        소아 호출기 환자에서 경구 투여된 Formoterol이 Theophylline의 혈중농도에 미치는 영향의 연구

        Theophylline and $\beta$-adrenergic agonists are frequently used together in patients with respiratory diseases. However the clinical impact of $\beta_2$-adrenergic agonists on the blood concentration of theophylline is not fully evaluated. Formoterol, a newly available oral ${\beta}_2$-adrenergic agonist is frequently used in pediatric respiratory patients. The objective of this study was to investigate the effect of oral formoterol on theophylline's blood concentration. Randomized prospective study was conducted. Twenty-four children were enrolled on the study. Their age ranged 2 to 73 months (mean 35.8 months). Theophylline group (12 patients) received 10 mg/kg/day of for theophylline orally. Theophylline/formoterol group (12 patients) received 10 mg/kg/day of theophylline and $4\;{\mu}g/kg/day$ of formoterol orally. All medications were administered at least for 5 days starting on admission day. Theophylline's trough concentrations were obtained on days 3 and day 5. Pulse rates were recorded before the study medications were given on admission, and days 3 and day 5. Statistical significance was calculated by two-tailed Student's t-test. Theophylline's levels in children given theophylline and formoterol together were lower an those given theophylline alone ($6.38\pm0.90\;{\mu}g/ml\;vs\;7.43\pm0.77\;{\mu}g/ml$ on day 3(p<0.05), $5.62\pm0.56\;{\mu}g/ml\;vs.\;6.78\pm0.61\;{\mu}g/ml$ on day 5 (p<0.05)). In both groups, theophylline's trough concentration on day 5 were lower than day 3. There was no significant side effects in both groups. In conclusion, the new ${\beta}_2$ selective adrenergic agonist formoterol reduced serum theophylline levels in children with respiratory diseases. Further investigation is needed to clarify the long term effect of this drug interaction.

        EU GMP 실사 부적합 영향 요인 분석연구 : Active Substance, Human Medicinal Product를 중심으로

        To export medicinal products to Europe, EU Good Manufacturing Practices (GMP) inspection is a critical step. Using the EudraGMDP database from 2009 to 2019, this study was conducted to determine factors influencing noncompliance in the EU GMP inspection. The results revealed that the number of EU GMP inspections has steadily increased in both EU and non-EU countries. The latter showed higher non-compliance rates compared to the former. For active substances, analysis of observations from non-compliance inspections indicated that “Documentation and records,” “Quality management,” and “Equipment” are common items observed as non-compliant. For human medicinal products, “Pharmaceutical quality system,” “Production,” and “Premises and equipment” are frequently observed as non-compliance items. “Building and facilities” and “Contract manufacturing” were found to be significant factors causing GMP qualification failure for active substances. For human medicinal products, “Quality control” was found to be a significant factor in GMP qualification failure. This study confirmed that non-compliance rates for OECD and non-OECD countries were significantly different.

        의약분업후 약사의 병원 원외처방전 검토 업무사례 분석

        The study has investigated an impact of the outpatient prescription checking program operated by pharmacists in the hospital since the implementation of Boonup. The prescriptions detected through the program operation were collected from March to May of 2001 and 2002. Total 1.3%(648/49,680) of prescriptions in 2001 and 1.5%(745/48475) of prescriptions in 2002 were canceled due to the prescription errors founded(p=0.001). Pricing errors were 2.0% in 2001 and 1.8% in 2002. Dosage error was the most frequent reason for canceling the prescription. Total prescription error rate was significantly increase from 2001. to 2002(p=0.001) in contrast that pricing error has been decreased(p=0.017). Request rate from outside pharmacies through fax was remained same in both years. The rate for bioequivalent drug exchange has been decreased significantly from 2001 to 2002(p=0.045). In conclusion, The outpatient prescription checking program by the pharmacist has shown to be effective for detecting prescriptions with problems. The program should be a beneficial tool to prevent medication errors from the outpatient clinics.
