        The Investigation of Treatment of Pistachio Processing Industrial Wastewater by Electrochemical Methods in Terms of Chemical Oxygen Demand and Total Phenol Removal

        This work aims to investigate the efficiency of electrocoagulation (EC) of pistachio processing industrial wastewater (PPIW) using the continuous EC process. The tubular reactor made of stainless steel with an internal diameter of 60 mm was used as a cathode electrode. The effect of some parameters was examined on the removal of chemical oxygen demand (COD) and total phenols (TP) removal efficiency. The influences of the initial pH of wastewater (from 4 to 8), flow rate (from 25 to 125 mL/min), current density (from 7 to 21 mA/cm<sup>2</sup>), and supporting electrolyte type (NaCl, NaNO<sub>3</sub>, and Na<sub>2</sub>SO<sub>4</sub>), supporting electrolyte concentration (from 10 to 100 mg/L NaCl) on removal efficiency were investigated to determine the best experimental conditions. The examination of the physico-chemical parameters during the EC treatment showed that the best removal efficiency was obtained under conditions where the flow rate was 25 mL/min (20 min reaction time), the pH value was 5.2, and the current density was 21 mA/cm<sup>2</sup> has set. Under these experimental conditions, COD and TP removal efficiency were found to be 75% and 97%, respectively, while energy consumption was 18.5 kW h/m<sup>3</sup>. The study results show that the EC can be applied to PPIW pre-treatment.

        An ANFIS perspective for the diagnosis of type II diabetes

        In this paper, we have detected on diabetes disease, which is a very common and important disease using Principal Component Analysis (PCA) and adaptive neuro-fuzzy inference system (ANFIS). The aim of this study is to improve the diagnostic accuracy of diabetes disease combining PCA and ANFIS. Firstly, we will introduce soft computing(SC) methods and briefly refer to the applications of these methods in medicine. After, an intelligent diagnostic system for diabetes will then be developed on the PCA and the ANFIS. The structure of ANFIS with PCA intelligent system for diagnosis of diabetes is composed by two processes: In the first process, it is used PCA to find that the distinct classes of T2DM subjects and controls. Second, features of two classes(healthy and T2DM patients) obtained in first process are given to inputs of ANFIS classifier.

        Canal Surfaces in Pseudo-Galilean 3-Spaces

        A Novel Sensorless Control Approach for IPMSM Using Extended Flux Based PI Observer for Washing Machine Applications

        Improvement of the low-speed sensorless position control for an interior permanent magnet synchronous motor (IPMSM), which drives a horizontal axis washing machine drum, extended-flux (EF) model-based PI observer (PIO) system is proposed. This control is designed to estimate the derivatives of the EF components, which are voltage components, from which the rotor position information is extracted. Motor parameters change during the operation because of the load and temperature, and those changes affect the performance of sensorless IPMSM drive, especially at low-speed operation. For satisfactory low-speed operation, observer gains should be carefully selected to reduce the influence of measurement noise and un-modeled dynamics. Linear quadratic (LQR) control method is used to calculate the optimal coefficients of a PIO to improve low-speed performance. The PI observer is chosen for its capability to decouple modeling uncertainties and measurement noise. The effectiveness of the proposed sensorless method is verified by the simulation on MATLAB/Simulink and experimental results in a 900W IPMSM drive for washing machine. The proposed estimator achieves stable operation at 87 rpm 100% and 45 rpm 25% load torque conditions. It has a much better dynamic performance and improved capability and is robust for speed and torque variations in the system.

        Resveratrol-induced Depression of the Mechanical and Electrical Activities of the Rat Heart is Reversed by Glyburide: Evidence for Possible KATP Channels Activation

        Resveratrol, a natural phytoalexin found in wine, has been suggested to have benefits in preventing cardiovascular diseases. However, the direct effects of resveratrol on the activity of cardiac tissues and its mechanism of action have not been determined. This study examined the effects of resveratrol on the right and left atrium and left papillary muscle isolated from the rat heart. The contractile responses of the right atrium and papillary muscle and the action potential from the left atrium were recorded and the effects of resveratrol on these responses were observed. The resting force of the isolated right atrium and the peak developed force of the left papillary muscle were depressed by resveratrol (0.1 nM - 0.1 mM). Exposure to the KATP channel blocker glyburide (3 µM) prevented significantly the resveratrol-induced decrease. Resveratrol (0.1 mM) shortened the repolarization phase of action potential recorded from the left atrium and this effect of resveratrol was reversed by glyburide (3 µM). These results indicated that resveratrol depressed cardiac muscle contraction and shortened action potential duration probably due to the activation of KATP channels in the rat heart.

      • A Retrospective Multicenter Evaluation of Cutaneous Melanomas in Turkey

        Background: We defined melanoma distribution in a large series of Turkish patients and evaluated the prognostic parameters of melanomas. Materials and Methods: A total of 1574 patients' data was retrospectively collected at 18 centers in Turkey. Demographic characteristics were questioned and noted. Prognostic parametres were evaluated based on sentinel lymph node involvement. Results: Mean age was 56.7 (4-99) years. While 844 (53.6%) cases were male, 730 (46.4%) cases were female. One thousand four hundred forty-seven (92%) cases were invasive melanoma and 127 (8%) cases were in-situ melanoma. The most common histopathological form was the superficial spreading melanoma (SSM) which was found in 549 patients (37.9%). It was followed by nodular melanoma in 379 (26.2%), acral lentiginous melanoma (ALM) in 191 (13.2%) and lentigo maligna melanoma in 132 (9.1%), respectively. On univariate analysis, lymphovascular invasion (p<0.001), tumor thickness (p<0.001), histopathological subtype (p<0.001), Clark level (p=0.001), ulceration (p<0.001), ${\geq}6/mm^2$ mitosis (p=0.005), satellite formation (p=0.001) and gender (p=0.03) were found to be associated with sentinel lymph node positivity. Regression was associated with sentinel lymph node negativity (p=0.017). According to multivariate analysis, lymphovascular invasion and tumor thickness were significant independent predictive factors of SLN positivity. Patient age, tumor localization, precursor lesions, lymphocytic infiltration and neurotropism were not related with sentinel lymph node involvement. Conclusions: In this retrospective analysis, it was found that the prevalence of SSM is at a lower rate while the prevalence of ALM is at a higher rate when compared to western countries. According to Breslow index; most of the melanoma lesions' thickness were greater than 2 mm, corresponding Clark IV. Vascular invasion and tumor thickness are the most important factors for sentinel lymph node involvement.

        Treatment of Syringomyelia due to Chiari Type I Malformation with Syringo-Subarachnoid-Peritoneal Shunt

        Chiari type I malformation is a tonsillar herniation more than 3 mm from the level of foramen magnum, with or without concurrent syringomyelia. Different surgical treatments have been developed for syringomyelia secondary to Chiari's malformations: craniovertebral decompression with or without plugging of the obex, syringo-subarachnoid, syringo-peritoneal, and theco-peritoneal shunt placement. Shunt placement procedures are useful for neurologically symptomatic large-sized syrinx. In this paper, authors define the first successful treatment of a patient with syringomyelia due to Chiari type I malformation using a pre-defined new technique of syringo-subarachnoid-peritoneal shunt with T-tube system.

        Use of noninvasive volume assessment methods to predict acute blood loss in spontaneously breathing volunteers

        Objective The use of noninvasive volume assessment methods to predict acute blood loss in spontaneously breathing patients remains unclear. We aimed to investigate changes in the pleth variability index (PVI), vena cava collapsibility index (VCCI), end-tidal carbon dioxide (EtCO2), pulse pressure (PP), and mean arterial pressure (MAP) in spontaneously breathing volunteers after acute loss of 450 mL blood and passive leg raise (PLR). Methods This prospective observational study enrolled healthy volunteers in the blood donation center of an academic hospital. We measured the PVI, EtCO2, VCCI, MAP, and PP before blood donation; at the 0th and 10th minute of blood donation; and after PLR. The primary outcome was the changes in PVI, EtCO2, VCCI, MAP, and PP. Results We enrolled thirty volunteers. There were significant differences among the four obtained measurements of the PVI, EtCO2, and MAP (P<0.001, P<0.001, P<0.001, respectively). Compared to the predonation values, post-hoc analysis revealed an increase in the PVI at the 0th min postdonation (mean difference [MD], 5.4±5.9; 95% confidence interval [CI], -7.6 to -3.1; P<0.001); a decrease in the EtCO2 and MAP at the 0th and 10th minute postdonation, respectively (MD, 2.4±4.6; 95% CI, 0.019 to 4.84; P=0.008 and MD, 6.4±6.4; 95% CI, 3 to 9.7; P<0.001, respectively). Compared with EtCO2 at the 10th minute, the value increased after PLR (MD, 1.8±3.2; 95% CI, 0.074 to 4.44; P=0.006). Conclusion The PVI and EtCO2 could detect early hemodynamic changes after acute blood loss. However, it remains unclear whether they can determine volume status in spontaneously breathing patients.

        Relapsed Wilms' tumor with multiple brain metastasis

        Wilms' tumor is the most common malignant renal tumor in childhood. The brain metastasis of a Wilms' tumor with anaplastic histopathology is rare. We present the case of an 8-year-old girl with Wilms' tumor, who presented with multiple brain metastases 5 years after her primary diagnosis. The brain masses were diagnosed after a generalized tonic-clonic seizure attack. The big solid mass in the cerebellum was resected, and whole-brain radiotherapy was performed, after which, she succumbed to her disease. In the case of clinical suspicion, cranial surveillance should be included in the routine clinical work-up for Wilms' tumor. Combined aggressive therapy (surgery+radiotherapy+chemotherapy) should be applied whenever possible, for both better survival and palliative aspects.

        Reliability of chest pain risk scores in cancer patients with suspected acute coronary syndrome

        Objective The history, electrocardiogram, age, risk factors, troponin (HEART), the thrombolysis in myocardial infarction (TIMI), and Global Registry of Acute Coronary Events (GRACE) scores are useful risk stratification tools in the emergency department (ED). However, the accuracy of these scores in the cancer population is not well known. This study aimed to compare the performance of cardiac risk stratification scores in cancer patients with suspected acute coronary syndrome (ACS) in the ED. Methods This prospective cohort study recruited patients with cancer who visited the ED because of suspected ACS. The development of any major adverse cardiac events (MACE) within 6 weeks was recorded, with the study outcome being a MACE within 6 weeks of ED admission. Results A total of 178 patients participated in this study, of whom 5.6% developed a MACE. Statistically significant differences were found between the mean HEART and TIMI scores in predicting MACE. The HEART score had the highest area under the curve (0.64; 95% confidence interval, 0.48–0.81), highest sensitivity (80%), and highest negative predictive value (97.5) in patients with cancer. Conclusion We found a similar rate of MACE in cancer patients with low-risk chest pain compared to that in the general population. However, the HEART, TIMI, and GRACE scores had a lower performance in cancer patients with MACE compared to that in the general population.
