        건선 조직에서의 c - fos 및 c - myc 암유전자 발현 증가

        The expression of the c-fos and c-myc oncogenes was investigated by dot blot analysis of total RNA extracted from biopsies of normal skin and psoriatic lesions. Densitometric analysis revealed 6.3 fold and 8.6 fold evevation respectively of c-fos and c-myc oncogenes mRNA levels in severe psoriasis compared to normal skin, and 1.5 fold and 1.6 fold elevation respectively of c-fos and c-myc oncogenes mRNA levels in mild psoriasis compared to normal skin. The ratio of c-fos/c-myc transcripts in normal skin and mild psoriasis was constant compared to that in severe psoriasis. The maintenance of a constant ratio of c-fos/c-myc is believed to be closely related to ordered cell growth and the ratio of c-fos/c-myc transcripts in severe psoriasis suggests a disordered pattern of cellular differentiation. We be1ieve that overexpression of these oncogenes is related to the pathogenesis of psoriatic epidermal hyperplasia. (Kor J Dermatol 29(3): 331 336,1991)

        건선 조직에서의 Epidermal Growth Factor Receptor 유전자의 발현

        Psoriasis is characterized by disregulation of keratinocyte growth with profound epidermal hyperplasia. Keratinocyte hyperplasia in psoriasis may be expained in part by overproduction of growth factor, and by altered metabolism of the epidemal growth factor receptors (EGFR) in affected skin. The expression of epidermal growth f ictor receptor was investigated by Northern blot and slot-blot analysis of total RNA extrated from biopsies of normal skin and psoriatic lesions. In Northern blot analysis, EGFR-specific mRNA transcripts from psoriatic tissues demonstrated the specificity of hybridizarion with a EGFR mDNA probe. The size of EGFR mRNA transcript was 6.7kb in psoriasis lesions which showed no change of quality. In slot-blot analysis, the levels of EGFR mRNA in poriasis revealed a 1.2 fold to 4.1 fold elevation when compared to normal skin. EGFR were present in all epidermal layers by immunoperoxidase staining, whereas in normal skin they were primarily present in the stratum basalis. These results indicate that the increased expresion of the EGFR gene may be, in part, responsible for the hyperproliferation of the epider nis and that retained EGFR may reflect incomplet; abnormal differentiation in active porasis. This altered process of EGFR metabolism may be involved in the pathogenesis of psoriasis. (Kor J Dermatol 1992;30(4):431-438)

        편평상피암에서의 c - myc , erb B 및 EGF - Receptor 유전자의 세포 - 분자생물학적 특성

        Background : Oncogenes and EGF-Receptor(EGFR) may be involved n different stages of the multistep carcinogenesis process. A specific pattern of karyotypic abnormalities in solid tumors can be detected by cytogenetic methods. Onjective : This study is intnded to observe the cytomolecular kiologic chracterization of c-myc, erb B and EGFR genes in squasnous cell carcinoma(SCC) of the skin and cervix. Methods : We have eytogenet,ically examined the short-term culturs from SCC. The rearrangement, amplification or expressi.on of erb B, c-myc, and EGFR genes were studied by Southern blot, analysis of genomic DNA and by slot blot analysis of tota! RNA extracted from biopsies of normal skin and SCC tissues. EGFR expression was examined immunohistochemially using monoclonal antibodies and the localizat,ion of the c-myc oncogene mRNA by in situ hybridization. Results : A remarkably structural aberration was del 6(q21-qter) counted 20 metaphases among 28 metaphases ana1yzed. In nunierical aberration, all chromosomes were lost or gained randomly. Amenploid including triploid and tetraploid were observed in 8 metaphases, 6 tumor cells contained marker chromosome. In Southern blot analysis, rearrangement and amglificaton of EGFR in primary squamous cell carcinoma of cervix uteri and skin respectively. In slot blot analysis, the levels of c-myc, erb B and EGFR mRNA increaaed respectively 3.5, 2.5 and 2.8 times in SCC when compared to normal tissues. In immunoperoxidase stain, EGFR was present, in SCC where keratinocytes with strong cyto-plasmic staining but no membr, line labelling, where as in normal skin the were primarily present in t,he membrane and cytoplasm of basal cells. In situ hybridization with c-myc cDNAs allowed detection of grains representative of biotin labelled cDNA-mRNA hybrids in the frozden section of SCC tissues. Conclusion : These results suggest that specific patterns of karyotypir abnormalites, rearrangement, or amplification of EGFR gene, and overexpression of oncogenes and EGFR gene may be associated with the carcinogenesis of SCC.         급성 발열성 호중구성 피부병 1례

        Acute febrile neutrophilic dermatosis, which was introduced to the dermatologic literature by Sweet in 1964, is characterized by persistent higt fever and preceding upper respiratory infection like symptom, polymorphonuclear leukocytosis in the peripheral blood, raised painful plaque on the face, neck and limbs, histologically a dense dermal infilteration with polymorphonuclear leukocytosis, dramatic reponse to corticosteroids, and the absence of scarring. A 41 year-old female who had suffered from malaise, arthragia, upper respiratory tract infection like symptoms one month before the each episode of the cutaneous manifestation. Erythematous, well demarcated, slight scaly plaque appeared on the face with tendency of recurrences. Histologically a dense inflammatory cell infilteration, composed predominantly of polymorphonuclear leukocyte, lymphocytes and histiocytes were seen in dermis. The skin lesion and general symptoms were well controlled with orally administered corticosteroid.

        Fordyce Granules

        Fordyce granules are small yellow spots that are multifocal and slightly elevated. They may occur in clusters or coalesce to from yellow plaques. They are bilaterally symmetrical and are located most frequently on the buccal mucosa of the cheeks, the inner surface of the lips, and on the mucosa distal to the mandibular molars. Fordyce spots are extremely common: probably 80% of the population have them. They are often not noticeable in children until after puberty, and they seem to be more obvious in males, in patients with greasy skin and in the elderly. The granules are totally benign, though the occasional patient or physician becomes concerned about them or misdiagnoses them as thrush, lichen planus or other diseases. We present two cases of Fordyce granules a 64 and a 50 year-old male who have cancer phobia, showing multiple involvement of minute yellowish white colored spots on the buccal mucosa and inner surface of the lip. Histopathologic finding shows a group of small but mature sebaceous lobules situated around a small sebaceous duct leading to the surface epithelium. We did not give any specific treatment but gave reassurance, presently under observation.

        건선 치료시 Etretinate와 PUVA의 병용요법에 대한 임상적 고찰

        RePUVA therapy is the combination of an oral aromatic retinoid(Etretinate) with oral photochemotherapy(PUVA) for the treatment of psoriasis. This combination therapy has the advantage of faster clearance with fewer side effects. Thirty ambulatory psoriasis patients received RePUVA treatment. Etretinate was administered in a dosage of 0.75㎎/㎏ body weight, and PUVA was tried two times per week. Initial response of RePUVA therapy occured in patients with an average of 13.8±9.5 day, 6.0±2.8 irradiation and an accumulated ultraviolet A(UVA) dose of 6.8±4.8J/㎠. Clearance of lesion occured in 53% of patients with an average of 115.1±63.5day, 20.1±6.5 irradiation and an accumulated ultraviolet A(UVA) does of 38.9±17.7J/㎠. Side effects were occured in 27 patients. Dry skin & lip was most prominent side effect and erythematous pruritus, massive desquamation were followed. GI trouble, brittle nail, hair loss manifested, too.

        1형 나반응을 동반한 근결핵양나 1례

        Reactions are acute or subacute hypersensitivity states that complicated the existing active and, more often, the treated infection. A modern description of reactions in Hansen's disease will include 3 types such as Type 1 reaction, erythema nodosum leprosum, and Lucio's phenomenon. Type 1 reaction occurs in patients suffering from borderline leprosy during the first six months of treatment. Characteristic symptoms are nerve pain concomittently with muscle paralysis, rapid swelling & erythema of some or all of the skin lesions and edema of face, hands & feet. It is generally considered that this type of reaction is a delayed hypersesitivity phenomenon in which antigen from breaking down bacilli reacts with T lymphocytes. Our case was sixty five year old woman who had multiple erythematous, swollen lesions on the both hands, feet and face, and suffered from paresthesia, dyshydrosis and mild tenderness on that sites 1 year ago. The symptoms were more aggrevated 20 days ago. General symptoms were accompanied such as fever, malaise, fatigue, loss of appetite. Family and past history was non contributory. And we tried many tests, skin smear and skin biopsy and then concluded final diagnosis was borderline tuberculoid leprosy with type 1 reaction. We had treated this patient with 20-40㎎ of prednisolone, 100㎎ of dapsone, 200㎎ of clofazimine and 600㎎ of rifampicin daily for a week, and then, had transfered to special leprosy clinic in the mild improved state. Now we present a case of borderline tuberculoid leprosy with type 1 reaction with references.
