        Representative of Cultural Heritage, Wood & Kündekâri Craftsman and State Artist of Traditional Handicrafts: Ahmet Ay

        The values that people reveal in the process of change and transformation can be defined as culture in particular and cultural heritage in general. In terms of preserving cultural assets and passing them on to future generations, the concept of "cultural heritage" is essential. Having ancient values dating back thousands of years, Anatolian geography has hosted many social groups in the socialization process. The Ahi organization, a socioeconomic order where fraternity, honesty, and benevolence are all integrated, has long guided the transmission of cultural legacy and allowed Anatolians to develop their talents in the arts and professions. The Ahi craftsmen ensured the continuation of their profession and passed on the traditional knowledge created by society to following generations with the help of the professional organization that was established under the direction of Ahi Evran. The Ahi organization has common approaches with UNESCO's Living Human Treasures Program in terms of its professional principles and methods. The main purpose of both practices is to transfer the values, philosophy, and nuances of the profession to future generations. In this sense, it is possible to assess traditional craftsmen as actors and actresses who serve society and keep the tradition alive. The identification and promotion of the local actors and actresses, who are the living carriers and developers of traditional knowledge and experience memory, will perpetuate the transfer of cultural heritage. Ahmet Ay, a woodworker and kündekâri craftsman living in Kırşehir, is a traditional connoisseur who has been practicing his art for 40 years. The aim of the study is to record the information provided by the wood and lectern craftsman Ahmet Ay, who was given the title of State Artist of Traditional Handicrafts by the Ministry of Cultureand Tourism in the province of Kırşehir, which is accepted as the organizational hub for the Ahi, and to come up with recommendations for the preservation and transfer of this profession, which has sunk into oblivion. Ahmet Ay's nomination for inclusion in the inventory of "Living Human Treasure" in 2023 is still pending.

        Real Life Data of Treat and Extend Intravitreal Ranibizumab and Aflibercept Therapy in Wet Age-related Macular Degeneration Patients: 3-Year Results

        Purpose: To compare functional and anatomic outcome of intravitreal ranibizumab (IVR) and intravitreal aflibercept (IVA) treatments in neovascular age-related macular degeneration by using the treat and extend (TE) protocol. Methods: In this retrospective chart study, treatment naïve 74 eyes of 74 age-related macular degeneration patients treated with IVR and IVA (38 eyes in IVR and 36 eyes in IVA group) with TE protocol were included. Following three consecutive monthly intravitreal injections, TE protocol was applied to each group. Patients were followed up for at least 36 months. Mean change in best-corrected visual acuity (BCVA), central macula thickness (CMT) and injection numbers over 3 years were compared. Results: Among 36 months period, the mean number of injections was 17 ± 4 for both groups (p > 0.05). In terms of CMT, there was no statistically significant difference between groups at 36 months compared to baseline. A decrease of 72.55 ±39.37 μm in CMT was detected in IVR group, whereas the decrease was 70.58 ± 33.96 μm in IVA group (p > 0.05). There was a significant increase in BCVA at 36 months of measurements. In addition, BCVA demonstrated an increase of 4.1 ± 0.44 letters in IVR group and 4.36 ± 0.67 letters in IVA group after 36 months compared to the baseline (p > 0.05). Conclusions: Both IVR and IVA injections provided significant improvements and stability in BCVA and CMT, however there was no significant difference between IVR and IVA injections with TE protocol of 36 months.

        Simple Noninvasive Scores Are Clinically Useful to Exclude, Not Predict, Advanced Fibrosis: A Study in Turkish Patients with Biopsy-Proven Nonalcoholic Fatty Liver Disease

        Background/Aims: Advanced fibrosis (F≥3) indicates poor outcomes in nonalcoholic fatty liver disease (NAFLD). Here, we examined the diagnostic performance of the fibrosis-4 index (FIB-4) and NAFLD fibrosis score (NFS) for detecting (or excluding) advanced fibrosis in patients with biopsy-proven NAFLD. Methods: The diagnostic performance of each noninvasive test according to previously identified cutoff points indicating low and high risk for advanced fibrosis was determined in 463 patients with NAFLD. Patients who scored <1.3 and >2.67 on the FIB-4 were considered at low and high risk for advanced fibrosis, respectively. Patients who scored <–1.455 and >0.676 on the NFS were considered at low and high risk for advanced fibrosis, respectively. Results: Eighty-one patients (17.5%) had biopsy-proven advanced fibrosis (F≥3). The published FIB-4 cutoff values for low and high risk were able to exclude advanced fibrosis with negative predictive values (NPVs) of 0.907 and 0.843 and specificities of 74% and 97%, respectively. The published NFS cutoff values for low and high risk were able to exclude advanced fibrosis with NPVs of 0.913 and 0.842 and specificities of 63% and 96%, respectively. If biopsies were performed in only patients with a FIB-4 above the low cutoff point (≥1.3), 67.1% could be avoided. Conversely, if biopsies were performed in only patients with an NFS above the low cutoff point (≥–1.455), 57.0% could be avoided. Conclusions: The main clinical utility of the FIB-4 and NFS in patients with NAFLD lies in the ability to exclude, not identify, advanced fibrosis.

        A Fuzzy TOPSIS Approach Based on Trapezoidal Numbers to Material Selection Problem

        Material selection is a complex problem in the design and development of products for diverse engineering applications. This paper is aimed to present a fuzzy decision‐making approach to deal with the material selection in engineering design problems. A fuzzy multi criteria decision‐making model is proposed for solving the material selection problem. The proposed model makes use of fuzzy TOPSIS (Technique for Order reference by Similarity to Ideal Solution) with trapezoidal numbers for evaluating the criteria and ranking the alternatives. And result is compared with fuzzy VIKOR (VlseKriterijumska Optimizacija I Kompromisno Resenje in Serbian, means Multi criteria Optimisation and Compromise Solution) which is proposed by Jeya Girubha and Vinodh [2012]. The present paper is aimed to also improve literature of fuzzy decision making for material selection problem.

        Hybrid Model Approach to the Complexity of Stock Trading Decisions in Turkey

        The aim of this paper is to predict the Borsa Istanbul (BIST) 30 index movements to determine the most accurate buy and sell decisions using the methods of Artificial Neural Networks (ANN) and Genetic Algorithm (GA). We combined these two methods to obtain a hybrid intelligence method, which we apply. In the financial markets, over 100 technical indicators can be used. However, several of them are preferred by analysts. In this study, we employed nine of these technical indicators. They are moving average convergence divergence (MACD), relative strength index (RSI), commodity channel index (CCI), momentum, directional movement index (DMI), stochastic oscillator, on-balance volume (OBV), average directional movement index (ADX), and simple moving averages (3-day moving average, 5-day moving average, 10-day moving average, 14-day moving average, 20-day moving average, 22-day moving average, 50-day moving average, 100-day moving average, 200-day moving average). In this regard, we combined these two techniques and obtained a hybrid intelligence method. By applying this hybrid model to each of these indicators, we forecast the movements of the Borsa Istanbul (BIST) 30 index. The experimental result indicates that our best proposed hybrid model has a successful forecast rate of 75%, which is higher than the single ANN or GA forecasting models.

        Investigation of bacteremia after toothbrushing in orthodontic patients

        Objective: The aim of this study was to investigate the occurrence rate of bacteremia following toothbrushing with toothpastes composed of several antibacterial agents and compare the results with the conventional oral hygiene maintaining methods in orthodontic patients. Methods: This clinical study included 100 adult orthodontic patients who were divided into 4 groups. Each group comprised of 25 patients, wearing fixed orthodontic appliances. In the first group, bacteremia was assessed after toothbrushing without using any toothpaste. In the second group, a 0.2% chlorhexidine gluconate mouthrinse was used before brushing with no toothpaste. In the third group, subjects brushed with a commonly used toothpaste which did not include an additional antimicrobial agent. The fourth experimental group used toothpaste which included tea tree oil, clove oil, peppermint oil and bisabolol as antimicrobial elements. Pre- and post-brushing blood samples were obtained using a strict aseptic technique. All samples were microbiologically evaluated using blood culture bottles. Results: Toothbrushing in orthodontic patients yielded to an increase in the occurrence rate of bacteremia when using normal toothpaste or no toothpaste at all. Conclusions: The use of chlorhexidine mouthwash before toothbrushing, and brushing with antimicrobial toothpaste did not show a statistically significant difference in preventing the occurrence of bacteremia (p > 0.05).

      • Effect of Pre-Procedural State-Trait Anxiety on Pain Perception and Discomfort in Women Undergoing Colposcopy for Cervical Cytological Abnormalities

        Background: Colposcopy is the gold standard procedure for evaluating cervical cytological abnormalities. Although it is essentially a minimally invasive intervention, referral for colposcopy may cause significant distress on patients. In this study, we aimed to determine if pre-procedural anxiety levels have a significant association with procedure related pain and discomfort in women undergoing colposcopy for evaluation of abnormal cervical cytology. We also assessed the impact of various clinical factors on anxiety, pain and discomfort in these patients. Materials and Methods: This prospective study was performed at the gynecologic oncology department of Zekai Tahir Burak Women's Health Education and Research Hospital in Ankara, Turkey between January and June 2013. After taking informed consent, State-Trait Anxiety Inventory (STAI) form and a 14-item questionnaire were filled for women who were admitted to our outpatient colposcopy unit for evaluation of abnormal cervical cytology. STAI scores were calculated for each participant. Immediately after the procedure, visual analog scale (VAS) scores for procedure-related pain and discomfort were obtained. Associations between STAI and VAS scores were investigated using correlation analyses. The effect of various contributing factors on anxiety, pain and discomfort were evaluated with linear regression analysis. The p values less than 0.05 were considered statistically significant. Results: A total of 222 women met the inclusion criteria within the study period. Mean patient age was $38.5{\pm}9.6$. Median state and trait anxiety scores were 47 and 46, respectively. Median VAS scores for pain and discomfort were 4 for both variables. State anxiety had a significant correlation with procedure related discomfort (p=0.02). Colposcopy related pain VAS scores were significantly affected by state anxiety level, marital status and prior gynecological examination (p<0.05). Colposcopy related discomfort VAS scores were significantly affected by state anxiety level, marital status, prior gynecological examination and educational status. Conclusions: Additional measures should be implemented in women that carry higher risk for experiencing pain and discomfort. Social, cultural and lifestyle issues may also affect women's experiences during colposcopy, therefore further studies are needed to define specific determining factors in various populations.

        Anti-inflammatory properties of samidin from Seseli resinosum through suppression of NF-κB and AP-1-mediated-genes in LPS-stimulated RAW 264.7 cells.

        <P>Seseli is a herb widely used for its anti-inflammation, anti-flatulence and various other healing properties. In the present study, we investigated the effects of samidin on the production of pro-inflammatory mediators in lipopolysaccharide (LPS)-stimulated RAW 264.7 cells. The results demonstrated that samidin significantly inhibited the production of nitric oxide, as well as the gene expression levels of inducible nitric oxide synthase and cyclooxygenase-2. The results from an electrophoretic mobility shift assay illustrated that samidin significantly suppressed NF-κB and AP-1 DNA-binding affinity. In addition, both the NF-κB subunit p65 and the AP-1-related c-jun were markedly inhibited by samidin. The time course experiment demonstrated that samidin showed significant inhibitory effect on p38 and JNK activation. Furthermore, tumor necrosis factor-α mRNA level were remarkably down-regulated by samidin in LPS-stimulated macrophages based on quantitative-real-time polymerase chain reaction. Our results suggested that samidin has a potential to be developed as a therapeutic agent for various inflammatory diseases.</P>
