        Performing angiographic intervention with a femoral entry shield: Element analysis microscopy and hand dose reduction for interventional radiologist

        To unveil and delineate the elements applicable to the radiation protection of a femoral entry shield, calculate its mass attenuation coefficient, and demonstrate its dose reduction efficacy for interventional radiologist performing transarterial embolization (TAE) of ruptured hepatocellular carcinoma (rHCC). The lead equivalency of the shield was firstly validated. Electron microscopy was used to confirm the femoral entry shield being lead-free and to analyze the elemental content, with which the mass attenuation coefficient of the shield was calculated. An adult phantom, irradiated at the upper abdomen to simulate the TAE of rHCC, was used together with a dosimeter attached to the palm of a hand phantom. The dose rates at the hand phantom were measured, with the rHCC clinical protocol, without and with the femoral entry shield placed over the right femoral access site of the adult phantom. Without using the shield, the average hand dose rate was measured to be 0.325 µSv/sec. While using the shield, it was determined to be 0.110 µSv/sec. There was significant 66% dose reduction to the hand dose of IRs performing angiographic intervention with the femoral entry shield.

        Daily adaptive proton therapy: Feasibility study of detection of tumor variations based on tomographic imaging of prompt gamma emission from proton–boron fusion reaction

        In this study, the images of specific prompt gamma (PG)-rays of 719 keV emitted from protoneboron reactions were analyzed using single-photon emission computed tomography (SPECT). Quantitative evaluation of the images verified the detection of anatomical changes in tumors, one of the important factors in daily adaptive proton therapy (DAPT) and verified the possibility of application of the PG-ray images to DAPT. Six scenarios were considered based on various sizes and locations compared to the reference virtual tumor to observe the anatomical alterations in the virtual tumor. Subsequently, PG-rays SPECT images were acquired using the modified ordered subset expectation-maximization algorithm, and these were evaluated using quantitative analysis methods. The results confirmed that the pixel range and location of the highest value of the normalized pixel in the PG-rays SPECT image profile changed according to the size and location of the virtual tumor. Moreover, the alterations in the virtual tumor size and location in the PG-rays SPECT images were similar to the true size and location alterations set in the phantom. Based on the above results, the tumor anatomical alterations in DAPT could be adequately detected and verified through SPECT imaging using the 719 keV PG-rays acquired during treatment

        Application of sigmoidal optimization to reconstruct nuclear medicine image: Comparison with filtered back projection and iterative reconstruction method

        High levels for noise and a loss of true signal make the quantitative interpretation of nuclear medicine (NM) images difficult. An application of profile optimization using a sigmoidal function in this study was used to acquire the NM images with high quality. And the images were acquired by using three kinds of reconstruction method using each same sinogram: a standard filtered back-projection (FBP), an iterative reconstruction (IR) technique, and the sigmoidal function profile optimization (SFPO). Comparison of image according to reconstruction method was performed to show a superiority of the SFPO for imaging. The images reconstructed by using the SFPO showed an average of 1.49 times and of 1.17 times better in contrast than the results obtained using the standard FBP and the IR technique, respectively. Higher signal to noise ratios were obtained as an average of 12.30 times and of 3.77 times than results obtained using the standard FBP and the IR technique, respectively. This study confirms that reconstruction with SFPO (vs FBP and vs IR) can lead to better lesion detectability and characterization with noise reduction. It can be developed for future reconstruction technique for the NM imaging.

        Characteristic Uptake Pattern of Bone Scintigraphy in Patients with Hepatocellular Carcinoma Following Treatment with High-Intensity Focused Ultrasound

        Purpose This study retrospectively reviews the characteristic bone scintigraphic findings in 18 patients with hepatocellular carcinoma (HCC) following treatment with high-intensity focused ultrasound (HIFU). A potential complication of HIFU is damage to the tissues along the path of the ultrasound beam and structures superficial to the lesion of interest. Methods Patientswith hepatocellular carcinomawho underwent a bone scan between 1st December 2005 and 31st December 2011 were considered for this study. Among these patients, only those who had bone scans after the HIFU treatment were included. The time between HIFU treatment and bone scans, HIFU energy, HCC sites, tumour sizes and related radiological findings were evaluated. Results In total, 20 bone scans of 18 patients were reviewed. Of these scans, two patients were normal; three patients showed decreased uptake, four patients showed increased uptake and nine patients showed mixed uptakes of the bony tracer in their rib cages. The defects were located in the anterior, lateral, anterolateral or posterolateral aspects of the rib cage. The majority of those cold defects were in the right anterior rib cages. SPECT/CT was used to localise the decreased uptake in ribs. The magnetic resonance imaging in individual patients invariably showed ill-defined rimenhancement along the right chest wall, signifying chest wall injury. Conclusions The results showed that tissue ablation using HIFU caused tissue injury along the pathway of highintensity ultrasound beams. The harm to tissues is presented as photopenic area on the rib cages due to necrosis or hot spots due to rib fractures in the bone scan. Since these cold defects are subtle, they are easily overlooked or mistaken as aggressive bony metastasis.
