Korean Red Ginseng Powder in the Treatment of Melasma

Exposure to the sun, ultraviolet radiation, and oxidative stress are the chief etiologic factors responsible for melasma. The ingredients of Korean red ginseng powder, which include ginsenoside and phenolic compounds, have antioxidative effects and reduce ultraviolet B-induced pigmentation. This study was designed to evaluate the effectiveness and safety of Korean red ginseng powder in patients with melasma. In 25 female patients, 3 g of Korean red ginseng powder was orally administered for a 24 week period. The level of pigmentation and erythema were determined and clinical improvement was evaluated by the melasma area and severity index (MASI), melasma quality of life scale (MELASQoL), and patient- and investigator-rated global improvement scale. After 24 weeks, the MASI score decreased from 8.8 to 5.6, and MELASQoL showed improvement in 91% of patients (p<0.05). The mean level of pigmentation decreased from 184.3 to 159.7 and erythema levels decreased from 253.6 to 216.4 (p<0.05). Additionally, 74% of the patients showed some improvement in both patient- and investigator-rated global improvement scales at week 24. Korean red ginseng powder was well tolerated by most of the patients. In conclusion, Korean red ginseng powder showed good tolerability and beneficial effects in patients with melasma. The use of Korean red ginseng would be counted as a useful adjunctive therapy for patients with melasma.

Blue toe syndrome involves blue or purplish toes in the absence of trauma, serious cold exposure, or disorderscausing general cyanosis. Clinical presentation can range from a cyanotic toe to a diffuse, multi-organ systemicdisease. A 75-year-old man presented with claudication, sudden bilateral painful discoloration of the sole, bluecoloredtoes, and anuria. Three weeks earlier, he had been diagnosed with acute myocardial infarction and hadundergone catheterization for percutaneous coronary intervention. Histopathologic findings showed vascular ectasiawith mild perivascular inflammation. Based on patient history, physical examination, and laboratory findings, he wasdiagnosed with blue toe syndrome. Our patient presented with clinical manifestations, including peripheral cutaneousinvolvement and acute deterioration of renal function. This case highlights the importance of prompt diagnosis ofblue toe syndrome by careful history-taking and physical examination in order to avoid multi-organ systemic disease. Nevus sebaceous appears at birth or within the first few months after birth. Secondarily to nevus sebaceous, benign or malignant tumors such as basal cell carcinoma, trichoblastoma, syringocystadenoma papilliferum, squamous cell carcinoma, sebaceoma, and sebaceous carcinoma might occur. Sebaceoma is a benign tumor composed of incompletely differentiated sebaceous cells of varying degrees of maturity. We experienced an uncommon case of malignant transformation of sebaceoma in a 47-year-old man who had a solitary, well-defined, 3.1×2.1 cm sized, round, erythematous nodule on the scalp. Histopathologic examination revealed the presence of irregular lobules of varying size with a well-defined margin from the adjacent interstitial tissue in the dermis. Focally there were atypical cells with a foamy cytoplasm and undifferentiated cells. To the best our knowledge, only one case, from another country, has been reported in the literature. Therefore, we report a very rare case of malignant transformation of sebaceoma that occurred in nevus sebaceous.

Carcinoma of the uterine cervix is a common malignancy for women in Korea. It frequently metastasizes to the lungs, bone, or liver, but cutaneous metastasis is rare, with a reported incidence of only 1.3%. In cases of cutaneous metastasis, the lesions present in the forms of nodules, ulcers, or plaques, and are generally located in the abdominal wall, vulva, anterior chest wall, lower extremities, and upper extremities. Here, we describe a case of a 64-year-old woman diagnosed with cutaneous metastases from squamous cell carcinoma of the uterine cervix on both flanks, lower abdominal wall, and both inguinal areas, three years after receiving chemoradiation therapy. At presentation, the primary carcinoma had been resolved completely. Histological and immunohistochemical findings revealed intact epidermis and infiltration of tumor cells in the dermis, which showed positivity for cytokeratin 5/6, cytokeratin 7, and tumor protein 63, consistent with metastatic squamous cell carcinoma of the uterine cervix.

Pterygium unguis occurs as a result of the scarring process between the proximal nail-fold and matrix and lichen planus is a typical example, although it has also been reported to occur in patients with sarcoidosis or leprosy. Lichen striatus is a childhood linear inflammatory dermatitis of unknown etiology. Many nail changes such as nail-bed hyperkeratosis, nail pitting, longitudinal ridging, onycholysis, and longitudinal fissures have been described in patients with lichen striatus, but pterygium unguis is very rare. It has been suggested that the onset and resolution of onychodystrophy correlates well with the course of skin eruption, but pterygium unguis results in permanent nail changes regardless of skin eruption. A 3-year-old boy presented with a linear papular eruption on the right hand and pterygium on the index finger. He was diagnosed with lichen striatus by histopathological findings, and the papular eruption was resolved with topical tacrolimus ointment, but the pterygium did not improve.

Kawasaki disease (KD) is an acute, systemic vasculitis of unknown etiology that occurs predominantly in infants and young children. The diagnosis of KD is based on the presence of fever lasting longer than 5 days and at least other four of the five clinical criteria. Patients with incomplete presentations of KD have a higher complication rate which is likely related with delayed diagnosis and treatment. We report the first Korean case of incomplete KD in which cutaneous change on a previous BCG vaccination site play an important role as a diagnostic clue. (Korean J Dermatol 2013;51(8):631∼634)

Livedo racemosa (LR) is characterized by a striking violaceous netlike patterning of the skin similar to the livedo reticularis, from which it differs by its localization (more generalized), and shape (irregular, broken circular segments). LR is probably caused by patchy impairment of cutaneous arteriolar circulation, resulting in venous dilatation and stasis of blood. LR is always associated with a pathological condition, including hematologic/hypercoagulable disease, vasculitis, connective tissue diseases, neoplasm, lymphoma, infection, cerebrovascular disease, adverse response to a drug, and etc. Hence, clinical, pathological and laboratory examinations are important for excluding these underlying diseases. To date, there have been few reports of LR secondary to posttransplant lymphoproliferative disease (PTLD) in dermatologic literatures. Herein, we report a case of LR associated with Epstein-Barr virus-induced PTLD of cervical lymph node in a 19-year-old female, who had generalized reticular erythematous to violaceous patch on the entire body after allogenic peripheral blood stem cell transplantation. Unilateral forehead paralysis is a well-documented symptom which follows the superficial temporal branch of the facial nerve injury. The temporal branch holds the highest risk of injury during head and neck surgery among facial nerve branches because it runs superficially above the level of the zygomatic arch, and is not protected by the galea-parietotemporal fascia. Injury to the nerves prior to its innervations into the frontalis muscles will result in unilateral frontalis paralysis which leads to a flattened forehead, eyebrow ptosis and an inability to raise the eyebrow. However, the postoperative forehead paralysis does not always leave permanent sequale. The restorations for motor functions of the nerves sometimes occur without any interventions from several days to several months after the injury. This transient forehead paralysis may result from inhibitory effects for local anesthetics of motor nerve function, neural edema, or direct nerve injury during the operation. Therefore, it is important to predict clinical courses and take appropriate management of postoperative facial nerve paralysis through the understanding neuroanatomy for temporal branch of facial nerves and sophisticated surgical techniques. Herein, we report a case of transient unilateral forehead paralysis which follows Mohs` micrographical surgery for basal cell carcinoma of left temple regions where the superficial temporal branch of the facial nerve runs. Irritation fibroma, also known as traumatic fibroma or focal fibrous hyperplasia, is the most common soft tissue exophytic lesion occurring in the oral cavity. It is a reactive lesion, caused mostly by chronic trauma to the oral mucous membranes. There is no gender or racial predilection, and the most common sites for irritation fibromas are the buccal and labial mucosa and the lateral surface of the tongue. Typical lesion presents as smooth, dome-shaped or pedunculated nodule with color of surrounding mucosa. Histopathologically, irritation fibromas are characterized by their dome shape with a keratinized stratified squamous epithelium, dense bundles of collagen fibers, spindle- or fibroblast-like cells, relatively few blood vessels and inflammatory cells in the submucosa. Although dermatologist should have interest in irritation fibromas due to their higher prevalence, there have been few reports in dermatologic literatures. Herein, we report two cases of multiple irritation fibromas on the lower lip, following lip biting.

Mycobacterium massiliense is a new emerging pathogen within the M. chelonae-M. abscessus group that has been described recently. Mycobacterium massiliense usually causes pulmonary or skin and soft tissue infections in immunocompromised patients; however, cutaneous infections have also been reported in healthy individuals following surgical procedures or laparoscopic surgery. This potentially hazardous microorganism should not be overlooked because it often shows resistance to conventional antibiotics. A 25-year-old woman presented with erythematous nodules on her left leg, which was a liposuction site. A histopathological examination showed suppurative granulomas with mixed cellular infiltrations within the dermis and fat layer. Rod-shaped bacilli were detected within the clear spaces of the dermis using Ziehl-Neelsen stain. An acid-fast bacterial culture showed colony formation, and using the polymerase chain reaction-restriction fragment length polymorphism analytical method and the ERM gene, we identified M. massiliense. After surgical excision and the systemic administration of medication comprising clarithromycin and linezolid for 6 months, all of the cutaneous lesions improved and no new lesions occurred.
