The purpose of this study is economic analysis of heating and cooling systems that are using gas, geothermal energy through life cycle cost analysis. The adapted research method selected the office building in Seoul region. The building is 15 stories and have total floor area of 270,000㎡. And the capacity of heating and cooling system applied in new building is 700RT to 750RT as a scale of heating and cooling system that can provide same capacity. The Adapted LCC Analysis period on 40 years. Comparing and analyzing life cycle cost about each heating and cooling system in cases, economical evaluation is estimated. The results are as follows: 1) The total life cycle cost of gas heating and cooling system is approximately 7,979million won, the total life cycle cost of geothermal heating and cooling system is approximately 8,073million won. 2) The cost of the geothermal heating and cooling system is higher than gas heating and cooling system about 1.18%. Thus, optimal plan is the gas heating and cooling system.

<P>In the present study, we studied the neuroprotective effects of berberry extract (BE) against ischemic damage and the temporal and spatial alterations of <I>N</I>-methyl-<SMALL>D</SMALL>-aspartate receptor type 1 (NR1) and NR2A/2B immunoreactivities in the gerbil hippocampal CA1 region after transient ischemia to examine anti-ischemic effects and its role in transient forebrain ischemia. In the vehicle-treated group, the percentage of cresyl violet positive pyramidal cells in the CA1 region was about 11.4% compared to the sham-operated group 4 d after ischemic insult. BE showed neuroprotective effects against ischemic damage after ischemia-reperfusion. In the BE-treated groups, about 60—75% of CA1 pyramidal cells were stained with cresyl violet 4 d after ischemic insult. We observed the percentage of berberine (7.45+0.85 mg/g in BE) by HPLC, which is active ingredient of BE. NR1 immunoreactivity in the stratum pyramidale of the CA1 region in the vehicle-treated group was significantly increased at 30 min after transient forebrain ischemia, while at this time the NR1 immunoreactivity in the BE-treated groups was significantly low compared to the vehicle-treated group. The pattern of NR2A/B immunoreactivity in the stratum pyramidale of the BE-treated group and its protein levels were similar to that in the vehicle-treated group after ischemic insult. These results suggest that BE has potent neuroprotective effects against ischemic damage <I>via</I> the reduction of NR1 activity.</P>

Background: The aim of our study was to evaluate the "diagnosis changed" rate in patients notified as tuberculosis (TB) on the Korean TB surveillance system (KTBS). Methods: A total of 1,273 patients notified as TB cases on the KTBS in one private tertiary hospital in 2011 were enrolled in the present study. Patients were classified into three groups: "diagnosis maintained", "diagnosis changed" (initially notified as TB, but ultimately diagnosed as non-TB), and "administrative error" (notified as TB due to administrative errors). Results: Excluding 17 patients in the "administrative error" group, the "diagnosis maintained" and "diagnosis changed" groups included 1,097 (87.3%) and 159 patients (12.7%), respectively. Common causes of "diagnosis changed" were nontuberculous mycobacterial (NTM) disease (51.7%, 61/118), and pneumonia (17.8%) in cases notified as pulmonary TB, and meningitis (19.5%, 8/41) and Crohn's disease (12.2%) in cases notified as extrapulmonary TB. Being older than 35 years of age (odds ratio [OR], 2.18) and a positive acid-fast bacilli stain (OR, 1.58) were positive predictors and a TB-related radiological finding (OR, 0.42) was a negative predictor for a "diagnosis changed" result via multivariate logistic regression analysis in pulmonary TB cases. Conclusion: Because of a high "diagnosis changed" rate in TB notifications to the KTBS, the TB incidence rate measured by the KTBS may be overestimated. Considering the worldwide trend toward increased NTM disease, the "diagnosis changed" rate may increase over time. Thus, when reporting the annual TB notification rate in Korea, the exclusion of "diagnosis changed" cases is desirable.

<P>Nitric oxide (NO) is a crucial molecule in inflammatory diseases and is synthesized from L-arginine by a specific enzyme, NO synthase (NOS). The expression of inducible NOS (iNOS) is activated in macrophages by various stimuli, such as lipopolysaccharide (LPS), a wall component of gram-negative bacteria. LPS binds to toll-like receptor 4 (TLR4) on the macrophage surface and activates several downstream signaling pathways, including mitogen-activated protein kinase (MAPK) and nuclear factor (NF)-κB pathways. This study investigated whether sargaquinoic acid isolated from <I>Sargassum siliquastrum</I> might have anti-inflammatory activity and interfere with NO production in macrophages by disrupting LPS-induced signaling. This study was conducted <I>in vitro</I> using RAW264.7 murine macrophages. LPS-stimulated cells were treated with sargaquinoic acid, and the effects on NO production, iNOS expression, and involvement of the NF-κB signaling pathway were investigated by Griess assay, western blotting, and confocal microscopy. The results demonstrated that sargaquinoic acid inhibited the production of NO and the expression of the iNOS protein in LPS-stimulated RAW264.7 macrophages. Moreover, sargaquinoic acid inhibited the degradation of inhibitory-κB protein (IκB)-α and the nuclear translocation of NF-κB, a key transcription factor for the regulation of iNOS expression. Also, sargaquinoic acid influenced the phosphorylation of JNK1/2 MAPK, except ERK1/2 and p38 MAPKs, stimulated by LPS. These results suggest that sargaquinoic acid specifically prevents NO production in macrophages <I>via</I> the blockade of NF-κB activation and may thus have therapeutic applications in various inflammatory diseases.</P>

Aims: Nivolumab showed durable response and safety in patients with hepatocellular carcinoma (HCC) in the previous trials. However, real-world data of nivolumab in HCC patients, especially those with Child-Pugh class B, is lacking. We aimed to investigate the efficacy and safety of nivolumab in a real- world cohort of patients with advanced HCC. Methods: This study retrospectively evaluated 203 patients with HCC who were treated with nivolumab between July 2017 to February 2019. Radiologic evaluation was based on mRECIST. Survival outcomes were estimated by Kaplan-Meier method and Cox proportional hazard model. Logistic regression model was used to identify the predictive factors of treatment response. Results: Of 203 patients, 132 patients were within Child-Pugh class A and 71 patients were within Child-Pugh class B. Objective response rate was lower in patients with Child-Pugh class B than A (2.8% vs. 15.9%; P=0.010 by unweighted analysis and P=0.034 by weighted analysis) and Child-Pugh class was an independent predictor for objective response (Odds ratio, 0.21; 95% confidence interval; 0.05-0.93; P=0.040). Median overall survival was shorter in Child-Pugh B patients (11.3 vs. 42.9 weeks; P<0.001 by both unweighted and weighted analyses). However, other efficacy outcomes including disease control rate, time to progression, and progression-free survival were comparable between Child-Pugh A and B patients by unadjusted, adjusted, matched, and weighted analyses. There was no significant difference in terms of safety between Child-Pugh A and B patients. Conclusions: Given the limited treatment options for advanced HCC in Child-Pugh B patients, nivolumab may be a viable option despite lower response in these patients. Further studies are needed in this patient population.
Real-Time Safety Driving Assistance System Based on a Smartphone

In this paper, we propose a method which implements warning to drivers through real-time analysis of risky and unexpected driver and vehicle behavior using only a smartphone without using data from digital tachograph and vehicle internal sensors. We performed the evaluation of our system that demonstrates the effectiveness and usefulness of our method for risky and unexpected driver and vehicle behavior using three information such as vehicle speed, azimuth and GPS data which are acquired from a smartphone sensors. We confirmed the results and developed the smartphone application for validate and conducted simulation using actual driving data. This novel functionality of the smartphone application enhances drivers' situational awareness, increasing safety and effectiveness of driving.
