Real world psoriasis treatment with biologics

Anti-cytokine biologics are main treatment agent in biologics for moderate to severe psoriasis. Anti-TNF agent, anti-IL-12/23 agent and anti-IL17 are the major three categories in psoriasis biologics. In those agents, anti-TNF has been used for the longest time and there are lots of cumulative clinical data for clinician`s decision. Interesting current topics in psoriasis biologics are prognostic factors for biologics response. From long term registry data, some clinical factors are now proven to be the determinant for patients` response to biologic agent. Considering anti-TNFs, it has been used for more than 15 years for all clinical application, at least 10 years` safety data of these agents are reported to be safe comparing with normal population in case we should pre-screen the latent tuberculosis before the initiation of anti-TNFs. In this lecture, I will discuss about my real world experience about psoriasis bioloics use since 2004, and also mention about new issues on anti-TNFs.
A practical approach to identify eligible patients on biologics for the treatment of psoriasis

Psoriasis is now known to be a disease caused by chronically altered immune reaction. A lot of systemic comorbidities such as cardiovascular diseases, metabolic syndrome and psoriatic arthritis are linked to psoriasis, dermatologists who firstly encounter psoriasis patients who have the possibilities to develop such comorbidities should have a systematic strategy to cope with the patient`s medical problem as a whole. Biologics for psoriasis show an excellent treatment response than conventional agents. However, owing to the medical cost issue, selecting eligible patients who are suitable for psoriasis biologics is a hard issue for psoriasis doctor. In this lecture, I will discuss about the importance of selecting eligible psoriasis patients who should be considered using biologics for the patients` life-long healthcare. Also, some recent issues about psoriasis biologics will be discussed.

Lots of cytokines have been known to play important roles in the pathogenesis of psoriasis. Major pharmaceutical companies target these cytokines to develop more efficient and less harmful drug for psoriasis. Among the cytokines, interleukins are the main target for psoriasis biologics development, the most successful biologics for psoriasis is currently one of anti-interleukin agent. Until recently,IL-12/23 dual blocking agent is the most commonly used agent that blocks the pathway form naive T cells to Th1 or Th17 cells. IL-17A has been known to be a proinflammatory cytokine. In psoriasis, IL-17A plays main inflammatory reaction in the psoriatic lesion development. Recently, phase III clinical trials of IL-17A antagonist finished and reported improvement of PASI75 more than 70% of participated subjects. IL-23 is important in the differentiation from naive T cells to Th17 cells. In phase II clinical trials of IL-23 antagonist, it also showed great improvement of PASI75. In fact, the maximum PASI75 improvement rates of these newer agents are near 70%, not 100%. But the main reasons for the developing new biologics for psoriasis are the diminishing drug efficacy after long term use of a certain biologics, individual difference of response by the pharmacogenetic variety and different side effect profiles according to the agents.

N/A Background : Currant changing trends in dermatology practices push most dermatologists to manage their own homepage. But. most dermatologists do not know bow to construct and maintain their homepage effectively and efficiency. Objectives & Methods . By testing web usability of four dermatology clinic homepages of relatively large scale, we tried to give some insight of constructing homepages in the viewpoints of homepage users to dermatologists who have a plan m make their own homepage. Results : Four well-known dermatology clinic homepages are relatively well-planned and multi-purposed ones. However, they show weaknesses in the aspect of information architecture and the selection of th, themes of contents. Most of the online shopping malls constructed on their homepages show only primitive forms. Conclusion : For the effective operation a clinic homepage, a dermatologist as a planner and manager of homepage should have the concept of web usability. It homepages are built with the concept of web usability, they will be operated more fruitfully.
