노인환자의 대퇴경부 골절에 대한 양극성 반 치환술과 고관절 전 치환술의 비교

We compared the clinical and radiological results of bipolar hemiarthroplasty (BH) and total hip arthroplasty (THA) by analyzing 50 cases (25- BH, 25 -THA) of femur neck fracture (sex ratio M/F: 8/17; av.age 74/70yrs). The mean folloup period was 28months (24-39months). At the last follow- up, Harris hip score (HHS) was 84 (25-100) in BH and 91(82-100) in THA. In preop. activities of the daily living (ADL), 10 cases were in grade 1 (40%), 9 (36%) in grade 2, 4 (16%) in grade 3, 2 (8%) in grade 4 and none in grade 5 of the BH gruop; 19 cases (76%) in grade 1, 5 (20%) in grade 2, 1 (4%) in grade 3 and none in grade 4 and 5 of the THA group. In postop. ADL, there were 1 case (4%) in grade 1, 12 (48%) in grade 2, 10 (40%) in grade 3, 2 (8%) in grade 4 and none in grade 5 of the BH group; and 15 cases (60%) in grade 1, 8 (32%) in grade 2, 2 (8%) in grade 3 and none in grade 4 and 5 of the THA group. Postoperative deterioration of ADL more than one grade was observed in 12 cases (48%) of the BH and 4 cases (16%) of the THA. The average walking aids period was 23months (1.5~38) in BH and 2.3months (1.5~3) in THA. In the radiological results, nonprogressive loosening findings were noticed in 2 cases of BH only and they were observed without any intervention because of the excellent clinical results without pain. There were no significant differences in the complication rates and the postop. morbidity and mortality rates between the two groups.
