CT Urography 검사 시 Compression bend와 호흡법을 이용한 영상 고찰

목적 CT Urography 검사에서 요관의 충분한 조영상과 생리적 협착부위의 진단적 가치를 높이기 위한 검사 방법을 알아보고자 하였다. 대상 및 방법 2011년 11월부터 2012월 3월까지 복부CT검사를 시행한 60명의 환자(남: 36명, 여: 24명-평균연령 60.5세)를 대상으로 흡기 시 5분 지연 상 20명(A그룹), 호기 시 5분 지연 상 20명(B그룹), 호기호흡과 함께 압박대의 압박을 시행한 5분 지연 상 20명(C그룹)을 대상으로 하였다. A,B,C 그룹 동일한 5분 지연 상을 Volume rendering 영상으로 재구성한 후 요관의 조영상과 생리적 협착부위인 요관 신우 이행부(UPJ), 좌우 총장골정맥과 요관이 교차하는 교차부, 요관 방광저 이행부(UVJ)를 중심으로 Score를 주어 평가하였다. 결과 A그룹(흡기시 5분 지연 상)에서의 평균은 36.8점, B그룹(호기 시 5분 지연 상)에서의 평균는 38.8점, C그룹(호기호흡과 함께 압박대의 압박을 시행한 5분 지연 상)에서의 평균은 43점이였다. C그룹 호기호흡과 함께 압박대의 압박을 시행한 방법에서 가장 높은 Score가 나왔고, 그 다음으로 B그룹, A그룹 순위였다. 결론 CT Urography 검사에서 호기호흡과 압박대의 압박을 통한 검사 방법이 요관의 충분한 조영상에 영향을 미치고 생리적 협착부위의 진단적 가치를 높이는데 유용한 검사 방법이라 생각된다. I. Purpose We studied test for sufficient contrast image of ureter and diagnostic value rise of physiological stenosis on CT Urography Test. II. Meterial and Methods We carried out research into 60 patients (36 men, 24 women - average age : 60.5) who had done abdominal CT scan from November, 2011 to March, 2012. We Classified them into 3 groups, 20 people 5 minutes delay image on inspiration breath (A group), 20 people 5 minutes delay image on expiration breath (B group), 20 people 5 minutes delay image on expiration breath with tourniquet, After reconstructuring same 5 minutes delay image into Volume rendering image, we gave a score focusing on urethral contrast image, ureter and renal pelvis junction, right-left common iliac veins and ureter track to cross intersection, ureterovesical junction as physiological stenosis. III. Result Average Point was 36.8 on A group (5 minutes delay image on inspiration breath), 38.8on B group (5 minutes delay image on expiration breath and 43 on C group (5 minutes delay image on expiration breath with pressure by tourniquet), C group executed expiration breath with pressure by tourniquet got highest score and then B group, A group came in order. IV. Conclusions It shows that the test with expiration breath and pressure by tourniquet gives effect to sufficient contrast image of ureter and it is a useful way for diagnostic value rise of physiological stenosis.
