국내 인지재활 현황 및 미래발전방안 기초조사연구: 전문가 델파이 조사를 중심으로

목적: 본 연구는 국내 작업치료 인지재활의 최근 현황과 문제점 및 개선사항을 조사하고, 미래 인지재활 발전을 위해 필요한 사항들을 도출하기 위함이다. 연구방법: 포커스 그룹의 토의를 통해 구성된 개방형 질문을 이용하여 전문가 패널 16명에게 1차 델파이 조사를 실시하였다. 이후 폐쇄형 질문으로 재구성하여 2차 델파이 조사를 시행하였다. 2차 델파이 조사를 통해 평균, 표준편차, 내용타당도 비율(Content Validity Ratio; CVR), 안정도, 수렴도, 합의도를 확인하였다. 결과: 1차 델파이 조사 결과 104개의 항목이 도출되었고, 2차 델파이 조사 결과 CVR 값이 .49 이하의 값을 보인 41개 항목을 제거하여 최종 63개 항목을 선정하였다. 질문별로 가장 높은 내용 타당도 비율을 보인 항목은, 국내 인지재활 발전 영역에서 ‘대학 교육과정에 인지재활 관련 과목 편성’, 현재 인지평가의 문제점 영역에서 ‘표준화된 작업수행기반 인지평가 도구 부족’, 현재 인지중재의 문제점 영역에서 ‘인지요소별 향상과 작업수행 향상 연결 부족’, 현재 제도적인 문제점 영역에서 ‘인지재활 관련 보험수가 부족’, 미래 발전을 위해 치료사가 갖추어야 할 전문성 영역에서 ‘작업수행기반 인지장애 평가와 중재 능력’, 새롭게 개발하고 도입해야 할 평가 또는 중재 영역에서 ‘작업 수행기반 인지평가도구’, 인지재활 발전을 위해 확대되어야 할 영역에서는 ‘작업수행기반 인지재활’로 파악되었다. 결론: 전문가 델파이 조사를 통해 작업치료 내 인지재활의 현황과 미래발전을 위해 노력해야 할 사항들을 조사하였다. 본 연구를 기반으로 국내 인지재활의 발전을 위한 연구가 활발히 진행되기를 기대한다. Objective: This study is to investigate the current status, problems, and improvements of domestic occupational therapy cognitive rehabilitation, and to derive necessary matters for the future development of cognitive rehabilitation. Methods: The first Delphi survey was conducted with 16 experts panel using open-ended questions formed through focus group discussions. The second Delphi survey was conducted by reorganizing the questionnaire into closed-ended questions. The mean, standard deviation, CVR, stability, convergence, and consensus were analyzed with the second Delphi data. Results: As a result of the first Delphi survey, 104 items were derived. As a result of the second Delphi survey, 41 items which was less than .49 of CVR value were removed, and 63 items were finally selected. The items with the highest content validity ratio for each question were ‘Organization of subjects related to cognitive rehabilitation in the university curriculum, ‘Lack of standardized occupational performance-based cognitive assessment tools’, ‘Insufficient connection between improvement by cognitive skills and improvement in occupational performance’, ‘Insufficient insurance coverage for cognitive rehabilitation’, ‘Occupational performance-based cognitive impairment assessment and intervention ability’, ‘Occupational performance-based cognitive assessment tool’, and ‘task performance-based cognitive rehabilitation’. Conclusion: The current state of cognitive rehabilitation in occupational therapy and the matters to be strived for in the future were investigated through expert Delphi survey. Based on this study, it is expected that research for the development of domestic cognitive rehabilitation will be actively conducted in Occupational therapy.
