as-received carbon fibers-reinforced composites. The results showed that the adhesion strength between the carbon fibers and P-PP had significantly increased after the oxyfluorination treatments. As the theoretical and practical comparisons, OF-CF-60s showed the best mechanical interfacial performance due to the good surface free energy. This indicates that oxyfluorination produced highly polar functional groups on the fiber surface, resulting in strong adhesion between carbon fibers and P-PP in this composite system.</P>
Inhalation injury after a landmine explosion: a case report

Blast injuries are divided into four classes, and inhalation injuries are a quaternary class of blast injuries. An inhalation injury can be critical to the patient due to the possibility of related complications, such as airway obstruction resulting from upper airway edema and pneumonia. Once diagnosed, an inhalation injury should be treated with early intubation, aerosol therapy, and antibiotics as soon as possible. We should suspect this injury in circumstances involving fire and especially bomb attacks in a military setting. Antipersonnel landmines designed to damage the soldier by amputating the leg can cause blast injuries, but their power is limited to the lower extremity. However, we found an inhalation injury in a victim whose leg had been amputated by an antipersonnel landmine. As soon as we suspected an inhalation injury, we intubated the patient to preserve his airway and started acetylcysteine/heparin aerosol therapy. The patient also was treated with proper antibiotics for right lower lung pneumonia that developed as a sequela of inhalation injury. We could extubate the patient without any complications such as airway obstruction on the third day of intensive care, after which the patient was transferred to the general ward for active rehabilitation. This report presents the first known case of inhalation injury due to a landmine explosion.
Acute methemoglobinemia after a blast injury: a case report

Methemoglobin is a structurally modified form of hemoglobin incapable of binding oxygen, and elevated levels of methemoglobin cause tissue hypoxia. Occupational exposure to 2,4,6-trinitrotoluene, commonly called trinitrotoluene, causes methemoglobinemia. This case report describes a 27-year-old male sergeant who developed methemoglobinemia upon exposure to trinitrotoluene after a blast injury while welding the walls of tank shells. This is the first case of its kind in Korea. The patient had multiple burns in his abdomen and open fractures in his right leg. While his body temperature, heart rate, respiratory rate, arterial blood pressure, and chest X-ray were normal, arterial gas analysis revealed acute (methemoglobinemia concentration, 13.5%; oxygen saturation, 92.0%), probably caused by nitroglycerin exposure. Aspiration and adsorption through the skin and respiratory system were suspected to be the routes of entry. His methemoglobinemia normalized after 4 days after treating the wounds surgically, administering oxygen therapy, and performing blood transfusion.
