        顎間固定이 家兎의 下顎頭 軟骨細胞에 미치는 影響에 關한 硏究 : A light and electron microscopic study 光學顯微鏡的 및 電子顯微鏡的 硏究

        The purpose of this study was to observe the effect of intermaxillary fixation on the chondrocytes of the mandibular condyle under the light and the electron microscope. For this study, twenty rabbits were placed in maxillomandibular fixation, and two were used as a control group. The experimental group was subdivided into 3, 7, 14, 21 and 28 day group. After the experimental period of 3, 7, 14, 21 and 28 days, the animals were sacrificed with a vascular perfusion of 2.5% glutaraldehyde. The condylar processes were exenterated, and decalcified in 0.1 M EDTA with 2.5% glutaraldehyde solution for two weeks. The specimens were rinsed with phosphate buffer solution and the post-fixation was carried out with 2 % osmium tetroxide at 4℃ for two hours. Thereafter the specimens were dehydrated in alcohol series, cleared with propylene oxide and embedded in Epon 812 resin. Thin sections and ultra-thin sections were made, and the cellular structures of the condylar cartilages were observed with light and electron microscope. The results were as follows : 1. In the intermaxillary fixation group, the cartilaginous tissues of mandibular condyules showed a marked decrease in the thickness compared to the control group. 2. A remarkable change was noticed in the proliferating and the hypertrophic zone of the condylar cartilages in the experimental group. 3. An atrophic change of the condylar cartilage was appeared in the 3 day experimental group and degenerative change was observed in the 7 day experimental group, and recovery was seen in thereafter 14 day experimental group. 4. Calcification, degeneration and resorption of condylar cartilage were recognizable, and the cellular zone of the condylar cartilage was appeared indistinctly in 3 day and 7 day experimental group. The chondroblasts, however, were differentiated into chondrocytes and resumed mitosis, and then the cellular zones of the condylar cartilage were reorganized from the 14 day experimental group under the findings of light microscope. 5. Under the findings of electron microscope, atrophic changes and decrease in number of intracellular organelles, degenerative changes of cytoplasm, and pyknosis of nuclei were observed in early stage, however, a gradual regeneration and reorganization of the intracellular organelles were observed from 14 day experimental group.

      • Biocompatible Osteoconductive Polymer와 吸收性 Hydroxyapatite가 骨化에 미치는 影響에 關한 硏究 : A HISTOLOGICAL STUDY

        The purpose of this study was to observe the osteogenic effects of biocompatible osteoconductive polymer(BOP D.T.I. Co., U.S.A.) and resorbable HA(Osteogen ; Impladent Co., U.S.A.) in bony defected area In order to perform this study, 30 male rabbits, weighing about 2.5kg were selected. The 3x5x7mm sized defects were made on the both sides of the mandibular body and the defect was filled with blood clot in control group and with resorbable HA and BOP respectively in experimental groups. All experimental animals were sacrificed at the 3rd day and the 1st, 2nd, 4th and 6th week after operation. The mandibular body and surrounding tissues were removed out, deminerahzed, dehydrated and embedded. The histological observation of the specimens in light microscopic level was performed after Azan staining. The results were as follows 1. Osteoblasts were observed after a eek both in control group and experimental groups, and fibrous tissues were formed in bony defected area. Especially, new bone formation was observed earlier in BOP experimental group than in any other groups. 2. New bone formation was developed from the adjacent normal bone to bony defected area both in control group and experimental groups. And in experimental groups, new bone was infiltrated rapidly into the graft material than in control group and large amount of bone formation was observed. 3. Both in two experimental groups, the resorption pattern of the graft material was observed after 2 weeks, an materials were almost disappeared after 4 weeks. 4. After 6 weeks, bony defected area was occupied with new bone both in control group and in experimental groups, but bony density in experimental groups was higher than in control group, especially in BOP experimental group which showed more mature heating state.

        筋肉迷入이 骨折治癒過程에 미치는 影響에 關한 實驗的 硏究

        The purpose of this study was to investigate the effect of muscle impingement to healing process of the fractured bone. For the purpose, the mandibular angle area of eight rabbit were used. In the experimental group the defects were made with 1㎜, 3㎜, and 6㎜, width and 10㎜ height, then the masseter muscle flaps were impinged in the defects and sutured with internal pterygoid muscles. In the control group the defects were made only as same manner on the opposite site. The experimental animals were sacrificed at 4 weeks after surgery and the operation sites were examined under the light microscope to observe the bony repair process histopathologically. The results obtained from this study were as follows: 1. The impinged muscular tissues were disparted and released by ribroblasts and collagen cell regeneration, but not resorpted. 2. Both fractured ends were united by collagen fibers containing muscular fibers. 3. New bone formation was observed under the bony cutting ends and cortical layers. 4. There are no remarkable differencies in the histologic changes of the healing processes according to the amount of the bony defects. 5. Large quantities of bony callus formation were observed in the bony defected portions of the control group.

        증예보고 : Globulomaxillary Cyst 에 대한 문헌고찰 및 치험예

        The globulomaxillary cyst has been known as a fissural cyst which arised from embryonic ectodermal cells entrapped between developing facial processes. But, the origin of epithelial remnants has been a subject of confusion and controversy in the dental literature still now. This is a case report of globulomaxillary cyst which was diagnosed through the clinical, radiological, and histological examinations and review of literatures.

        증예보고 : 하악골수염의 치험예 및 문헌고찰

        These are to report two cases of osteomyelitis occurred in the mandibles of 22-year-old and 36-year-old male patients. They were admitted to Department of Oral Surgery, Kyung Hee University Medical Center, with the chief complaints of left facial swelling, pain and mouth opening interference. The one was treated by conservative treatment without sequestrectomy, and the other was treated by extensive sequestrectomy from left mandibular body to left condylar region. The results were as follows: 1. In the case (1), the patient has visited with the state of acute osteomyelitis, but it was changed to chronic osteomyelitis in spite of massive antibiotic therapy it was diagnosed as gramnegative osteomyelitis caused by klebiella pneumoniae through the several cultures. 2. In the case (1), we observed in the radiogram, the sequestra about 15×10mm w size which has occurred in the inferior border of mandible near the left mandibular angle region. After 7 weeks of conservative treatment without sequestrectomy, which consisted of local wound care and massive antibiotic therapy, the author could notice the resorption of sequestra, in the radiogram. Six months later, in the floow-up check of the patient, we could observed marked bone regeneration without recurrence, 3. In the case (2), extensive bone destruction was noted on the left mandibular body, angle, ascending ramus, mandibular notch and condylar region. Following sequestrectomy, we could observed marked bone regeneration after 6 months. The author thought this bane regeneration as the result of osteogenesis of the periosteum and lingual cortical plate which had been remained healthy during the operation.

        증예보고 : 편측성 하악두 증식에 의한 비대칭 안모의 악교정 외과적 치험예

        This is to report a case of facial asymmetry from unilateral condylar hyperplasia. 1. The patient, 29 years old female, has being suffered from facial asymmetry and masticatory disturbance for 12 years. 2. Clinical and radiographic examinations revealed hyperplasia of right mandibular condyle and elongation of condylar neck which was diagnosed as condylar hyperplasia. 3. We obtained excellent and functional results by the treatments of subcondylar ostectomy and pre-& post-operative orthodontic treatment.
