하지절단자의 의지보행훈련 전·후 동적 균형 및 보행 능력에 대한 정량적 평가

본 연구는 재활공학연구소 통합재활프로그램에 참여한 하지절단자 16명을 대상으로 의지보행훈련 전·후 동적 균형 및 보행 능력의 변화를 비교분석하였다. 의지보행 훈련내용은 근력강화, 체중지지, 의지조절훈련, 균형능력강화, 체간안정화, 일상생활 강화훈련으로 구성되었다. 동적 균형능력은 functional reach test, L-test, 4 SS test를 실시하였고, 보행능력은 보행속도, 보폭과 보장비율을 측정하였다. 연구결과, 의지보행 훈련 후 functional reach test에서 이동거리가 평균 9.9% 증가하였으며(p<.01), L-test와 4 SS test에서도 수행시간이 22.6%와 29.1% 각각 단축되었다. 또한 보행속도가 27.6% 빨라진 반면(p<.01), 보폭과 보장비율의 유의한 변화는 없었다. 본 연구를 통해 하지절단자의 의지보행훈련 후 동적 균형 및 보행능력의 향상을 정량적으로 확인하였으며, 향후 절단자 재활훈련 프로그램 개선을 위한 기초자료로써 활용하고자 한다. The purpose of this study was to analyze the changes in dynamic balance and walking ability before and after prosthetic gait training in 16 lower limb amputees participating in the integrated rehabilitation program. The contents of the prosthetic gait training consist of muscle strengthening, weight support, strengthening of balance, stabilization of the trunk, and functional gait training. Dynamic balance was measured to functional reach test, L-test, 4-SS test, and walking ability was evaluated by the walking velocity, step width and step length ratio. After prosthetic gait training, the mean moving distance of the functional reach test increased 9.9% (p<.01), and also the execution time of L-test and 4-SS test was shortened by 22.6% and 29.1%, respectively. Walking speed was also 27.6% faster (p<.01), but there was no significant change in step width and step length ratio. This study quantitatively confirmed the improvement of dynamic balance and walking ability after prosthetic gait training in people with lower limb amputation. Our results will be used as the basic data for improvement and correction of rehabilitation training program with amputation.
척수손상 장애인 대상 휴식기 신진대사량과 휠체어 주행속도별 신진대사 해당치 측정 연구

척수손상 장애인들의 신체활동량 저하와 관련된 문제점들이 대두되면서, 그들의 신체활동 모니터링을 위해 휠체어 활동과 관련된 트래커 개발이나 에너지소비 추정 알고리즘 개발 연구들이 진행되고 있다. 본 연구는 척수손상 장애인의 에너지소비 추정 알고리즘을 개발하기 위한 기초연구로써, 척수손상장애인의 휴식기 신진대사량과 휠체어 주행 속도에 따른 신진대사 해당치를 측정하였다. 9명의 척수손상 장애인을 대상으로 이동식 무선 호흡가스분석기를 이용하여 RMR과 MET를 측정한 후 RMR 측정치와 상용화된 5종 RMR 방정식의 추정치를 비교하였다. 연구결과, 척수손상 장애인의 실제 측정 RMR 값은 Revised Harris-Benedict 방정식 추정치와 가장 근사하였으며, 휠체어 주행속도별 MET 값은 2 km/h 에서 3.00±0.98, 3.5 km/h 에서 4.24±1.48, 5.0 km/h 에서 6.43±2.04으로 나타났으며, 이는 각각 RPE 9, 11, 13단계의 활동수준으로 정상인 RPE 수준보다 고강도의 운동임을 확인하였다. 본 연구결과는 척수손상 휠체어 사용자의 에너지소비 추정 알고리즘 개발을 위한 기초 데이터로써 활용하고자 한다. As problems related to the decrease in physical activity of disabled people with spinal cord injuries are being raised, researches on the development of tracker or energy consumption estimation algorithm related to wheelchair activity are being conducted to monitor their physical activity. This study is a basic study to develop an energy consumption estimation algorithm for disabled people with SCI, and we measured the Resting Metabolic Rate and the Metabolic Equivalent of Task of their by wheelchair driving speed. RMR and MET were measured using a portable wireless breathing gas analyzer for 9 patients with spinal cord injury and the measured RMR values and the estimated values by five RMR equation were compared. The results showed that the actual measured RMR values of SCI patients were close to those of the Revised Harris-Benedict equation. And the MET values for each wheelchair speed was 3.00 ± 0.98 at 2 km / h, 4.24 ± 1.48 at 3.5 km / h and 6.43 ± 2.04 at 5.0 km / h, respectively. It was confirmed that the exercise intensity was higher than the normal RPE level. Our results are intended to be used as basic data for the development of energy consumption estimation algorithm for wheelchair users with SCI.

In this study, we developed a folderble electric wheelchair that can separate the manual wheels and battery from the wheelchair body to help the disabled with limited physical activities or the elderly who have difficulties in walking. Therefore, usability evaluation was conducted in indoor and outdoor driving environments for 12 persons with disabilities using wheelchairs. The results of this study are as follows: First, the satisfaction with the device of QUEST 2.0 was evaluated with an average of 31.2 points, 78% of the satisfaction, and among them, the efficiency was evaluated the highest with 85 points. In the electric wheelchair usability evaluation (USPW), the average score was 149.8 points, which was evaluated as 86.1% satisfaction, and the satisfaction in operability and efficiency was evaluated more than 90%. Second, the satisfaction of caregiver was rated as 80% on average about disassembling, reassembling, and transporting a wheelchair. In addition, although it is not difficult to disassemble or reassemble the wheelchair, it is suggested that the elderly caregiver is still heavy when loading the vehicle. Third, it is expected to be commercialized through additional R&D and continuous user monitoring based on the inconvenience and improvement directions shown in the usability evaluation, thereby reducing the mobility restrictions of wheelchair users and the physical load of the caregivers.
