In addition to the problems due to spinal anomalies, patient`s physico-social life may also be affected. Here, we report a case of 13 years old female patient with Faun-tail in association with sypinal dysraphism, in which cosmetic improvement was achieved with the help of Alexandrite laser. <P>Alendronate (ALN) is a bisphosphonate drug that is widely used for the treatment of osteoporosis. Furthermore, local delivery of ALN has the potential to improve the bone regeneration. This study was designed to investigate an ALN-containing fibrin (fibrin/ALN) gel and evaluate the effect of this gel on both in vitro cellular behavior using human mesenchymal stem cells (hMSCs) and in vivo bone regenerative capacity. Fibrin hydrogels were fabricated using various ALN concentrations (10<SUP>−7</SUP>–10<SUP>−4 </SUP>M) with fibrin glue and the morphology, mechanical properties, and ALN release kinetics were characterized. Proliferation and osteogenic differentiation of and cytotoxicity in fibrin/ALN gel-embedded hMSCs were examined. In vivo bone formation was evaluated using a rabbit calvarial defect model. The fabricated fibrin/ALN gel was transparent with Young's modulus of ~13 kPa, and these properties were not affected by ALN concentration. The in vitro studies showed sustained release of ALN from the fibrin gel and revealed that hMSCs cultured in fibrin/ALN gel showed significantly increased proliferation and osteogenic differentiation. In addition, microcomputed tomography and histological analysis revealed that the newly formed bone was significantly enhanced by implantation of fibrin/ALN gel in a calvarial defect model. These results suggest that fibrin/ALN has the potential to improve bone regeneration.</P>

Background: Animal and other experimental studies have demonstrated increased block time and quality when α- and β-cyclodextrin drugs are combined with local anesthetics. However, to our knowledge, no study has utilized γ-cyclodextrins in such a combination. In the present study, we used an animal model to evaluate the effects of different doses of the combined administration of γ-cyclodextrin (sugammadex) and bupivacaine on the duration of sciatic nerve blocks in rats. Methods: Sciatic nerve blocks were performed with a 0.20 ml mixture in all groups. For the non-experimental groups, this mixture consisted of 0.2 ml saline (Sham group), 0.2 ml sugammadex (Group S), or 0.16 ml bupivacaine 0.5% and 0.04 ml saline (Group B). For the experimental groups, 0.16 ml bupivacaine 0.5% was administered along with 0.01 ml sugammadex and 0.03 ml saline (Group BS1), 0.02 ml sugammadex and 0.02 ml saline (Group BS2), or 0.04 ml sugammadex (Group BS4). Proprioception, nociception, and motor function were evaluated until the sciatic block was completely reversed.Results: Motor, proprioceptive, and nociceptive blockades occurred within 5 min in all experimental groups. In Group BS4, the duration of the motor, proprioceptive, and nociceptive blockades was significantly increased compared with the other experimental groups. However, in Groups BS1 and BS2, only the duration of the nociceptive blockade was significantly increased.Conclusions: The combined administration of sugammadex and bupivacaine for sciatic nerve blocks in rats led to a significant increase in the duration of motor, proprioceptive, and nociceptive blockades.

Surgical therapy for adenocarcinoma of the esophagogastric junction II requires distal esophagectomy, in which a transhiatal management of the lower esophagus is critical. The 'dorsal track control' (DTC) maneuver presented here facilitates the atraumatic handling of the distal esophagus, in preparation for a circular-stapled esophagojejunostomy. It is based on a ventral semicircular incision in the distal esophagus, with an intact dorsal wall for traction control of the esophagus. The maneuver facilitates the proper placement of the purse-string suture, up to its tying (around the anvil), thus minimizing the manipulation of the remaining esophagus. Furthermore, the dorsally-exposed inner wall surface of the ventrally-opened esophagus serves as a guiding chute that eases anvil insertion into the esophageal lumen. We performed this novel technique in 21 cases, enabling a safe anastomosis up to 10 cm proximal to the Z-line. No anastomotic insufficiency was observed. The DTC technique improves high transhiatal esophagojejunostomy.

The D2 dopamine receptor (DRD2) gene is the first candidate gene that has shown promise of an association with alcoholism. A correlation between alcoholism and Al and A2 alleles of this gene has been under research. In this study, in order to determine the frequencies of allele polymorphisms in 52 unrelated type Ⅱ alcoholics and in 93 unrelated controls, extraction of DNA was performed and appropriate condition of polymerase chain reaction (PCR) was established before TaqI enzyme digestion. To determine the significant difference in genotypes and allelic frequencies between the two groups, Fisher's exact test and Yates' corrected chi-square test were performed, respectively. No significant differences were found between the alcoholics and the controls in the current study in genotypes (p = 0.1600) and in the frequency of the Al allele (p = 0.7513). As a result, the data from the study do not support the hypothesis that the Al allele of the DRD2 gene has an association with alcoholism but it could have put additive effect on the determination of the racial differences in the allele frequencies of the TaIA polymorphism.

Inducing genetic and morphological variation through conventional method is very difficult. Therefore, mutation induction through in vitro technology brings numerous advantages over the conventional breeding. Thus, the individual shoots (1 ~ 2 cm) were irradiated with gamma rays (10 ~ 70 Gy). The result revealed that the explants treated with higher doses (40, 50, 60, and 70 Gy) showed deleterious effects of ionizing radiation. The highest survival rate among γ treated explants recorded was 71% in 10 Gy treatments while the lowest survivality was 15% in 70 Gy. Lethal dose 50% (LD50) dose was found to be 33 Gy. In the in vitro condition, rooting reponse showed that increase in gamma irradiation dose resulted in the inhibition of root growth. Meanwhile, non-treated explants had the best rooting ability with the maximum number of root per explant (20) within a short period of time (6 days), with the highest root length of (15.1 cm). The longer period in rooting (12 days) and lowest number of root per explant (8) with shortest root length (10.1 cm) were recorded at 30 Gy treatment. The highest shoot length (13.6 cm) was observed at control treatment and the shortest shoot length (10.4 cm) was observed at 30 Gy. In the nursery, lowest leaf number (5) was observed at 30 Gy compared with other treatments. The highest chlorophyll content (49.8) was recorded at 10 Gy treated seedling. Irradiated explants with 10 Gy found to be superior over the control treatment and had positive effects in main growth parameters such as chlorophyll content.
