즐례 : 전신경화증 환자에서 발생한 동기능부전증후군 1예

전신경화증은 심장 조직에도 침범할 수 있으며 그 결과 심낭삼출, 심근허혈(myocardial ischemia), 심근비대(myocardial hypertrophy), 심부전(heart failure), 판막부전(valvular impairment), 부정맥 및 전도장애 등을 일으킬 수 있다. 그 중에서 도 부정맥과 전도장애는 매우 드물며 예후가 좋지 않은 것으로 추정된다. 본 환자의 증례에서와 같이 전신경화증과 동반된 동기능부전증후군은 문헌고찰 결과 유일하게 국외에 1증례[4]가 있을 뿐이었으며 아직까지 국내 보고는 없었다. 이에 저자들은 제한형 경피증에 동반된 동기능부전증후군 환자에서 아미노필린(aminophylline)과 테오필린(theophylline) 투여로 정상동리듬을 회복한 성공적인 치료를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Systemic sclerosis (SSc) is characterized by the presence of microvascular disease and various patterns of cutaneous and parenchymal fibrosis. Manifestation of SSc may occur in numerous tissues and organs and can be particularly problematic when present in the lungs, kidneys or heart. Cardiac involvement in SSc includes pericarditis, myocardial disease, conduction abnormalities, and arrhythmia. Sick sinus syndrome is described as a combination of symptoms (dizziness, confusion, fatigue, syncope and congestive heart failure) caused by sinus node dysfunction and manifested by marked sinus bradycardia, sinoatrial block, or sinus arrest. Sinus node dysfunction is most often seen in the elderly but also, rarely, in systemic amyloidosis and connective tissue disease. Sick sinus syndrome is rarely found in patients with SSc, of which there has been only one case report, according to a review of the literature. Therefore, we report a case of sick sinus syndrome in a 71-year-old female with SSc. (Korean J Med 2014;86:110-115)
