        Objectives: The purpose of this study was to evaluate the incidence and predictive factors of contralateral occult lymph node metastasis in oral cavity squamous cell carcinomas to form a rational basis for elective contralateral neck management. Materials and Methods: We performed a retrospective analysis of 66 N0-2 oral cavity cancer patients undergoing elective neck dissection for contralateral clinically negative necks from 1991 to 2003. Results: Clinically negative but pathologically positive contralateral lymph nodes occurred in 11%(7 of 66) . Of the 11 cases with a clinically ipsilateral node positive neck, contralateral occult lymph node metastases developed in 36%(4 of 10, in contrast with 5%(3/55) in the cases with clinically ipsilateral node negative necks(p<0.05). Based on the clinical staging of the tumor, 8%(3 of 37) of the cases showed lymph node metastases in T2 tumors, 25%(2 of 8) in T3, and 18%(2 of 11) in T4. None of the T1 tumors(10 cases) had pathologically positive lymph nodes. The rate of contralateral occult neck metastasis was significantly higher in advanced stage cases and those crossing the midline, compared to early stage or unilateral lesions(p<0.05). Patients with no evidence of contralateral nodal cancer had significantly improved disease-specific survival over patients with any pathologically positive nodes(5-year disease-specific survival rate was 79% vs. 43%, p<0.05). Conclusion: The risk of contralateral occult neck involvement in the oral cavity squamous cell carcinomas above the T3 stage or those crossing the midline with unilateral metastases was high. Therefore, we advocate an elective contralateral neck treatment with surgery or radiotherapy in oral cavity squamous cell carcinoma patients with ipsilateral node metastases or tumors that are greater than stage T3 or crossing the midline.

        조명의 분광분포 형태 따른 칼라 어피어런스 및 색 감성 변화 연구

        본 논문에서는 조명의 분광분포의 형태가 칼라 어피어런스와 색 감성에 미치는 영향에 대해 기존 색감성 모델을 이용하여 분석 하였다. 5종류의 LED 분광분포를 조합하여 구성된 4 수준의 색온도를 갖는 20개의 LED 조명을 구성하였으며, 23개의 테스트 샘플을 선정하였다. 각 LED 광원의 연색성은 CRI-CAM02UCS를 이용하여 평가하였고 물체색의 칼라 어피어런스는 CAM02UCS를 통해 도출하였다. 기존의 색감성 예측 모델인 Ou, Sato, Xin-Cheng모델을 이용하여 ‘능동적인-수동적인’, ‘무거운-가벼운’, ‘단단한-부드러운’, ‘따뜻한-시원한’ 색감성 스케일의 변화량을 계산하였다. 데이터 분석 결과, Daylight 조명에 비해 LED 조명에서 red-green 색들은 채도의 증가가 가장 두드러지게 나타났고, 이는 테스트 샘플들의 감성을 더욱 ‘능동적이고’, ‘따뜻한’ 색은 더 따듯하게, ‘시원한’ 색은 더 시원하게 느껴지도록 하는 것으로 나타났다. 이 결과는 LED 조명의 연색 특성을 적절히 조절하여 공간의 색감성을 특정한 방향으로 변화시킬 수 있는 것을 의미한다. 향후 본 논문에서 제시된 시뮬레이션 결과에 대해 실제 조명 및 피험자를 이용한 감성 실험을 통한 검증이 요구된다. In this research, effects of the spectral power distribution of the LED light on color appearance and color emotion was investigated. 20 LED illuminants with 4 different color temperatures were simulated by combining spectral power distributions of 5 different color LEDs. 23 color patches were selcted as test colors. The color rendering of the test illuminants were evaluated using the CRI-CAM02UCS and color appearance of each test sample was calculated using CAM02UCS. Existing color emotion models proposed by Ou, Sato, Xin-Cheng were used for calculating 4 color emotion scales; ``active-passive``, ``heavy-light``, ``hard-soft`` and ``warm-cool``. The experimental result showed that chroma of red-green colors was increased under LED illuminant compared to that under daylight. Such an color appearance change affected color emotion that color evoked more ``active`` feeling and warm color become ``warmer`` and cool color become cooler under LED illuminants. This experimental result suggests that LED lights can be used to induce specific color emotion. Further study is needed using real LED lights and human observers to prove this simulation result.

        Osseous choristoma is a rare, benign proliferative osseous lesion, which is defined as the growth of normal tissue in an abnormal location. The etiopathogenesis for its formation is unknown, but various hypotheses have been proposed. Treatment of choice is en-bloc resection, and no recurrence has been reported. Here, we report the two cases of osseous choristoma, presented with a mass on the base of the tongue with/without globus symptom and were treated with surgical excision.

        사례 적용 Praat 기반 CSL 대체 자동화 음성분석 프로그램

        There are a number of voice analysis programs around the world. Domestic voice analysis is performed by relying heavily on specific commercial program. We intend to develop coding for voice analysis using Praat and apply it to clinical practice. This study consisted of Experiment 1 and Experiment 2. Experiment 1 was the development of automated voice analysis coding based on Praat. The coding was largely divided into a recording, an analysis, and a storage section. Experiment 2 was applied to the voice analysis of 2 male patients pre- and post-operation with this coding. The analysis parameters of this coding provided 26 parameters for vowel /a/, nine parameters for sentence analysis, and a total of 4 parameters for voice range profile analysis. In two male patients, the pitch and the intensity increased, the voice quality improved, and the sentence length decreased after surgery. The coding was well made, so the output was good in real time. The code is automated as much as possible to block manual errors and increases convenience and efficiency by generating the result sheet in real time.

        Cystadenocarcinoma of salivary gland is an rare, slow-growing, low-grade malignant neoplasm characterized by cysts and papillary endocystic projections. Major locations of this tumor are parotid glands, and minor salivary glands, while occurrence in submandibular gland is extremely rare. Only few cases have been reported in the literatures. Cystadenicarcinoma behaves in an indolent manner and recurrence is rarely occured, surgical complete excision is considered treatment of choice. We experienced a case of cystadenocarcinoma arising from submandibular gland, so we report this case with a review of literature.

        음성장애환자에게서의 우울감 연구

        The objectives of this study are to research the frequency of depression symptom in patients with voice disorders and to investigate parameters associated with depression from voice evaluation. A hundred ninety six patients(106 males and 90 females) who had been diagnosed with voice disorders first in their lifetime were selected. All the patients were examined by laryngeal stroboscopy. For depression and voice study, personal interview, acoustic and aerodynamic analysis, voice handicap index(VHI), reflux symptom index(RSI), and beck depression index(BDI) were done respectively. Mild to severe BDI were seen in 26.2%(52 patients) of the whole patients. A BDI mean score of female patients was 8.8±7.5 which was higher than that of male patients(5.6±6.6), the difference observed being statistically significant(p<0.001). In the acoustic analysis, the score of sent_duration parameter was increasing in the patients with depression, which was significantly higher than the score of the patients without depression(p<0.05). In the addition, the scores of VHI and RSI were higher in the patients with depression(p<0.001). Our findings suggest that the prevalence of depression in patients with voice disorders is related to female, speaking velocity, and self-questionnaire. This result can be used for psychologically based approach to therapy.

        갑상선 수술범위에 따른 음성의 음향적 분석

        Voice complication without the laryngeal nerve injury can occur after thyroidectomy. The purpose of this study is to investigate voice changes according to extent of thyroidectomy with acoustic analysis. Thirty-five female patients with papillary thyroid carcinoma took voice evaluation at before and 1 month, and 3 months after thyroidectomy. Acoustic analysis parameters were speaking fundamental frequency(SFF), min F0, max F0, dynamic range F0, jitter, shimmer, noise-to-harmonic ratio(NHR), and Cepstral prominence peak(CPP). Repeated-measured analysis of variance was applied. Time-related voice changes showed significant differences in all parameters except NHR. At 1 month after surgery, voice quality was worse and pitch was decreasing, but voice quality and pitch were improving at 3-month follow-up. Voice changes according to the extent of surgery were in SFF, max F0, and dynamic range F0. Time by surgery-related voice change existed only in min F0. The result showed that the severity of voice complication depended on the extend of thyroidectomy which had a negative impact on F0-related parameters. The deterioration of voice quality at 1 month after thyroidectomy may be affected by the loss of thyroid hormone in the blood. The descent of F0-related parameters may be impacted by laryngeal fixation of surgical site adhesion.

      • 무인주행차량을 위한 비포장 도록추적

        무인 주행 차량에 있어서, 포장 또는 비포장 도로의 시각적 추적은 매우 중요한 문제중의 하나이다. 따라서, 비디오 이미지로부터 비포장 도로를 추적할 수 있는 신속한 비젼 알고리즘의 개발이 필요하다. 이 논문에서는 칼만 필터와 EM(Expectation Maximization) 이론을 이용해 토로를 예측하고 시스템 파라미터를 갱신하는 방법을 제시한다. 시스템 파라미터, 도로 state, 도로 경계선, 그리고 모든 과거 데이터들을 각각 EM 파라미터, hidden data, incomplete data와 complete data로 정의함으로써 도록 state를 예측하고 시스템 파라미터를 추정할 수 있는 시간 회귀적 수식을 유도해낼 수 있다. 이러한 방법을 이용하여 도록 state는 칼만 필터에 의해 매 프레임마다 예측되며, 시스템 파라미터들은 주기적으로 갱신되는 것이다. 결과적으로 이 방법은 주변환경과 날씨에 많은 영향을 받는 도로의 모양과 특징을 잘 찾아낼 수 있다. 또한 도로의 다음 state를 예측할 수 있는 점을 이용하면 계산량을 줄일 수 있으므로 실시간 구현에 용이하다. 이와 같은 방법으로 우리는 0.1sec/frame 처리속도를 보장하는 도록추적 시스템을 구현하였다.

      • 조영증강 전산화단층촬영검사에서 보이지 않는 이하선의 다형선종

        Pleomorphic adenomas is the most common tumor of parotid gland and usually located and confined in superficial lobe of parotid gland. Computed tomography (CT) is commonly used to initially evaluate salivary gland lesion, but contrast-enhanced CT may sometimes fail to reveal lesions in spite of a high clinical suspicion. For this reason, ultrasonography (US) can be used as the first-line image work-up in some cases of parotid gland benign tumors. We experienced a case of a 60-year-old woman without underlying disease presenting a palpable parotid mass in which the initial CT examination was reported as ‘no obvious mass detected’. However, the lesion was revealed in US and histologically confirmed as pleomorphic adenoma. The patient underwent superficial parotidectomy through modified facelift incision. To the best of our knowledge, this is the first report of invisible major salivary gland tumor on CT in the Korean literature.

        파킨슨병 환자 대상 조음교대운동의 음향적 분석

        The acoustic analysis of diadochokinesis(DDK) has been used to evaluate dysarthria. However, there has not been an automatic method to evaluate dysarthria. The aim of this study was to introduce a new automated program to measure DDK tasks and to apply this to clinical patients with idiopathic Parkinson"s disease(IPD). Fourty-seven patients with IPD and a healthy control group of twenty participants were selected with every DDK task recorded three times. Twenty-five acoustic parameters in the program were developed. The relevant parameters were times of DDK, pitch related parameters, intensity parameters which were analyzed by 2-way ANOVA. Significant differences between the groups were found in the times of DDK, pitch related parameters, and intensity parameters. The findings indicated that the pitch of control group was more stable than that of the IPD. Even though the patients with IPD had a higher intensity value, this phenomenon was caused by the weakness of the IPD group who could not control their speech with a breath.
