        위정맥류 출혈 환자에서의 역행성경정맥위정맥류폐색술

        목적: 위정맥류 출혈환자에서 풍선 카테타를 이용한 역행성경정맥위정맥류폐색술의 기술적 용이성과 임상적 유용성에 대해 알아보고자 하였다. 대상과 방법: 2001년 9월부터 2002년 5월까지 위신정맥 단락과 위정맥류 출혈을 보이는 10명의 환자를 대상으로 역행성경정맥위정맥류폐색술을 시행하였다. 남자가 7명, 여자가 3명이었으며 평균 연령은 52세(40-77)였다. 3명의 환자는 간성 뇌병증을 보였으며 2명의 환자는 경경정맥간내문맥정맥단락술 경로에 간암이 있었다. 위신정맥 단락과 간문맥 혈전증 유무를 평가하기 위해 모든 환자에서 시술 전에 전산화 단층촬영을 시행하였다. 시술 방법은 5-F 폐색풍선 카테타를 우측 목정맥 경로를 통해 좌부신정맥에 위치시킨 후 풍선을 부풀려서 좌부신정맥을 막은 상태에서 정맥 조영술을 시행한 다음에 경화제인 5%ethanolamine oleate-lipiodol 혼합액을 위정맥류 내에 주입하였다. 정맥 조영술에서 측부 혈관이 보이는 4예에서는 코일로 색전한 다음 경화제를 주입하였다.1-4주 후 3예에서 전산화 단층촬영을, 8예에서는 내시경을 시행하여 치료효과를 평가하였다. 모든 환자에서 간 기능 수치를 퇴원할 때까지 매일 점검하였다. 결과: 10명 중 9명에서 성공적으로 시술을 하였으며 실패한 1예는 시술 도중 풍선이 터져 경경정맥간내문맥정맥단락술을 시행하였다. 시술에 성공한 모든 환자에서 추적 내시경과 컴퓨터단층촬영에서 출혈이 멈추었으며 정맥류가 폐색 되었다. 간 기능은 8명의 환자에서 시술 첫 3일 동안은 약간 악화되었으나 1주일 후 호전되었다. 1예는 담관을 침범한 침윤성 간암으로 인해 간 기능이 악화되어 시술 3주 후 패혈증으로 사망하였다. 간성 뇌병증을 보인 3명의 환자는 시술 후 증상이 호전되었다. 결론: 증례가 적고 장기 추적관찰이 미비한 문제점을 가지고 있지만 위정맥류 출혈 환자와 간성 뇌병증 환자에서 역행성경정맥위정맥류폐색술은 기술적으로 용이하며 임상적으로 효과적인 시술이라 할 수 있다. Purpose: To evaluate the technical feasibility and clinical efficacy of balloon-occluded retrograde transvenous obliteration (BRTO) in the treatment of gastric variceal bleeding. Materials and Methods: Between September 2001 and March 2002, ten patients with gastric variceal bleeding and gastrorenal shunt, underwent BRTO. Three of the ten also had hepatic encephalopathy. To evaluate the gastrorenal shunt and exclude portal vein thrombosis, all patients underwent pre-procedural CT scanning. An occlusion balloon catheter was inserted from the right internal jugular vein and on ballooning was wedged into the left adrenal vein. A sclerosing agent (5% ethanolamine oleate-lipiodol mixture) was injected until the varices were completely filled. In four patients, the collateral veins seen at balloon-occluded adrenal venography were embolized with coils prior to sclerotherapy. Post-procedural follow-up CT (n=3) or endoscopy (n=8) was performed 1-4 weeks later, and both before and after the procedure, hepatic function was also monitored. Results: Treatment was successful in nine cases: the failure involed rupture of the occlusion balloon during inflation, and a transjugular intrahepatic portosystemic shunt was performed. The cessation of bleeding was confirmed endoscopically or clinically; in three patients, follow-up CT showed complete obliteration of the varices. Hepatic function improved in eight patients, but three weeks after the procedure, one expired due to progressive infiltrative hepatoma. The clinical symptoms of the three patients with hepatic encephalopathy showed remarkable improvement. Conclusion: Although more extensive studies and long-term follow up are needed to overcome the limitations of our study, we believe that BRTO is a technically feasible and clinically effective treatment for gastric varices and hepatic encephalopathy.

        예이츠의 신화적 공간과 영토적 정체성

        The purpose of this study is to explore mythological place and territorial identity in Ireland in Yeats' poems and proses. Territorial nationalism is the desire to revive spiritually to its own territorial rights. Irish people who opposed British imperialism have tried to find true Irishness. As a result, they strengthened the meanings of place, thrived to revive Gaelic and recognized the plurality of identities between England and Ireland. At first, Yeats regards the undeveloped and primitive ground in Ireland as divine ground, or the place of soul. His territorial consciousness of Ireland is instinctive, unconscious and spiritual, so it surpasses the industrial society of England. The mythological hero that Yeats created is also related to Ireland's territorial dignity. Druidism is the religion that had been practised by the Irish native monks called Druids, and is used in Yeats' mythological imagery. Yeats thinks of the divine ground as Arcady, which pursues its ideal, ultimate and romantic character. He may think that Ireland was the beauty of unattainable perfection. We can know this attitude through the first and second “Irish Literary Movement,” that Yeats participated in. Those movements resisted the British-centered discourses in terms of culture in literature. Therefore, Yeats applied to the territorial identity in his poetry to make Irish nationalism come true. In addition, his interpretation and attitude toward the mythology in his poetry represented both romantic and idealistic Irish nationalism.

        TV 예능 프로그램에 나타난 다문화주의 엿보기 - 『미녀들의 수다』와 『무한도전』을 중심으로

        This study is to clarify the background and characteristic of multiculturalism and to indicate its limits and scopes, so to grope for possibilities in many policies and cultures. Multiculturalism is the acceptance or promotion of multiple ethnic cultures, applied to the demographic make-up of a specific place. We should tear down the boundary of fixed identity that the self and the other are separated by enhancing the understanding for diverse cultures. Moreover, multiculturalism is sure to consider the responsibility that it should redistribute the economic, social and political resources to the excluded and othernized groups. Therefore, it is not to plan for cultural variety show of many ethnic minorities but to be one of social practices as the preliminary action eliminating the structural contradiction in society. In the Korean popular TV shows, Chattering Beauties and Infinite Challenge, the effects in multiculturalism have been revealed. Chattering Beauties is the program that various foreign women discuss Korean culture one another, doing it several times, it has passed different from its original purpose, so experienced the controversy and criticism of public, like “loser girl.” Infinite Challenge, on the other hand, showed an effort that would accept and reflect the criticism. In the “New York Special” or “Boxing Special”, it was absolutely different from the previous typical shows. Trying to listen and apply to the opinions of minor viewers, and to show process and efforts of people as they are rather than the outcome of the game, are considered as the desirable attitudes in multiculturalism.

        「이블린」에 나타난 에피파니와 상징 그리고 마비

        The main point of concentration in the paper is to define the notion of paralysis in terms of symbolism and narratives in the short story “Eveline” in Dubliners, and to show how different symbols and different voices draw upon the desired theme of the author; how the notions such as religion, language and nationality are packed into variant symbols in order to enhance their significant function in issue of paralysis and how the various methods of narration can depict the nature of paralysis with which the characters struggle. The dilemma of his plots revolves around at least one of notions. Joyce believes that for a man to seek and reach the true nature of freedom in his life, it is necessary to leave these boundaries behind. Usually in most cases, most characters in Joyce's writings are captive by those frame. They are put in a dramatic situation in which a revelation would lead a protagonist to an epiphany. Joyce's use of symbolism and realism and also his different layers of narration is what endow significance, life and appeal to the simple plots of his stories. As a keen reader, we can find that it is important not to trace the cause for Eveline's decision not leaving Dublin, but to understand Eveline's psychological process, her memory in childhood and her pity and commitment for her parents in the stages-of-life.

        히니의 초기 시에 나타난 영토적 정체성

        The purpose of this essay is to explore the early poetry of Seamus Heaney from the perspective of the mythology and territorial consciousness as it appears in his poetry and prose. Territorial nationalism is the desire to revive spiritually to its own territorial rights. Heaney shows the realistic aspects of Irish identity as people relate to their tradition of territorial worship with their reality. Although common people in Ireland have suffered harshly from British domination, Heaney expresses their lives as proud and independent characters with a positive vision. Moreover, Bogland has an image of colonial experience and archeological quality and revival. Heaney transforms it into a imagery of place symbolizing feminine receptivity. Heaney’s pursuit of identity is a little fateful, generally based on the recognition of realistic lives.

        원형적 상상력으로써의 시적 글쓰기

        The purpose of this study is to investigate Seamus Heaney's texts in terms of material and archetypical images of Gaston Bachelard's poetics. Jungian terms, anima and animus can be discovered in Heaney's poems. Anima is represented in repose of the feminine, gentleness, slowness and peace. Animus relates to adjectives such as active, clairvoyant, prudent, audacious and complex. In his poems, historical facts and political situations include the attributes of anima while usual space and repetition include the attributes of animus. Furthermore, each lively image and calm image can be extracted from material poetic objects. Bachelard's intimacy that concentrates on an images itself, his dynamism that makes poetical imagination combinate opposed images as well as his coexistence that happens to interact and interconnect with images, are considered remarkable in Heaney's early poems. “Dialectics of outside and inside” and “Every being seems in itself round” stated by Bachelard, can be connected with Heaney's poetic images. It is obvious that Heaney projects his poetic space onto the embodiments of material imagination and phenomenological images.

        「어셔 가의 몰락」의 이율배반과 근원으로의 회귀

        This paper is to analyze “The Fall of the Usher” by E. A. Poe with characteristics of Gothic architecture and Gnostic philosophy. In the novel, the images of dungeon, nooks, archway, vaults, labyrinthine or winding stairs with their darkness remind us the conception of fear, anxiety and death. The suffering, confinement, physical collapse of the House and falling of Roderick and Madeline Usher are considered to belong to Gothic elements. The novel reveals biblical and mythical materials implicitly, but Poe do not aim at Christian redemption. It is possible for him to be affected by gnosticism including alchemy and mysticism. For Poe, the immortal destiny dwells in the house before all beings return to God of Abyss or Death. The dragon and evil in the dark Gothic building would test the soul of men. While Roderick's anxiety and Madeline's illness are painful, their death means the end of suffering and the start of liberation. His burying her alive and facing their death together are regarded as the attempt of returning the pure and perfect state in heaven before they exist on earth. Poe's writing process is associated with the escape from the evil, mystic experience of Gnostic philosophy, the disappearance of Matter and ultimate unity with divinity through spiritual awareness.
