Eccrine poroma is a benign neoplasm that originates from an eccrine sweat duct. Although it is most commonly found on the soles of the feet, it has also been unusually observed on many other areas of the skin, such as the nose, neck, chest, and scalp. We report two cases of eccrine poroma occurring on the scalp, which is an unusual location. (Korean J Dermatol 2011;49(8):768∼771)
Differential diagnosis and pathophysiology of red face: Focusing on the skin barrier

Facial erythema (the red face) is a straightforward clinical finding; however, a red face does not represent a single cutaneous entity. In other words, in clinical practice, dermatologists do not encounter only one type of facial erythema but rather a number of different shades of red. Diagnosis is based on different data: date and mode of appearance, characteristics of the erythema, functional signs, and associated systemic manifestations. A case of red face can have an infectious origin, caused by vascular, congenital, or acquired lesions, or be caused by photodermatosis, or be the main location of inflammatory dermatosis or collagenosis, but depending on the clinical context, many other diagnoses can be suggested. This lecture presents the clinical spectrum of facial erythemas and addresses the question of what lies beneath a red face by discussing the key pathophysiologic characteristics focusing on skin barrier.

Nodular hidradenocarcinoma was first reported as a clear-cell papillary carcinoma by Liu et al. in 1949. With a potential for uncontrollable local recurrence, it tends to metastasize and often causes death. We report a case of a 68-year-old man with nodular hidradenocarcinoma on the left preauricular area. Histopathology of the tumor showed a multilobulated intradermal mass. In the tumor nodule, there were many cystic spaces of various sizes, resulting from tumor necrosis. The tumor consisted of large, atypical, polygonal cells with eosinophilic cytoplasm, and clear cells with clear cytoplasm. Numerous mitoses were evident. We obtained positive results from an immunohistochemical study with antibodies to cytokeratins, epithelial membrane antigen, and PAS. The patient was treated with a wide local excision. However, we observed a recurrence one month later and performed a wide excision and local skin flap. There has been no evidence of recurrence or metastasis for six months after the second excision. Background: A previous study reported that calcineurin inhibition by cyclosporin A (CsA) showed tumor-enhancing effects through the induction of the ATF3 transcription factor and the associated suppression of p53. The development and aggressiveness of cutaneous squamous cell carcinoma (SCC) may be determined by cancer stem cell populations, which have self-renewing potential. Objective: To determine the role of ATF3 and calcineurin inhibition in the proliferation of SCC and evaluate the existence of putative SCC stem cells. Methods: We performed real-time PCR, fluorescence activated cell sorting, and clonogenicity assays in SCC13 cells under conditions of calcineurin inhibition by CsA or ATF3 and p53 overexpression. The relationships amongstcalcineurin inhibition, p53, and ATF3 were demonstrated by western blot analysis and transient transfection assays in SCC13 cells. Results: In putative stem cell populations of SCC13 cells enriched in self-renewal potential, p53 expression was lower than that in differentiated SCC13 cells. CsA treatment or ATF3 overexpression caused an expansion of stem cell populations. Additionally, p53 overexpression inhibited cellular proliferation and reduced clonogenicity in SCC13 cells. CsA treatment led to a decrease in p53 expression and an increase in ATF3 in SCC13 cells on western blots. SCC13 cells with CsA and small interfering RNA against ATF3 demonstrated lower cell viability than SCC13 cells with CsA only and SCC13 cells with CsA and small interfering control RNA after 14 days. Conclusion: Putative cancer stem cell populations and differentiated cell populations in SCCs are positively regulated by ATF3 and p53, respectively. (Korean J Dermatol 2014;52(10):692∼700)
