        Dermatofibrosarcoma Protuberans: A Study of Clinical, Pathologic, Genetic, and Therapeutic Features in Korean Patients

        Purpose: Dermatofibrosarcoma protuberans (DFSP) carries a translocation resultingin the collagen type I alpha 1 (COL1A1)-platelet-derived growth factor beta (PDGFB) fusion gene, which is responsible for PDGFB activation. The purpose of this study is to evaluate the clinicopathological, genetic, and therapeutic features of DFSP in Korean patients. Materials and Methods: Clinicopathological featuresof 37 patients with DFSP were reviewed. Multiplex reverse transcriptase-polymerase chain reaction (PCR) was carried out in 16 patients using formalin-fixed, paraffin-embedded tissues and specific primers for COL1A1 and PDGFB. Results: The mean age of 37 patients was 37.4 years old. The most common tumor location was the trunk. All patients were treated primarily with surgery: 34 (91.7%) cases with Mohs micrographic surgery (MMS) and 3 (8.3%) cases with wide local excision. The median follow-up time was 33.7 months. Two patients, one in each treatment group, demonstrated local recurrence during the follow-up period. The COL1A1-PDGFB fusion gene was expressed in 14 (87.5%) cases, demonstrated by reverse transcriptase PCR analysis. No association was found among the differentCOL1A1-PDGFB fusion transcripts, the various histological subtypes and clinical features. Conclusion: Our results support the effectiveness of MMS in treating DFSP. The COL1A1-PDGFB fusion transcript was observed in 87.5% of patients. Therefore, COL1A1-PDGFB is a useful and accurate tool in diagnosing DFSP in Koreans.

      • Proteome analysis of sera of patients with active Behcet`s disease

        Background: Behcet’s disease (BD) is a chronic, multisystemic vasculitis that theoretically affects all sizes and types of blood vessels. The validated biological marker to determine the disease activity of BD is still lacking. Therefore, our aim of this study was to search relevant markers for active BD using proteomic analysis. Objectives: The aim of this study was to search relevant markers for active BD using proteomic analysis. Methods: To find a novel serum marker in BD, we used pooled sera (equal volumes from 5 active BD patients and 5 healthy volunteer donors) and proteomics approach. For proteomics analysis, two-dimensional gel electrophoresis along with matrix-assisted laser desorption ionization time-of-flight mass spectrometry were performed. Results: There was significant differential expression of proteins in active BD and healthy control. We identified 13 maximally differentially expressed proteins after LC-MALDI-TOF/TOF analysis of 21 spots. Among 13 proteins, four (visual system homeobox 2, complement receptor Cr2, complement component C3c, truncated human apolipoprotein A) was downregulated and two proteins (Haptoglobin, FAM123A protein) were up-regulated in BD compared with healthy control. In addition seven proteins (myosin-15 precursor, vitronectin, heparin cofactor Ii, growth-inhibiting protein 25, bamacan homolog, amyloid related serum protein SAA, proapolipoprotein) were only detected in BD serum. Conclusion: Our study provides potential protein markers of active BD.

      • The Disturbance Rejection Design Based on Physical Feedforward for Solid Oxide Fuel Cell

        The output voltage control of the Solid Oxide Fuel Cell (SOFC) is challenging due to the strong nonlinearity, modelling uncertainty, the dramatic change of the external current load and constraints on the amplitudes and change rates of the actuator. Besides, the desired range of the fuel utilization is another difficult for the control strategy. In this paper, Active disturbance rejection control (ADRC) is proposed to handle the nonlinearity, uncertainty and the external disturbance, and physical feedforward is designed to speed up the response of the whole system and keep the fuel utilization in the desired range. Instead of the SOFC model, the design of physical feedforward is based on the definition of fuel utilization which is featured by simplicity, generality and effectiveness. Simulation results show that the proposed control strategy has smaller variation range of the fuel utilization, shorter time of the changing process of the output voltage and better robustness than others control strategies.

Methods: Monocytes and T cells were isolated from peripheral blood. Protein levels were measured using western blot, ELISA, and flow cytometry. Co-cultures between monocyte and responder T cells were maintained in media for 7~14 days. Knock-down of TRIM21 was performed using siRNA technique Results: TRIM21 expression was increased in peripheral blood monocytes from BD patients. The expression of IRF8, a representative ubiquitination target of TRIM21, was also decreased. BD monocyte promoted secretion of pro- inflammatory cytokines including Th17 promoting cytokine following NF-kB activation. BD monocytes promoted Th17 polarization after co-culture with responder T cells from healthy donors, and IL-17A production from these T cells was also increased. Knock-down of TRIM21 in monocyte prevents NF-kB activation and decreased Th1,Th17 polarization of responder T cell Conclusion: BD monocytes facilitate Th/1Th17 differentiation of naive T cells and TRIM21 may regulate the secretion of pro-inflammatory cytokine from monocytes.

        Background: Behcet’s disease (BD) is a chronic, multisystem vasculitis and autoinflammatory disorder, characterized by oral ulcers, genital ulcers, various inflammatory skin lesions, and uveitis. The exact cause of BD is still not known, many study suggest that this disease triggered by environmental factor including infectious agents such as herpes simplex virus (HSV) and streptococcus sanguinis. Tripartite motif containing 21 (TRIM21), an E3 ligase protein, regulates the production of cytokine by ubiquitination of transcriptional factors such as interferon regulatory factor (IRF) family and NF-kB. Moreover TRIM21 can present antiviral properties, mediating antiviral defense mechanisms to DNA/RNA viruses. Objective: The purpose of this study was to elucidate TRIM21 proteins expression in monocyte of BD patient, and to identify the role of TRIM21 on immune dysregulation in BD. Methods: Monocytes and T cells were isolated from peripheral blood using Ficoll-Hypaque density-gradient centrifugation and relevant immunomagnetic negative selection method. Protein levels were measured using western blot, ELISA, and flow cytometry. Co-cultures between monocyte and responder T cells (1:1 to 1:2 ratio) were maintained in media for 7~14 days. Knock-down of TRIM21 was performed using siRNA technique on THP-1 or freshly isolated monocytes. Results: TRIM21 expression was increased in peripheral blood monocytes from BD patients compared to healthy control. The expression of IRF8, a representative ubiquitination target of TRIM21, was also decreased in BD monocyte. BD monocyte promoted secretion of pro- inflammatory cytokines including Th17 promoting cytokine such as IL-6, IL-1β and IL-23 following NF-kB activation. BD monocytes promoted Th17 polarization after co-culture with responder T cells from healthy donors, and IL-17A production from these co-cultured T cells was also increased. Known-down of TRIM21 in monocyte using siRNA prevents NF-kB activation and decreased Th1 and Th17 polarization of responder T cell in co-culture system. Conclusion: From our results suggest that BD monocytes facilitate Th/1Th17 differentiation of naive T cells and TRIM21 may regulate the secretion of pro-inflammatory cytokine from monocytes.

      • FCP 12 : The role of bone scintigraphy in Behcet`s disease from the view of the dermatologist

        Background: Behcet’s disease(BD) is a multisystemic inflammatory disease with articular involvement(9%-70% in previous reports). However, non-specific arthralgia is also common in BD patients, and the exact diagnosis of joint involvement is often challenging especially for non- rhuematologists. Objectives: To evaluate the usefulness of 99mTc- methylenediphosphonate(Tc-99m-MDP) bone scintigraphy from the viewpoint of the dermatologist Methods: A total of 211 patients were analyzed. Agreement between bone scintigraphy findings and clinical symptoms was assessed with the Cohen’s kappa statistic. At the same time, 104 patients who showed the accordance between imaging studies and clinical symptoms were re-evaluated by rheumatologists. Results: The total kappa value of 211 cases was 0.604, and the agreement on clinical symptom and scintigraphy image was fair. Individual analysis of the 11 categorized jointsshowed that there were statistically significant correlations in wrist(κ = 0.677), shoulder(κ = 0.661), and foot joints(κ = 0.618). Of the 104 cases referred to the rheumatologist, 95 cases(91.34%) were confirmed as articular involvements of BD. The joint involvement of 9 cases diagnosed as other disorders was noticed in hip, spine and knee joints, all of which were the joints showing low correlation of kappa value. Conclusion: Bone scintigraphy can be a simple and useful option to determine the joint involvement of BD for the dermatologist, especially in certain anatomical sites.

        Combination of High- and Low-Fluenced Erbium:Yttrium- Aluminum-Garnet Laser Treatments for Striae Distensae in Asian Patients

        The safety and efficacy of 2,940-nm erbium (Er):yttrium-aluminumgarnet (YAG) laser treatments have been remarkably improved by incorporating technology in which high-fluenced, fractionated laser pulses are delivered to target areas, leaving the surrounding skin uninjured, and low-fluenced resurfacing technology delicately generates photothermal tissue reactions in the uppermost epidermis. In this report, we describe two Korean patients with striae distensae on the lower back and abdomen, respectively, who were effectively treated with a single session of combined high-fluenced, fractionated Er:YAG laser and lowfluenced Er:YAG resurfacing laser treatment. High-fluenced, 2,940-nm fractionated Er:YAG laser treatment was delivered along the striae distensae lesions at a fluence of 37 J/cm2 and coverage of 11% over a single pass. Immediately after high-fluenced, fractionated Er:YAG laser treatment, low-fluenced Er:YAG laser resurfacing was performed with a beam size of 4 mm and a fluence of 2.0 J/cm2 at the expected penetration depth of 8 μm and an overlap of 30% over a single pass. Two to three months after treatment, both patients exhibited marked improvements in the striae distensae lesions without remarkable major side effects. Our data demonstrate that combined high-fluenced, fractionated Er:YAG laser and low-fluenced Er:YAG resurfacing laser treatment can be used to effectively treat striae distensae in Asian patients.

        <P>As an angiogenic factor, thymidine phosphorylase (TP) expression in primary tumours has been thought to be a risk factor for lymph node (LN) and hepatic metastasis in patients with gastric adenocarcinoma. However, the molecular basis for the induction of metastasis by TP is largely unknown. We aim to elucidate the role of TP expression in gastric cancer neovascularisation and LN metastasis.The angiogenic and lymphangiogenic activity (CD31, D2-40, Ki-67, VEGFC, VEGFR3) and expression status of TP were detected in 103 resected human gastric carcinoma samples by immunohistochemistry. The influence of TP expression on neovascularisation and cancer cell invasion was further comparatively investigated in two groups of nude mice intraperitoneally injected with TP overexpressing MKN-45 cells (MKN-45/TP) and control cells (MKN-45/CV). In gastric cancer tissues, we found that high TP expression and various angiogenic and lymphangiogenic activities were significantly associated with poor prognostic outcomes. In addition, TP expression was also found to be associated with neovascularisation activity of gastric cancer tissues. In vivo, the MKN-45/TP group exhibited significantly increased infiltrating tumour nodules and neovascularisation activity compared to the MKN-45/CV group. TP could strongly influence gastric cancer progression via the dual activities of angiogenesis and lymphangiogenesis.</P>
