Public-Private Interests and Conflicts in Ports: A Content Analysis Approach

Various levels of private sector involvement such as changes to incentives, capital utilization, flexible finance schemes, and the infrastructure investment has occurred as consequences of port devolution and reform processes. Our investigation uses a basic content analysis across both academic literature and maritime specialized media with research appropriate selected keywords to identify potential conflict within the changing institutional processes. Two major ports, New York/New Jersey (USA) and Santos (Brazil), with different characteristics in institutional background, are studied aiming to identify these conflicts. The results presented offer a contribution that enhances the academic debate and potentially enlightens solution formulation for those active in the port management.
Developing a comprehensive approach to port performance assessment

This paper investigates the port performance assessment and proposes a comprehensive framework with various perspectives as well as priority distributions of stakeholders* expectations in port business and operations. One fundamental questions in port performance studies is how to evaluate and quantify intangible indicators on performance. There are definitely inputs and outputs, which may be measured and evaluated through conventional instruments. However, each instrument deals with a particular aspect of whole performance problem (e.g. port efficiency, port productivity,), and many of these instruments are limited to ‘measurable’ data on port management and operations. In this paper, these two common problems are put under the scope, and a comprehensive port performance assessment scheme is established by gathering various perspectives. Quality function deployment (QFD) approach is preferred to formulize the problem and define priority ratings of expectations by each stakeholder in the port domain. The framework can contribute to a broader understanding of ports as business units, its development drivers and interaction with community neighbors, and to shed light to port performance assessment considering both public and private entities.

Temporomandibular disorders (TMD), recognized as the most common conditions of chronic orofacial pain, have a multifactorial etiology. Acupuncture can help to relieve the pain and discomfort associated with these conditions, because it can rebalance the energy (Qi) circulating in the meridians. The aim of the study was to verify the effectiveness of acupuncture in treating the pain; mouth opening limitation, and energy circulating in the meridians of patients with TMD of muscular or mixed origin. This was a controlled, randomized, double-blind clinical trial conducted at the Piracicaba Dental School (FOP/Unicamp), in Piracicaba SP, Brazil. The Treatment Group received acupuncture with real penetration of the needle, and the Placebo Group received a sham treatment without needle penetration. The acupoints used were: ST6, ST7, SI18, GV20, GB20, BL10, and LI4, during treatment performed for four weekly sessions. The TMD and mouth opening were evaluated according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC). The measurements of the energy at the meridians were performed by the Ryodoraku method, before and after acupuncture in all of the sessions in both groups. The results showed no decrease in pain in the Treatment Group when compared with the Placebo Group (p Z 0.2261). There was no increase in the oral opening limit in the Treatment Group compared with the Placebo Group (p > 0.05). Regarding the energy levels, after acupuncture, there was a decrease in Yang energy in all sessions (p < 0.05), in both groups, however, only real acupuncture was effective in maintaining the Yin energy average throughout the four sessions, with significant difference between groups (p Z 0.0198). In conclusion, volunteers with TMD presented a pattern of energy deficiency and the most prevalent imbalance patterns identified were in the meridians coupled to the kidney and bladder, and in the Shao Yin (heart/kidney) and Shao Yang (triple energizer/gall bladder) energetic planes. The acupuncture points used were equally effective in reducing pain in both groups; increasing the unassisted mouth opening limitation without pain in the Treatment Group, and were also effective in preserving the Yin energy in the Treatment Group. The Yang energy decreased equally in both groups.

Purpose: In this study, we used ultrasonography to monitor the use of hyaluronic acid (HA) as a filler in the face for esthetic reasons. We monitored changes in the filler shape, distribution, and relationship with adjacent anatomical structures over a 180-day period. Materials and Methods: Two patients each received an ultrasound-guided injection of HA, with different products and application sites for each patient. In 1 patient, the injection was administered in the angle of the mandible, while in the other, it was administered in the zygomatic region. The injection sites were monitored via ultrasonography at 24 hours, 30 days, and 180 days, at which times the imaging characteristics of the filler were observed. All injections were performed by the same professional, as were the ultrasound exams, which were conducted using the same equipment. Results: In both cases, the HA fillers were visualized using ultrasound at all time points. Some differences were observed between the cases in the images and the distribution of the pockets of filler. In 1 case, the filler appeared as a dark hypoechoic region with well-defined contours, and the material was observed to have moved posteriorly by the 180-day mark. In the other case, the material appeared hyperechoic relative to the previous case and presented no noticeable changes in its anteroposterior distribution over time. Conclusion: Based on these 2 cases, ultrasonography can be a complementary tool used to monitor facial fillers over the long term, allowing for the dynamic observation of different fillers.

Botryosphaeran, a water-soluble exopolysaccharide of the β-(1 → 3;1 → 6)-D-glucan type that has been isolated from the culture medium of Botryosphaeria rhodina MAMB-05 grown in submerged fermentation using glucose as the sole carbon source, was previously demonstrated to be non-genotoxic in peripheral blood and bone marrow, and exhibited strong anticlastogenic activity. In the present study, the effects of botryosphaeran were investigated in streptozotocin-induced diabetic rats as well as in high-fat diet-fed hyperlipidemic Wistar rats. The plasma glucose level was reduced by 52% in the diabetic group of rats after administration of 12 mg botryosphaeran/kg body weight of the rats (b.w.)/day by gavage over 15 days. A reduction in the median ration intake was accompanied by an increase in the median body weight gain, as well as the efficiency of food conversion. These results demonstrate that botryosphaeran has protective effects by reducing the symptoms of cachexia in Diabetes mellitus. Botryosphaeran administered by gavage at a concentration of 12 mg botryosphaeran/kg b.w./ day over 15 days also reduced the plasma levels of total cholesterol and low density lipoprotein-cholesterol by 18% and 27%, respectively, in hyperlipidemic rats. Based on these findings, we conclude that botryosphaeran possesses hypoglycemic and hypocholesterolemic properties in conditions of diabetes mellitus and hyperlipidemia, respectively, and may be used as an oral anti-diabetic agent.

Leukemia is the most common neoplastic disease of the white blood cells which is important as a pediatric malignancy. Oral manifestations occur frequently in leukemic patients and may present as initial evidence of the disease or its relapse. The symptoms include gingival enlargement and bleeding, oral ulceration, petechia, mucosal pallor, noma, trismus and oral infections. Oral lesions arise in both acute and chronic forms of all types of leukemia. These oral manifestations either may be the result of direct infiltration of leukemic cells (primary) or secondary to underlying thrombocytopenia, neutropenia, or impaired granulocyte function. Despite the fact that leukemia has long been known to be associated with oral lesions, the available literature on this topic consists mostly of case reports, without data summarizing the main oral changes for each type of leukemia. Therefore, the present review aimed at describing oral manifestations of all leukemia types and their dental management. This might be useful in early diagnosis, improving patient outcomes.
