        The purpose of this lecture is to provide information about safe use of medical lasers. In the U.S.A., there is a safety guideline provided by American National Standard Institute (ANSI Z136.3). In Korea, there is no forced regulation by law regarding medical laser safety, though guideline has been provided by the Korean Food & Drugs Administration.

        수해 후 피부질환에 대한 고찰

        N/A Background : In September 2002, typhoon RUSA brought heavy rains to pans of Kangwondo province. The river rose above the flood stage, and muddy water covered many houses. Along the damaged infrastructures, there were many patients who needed medical care. Among them, a significant portion had dermatologic problems. Objective : The aim of this study was to find out the relationship between the floods and skin diseases. The other purpese was to evaluate how the floods can affect the distribution of dermatoses. Methods : The 177 dermatology Patients who were flood victims were analyzed according to their sexs, age, and their diseases. Results : The results can be summarized as follows. Among 757 patients who were registered to special clinic for flood victims, 177 were seen in dermatology department. Although dermatology Patients increased compared to ordinary times, the proportion of them decreased compared to that of flood victims in northern Kyonggi-do in 1998. In the second group of patients who were seen from September 11th to 13th , the proportion of dermatology patients were increased to 23.0% compared to the first group of Patients who were seen from September 4th to 6th(12.6%). In age distribution, the most frequent age groups were 3rd decade(24.3%), 4th decade(35.9%), 5th decade(25.4%) amounting to 85.6% of the total dermatology patients. The ratio of male to female was 1:2.1. The four most common dermatoses were eczematous dermatoses, superficial fungal infections, pruritus & urticaria, and infectious dermatoses. The most commonly used medications were oral antihistamines and topical steroids. Conclusion : The most common skin diseases among flood victims were eczematous dermatoses. It was contradictory finding compared to the victims of northern Kyunggido floods in 1998 who showed superficial fungal infection most commonly. (Korean J Dermatol 2003;41(4) : 461~465)

      • Mini Lecture 1 : Treatment of pigmentation: a challenge to dermatologists

        As dermatologists, we face pigmentary disorders everyday. Sometimes, it is easy to remove, but sometimes it becomes darker in spite of all kinds of treatment. Moreover, there are all kinds of pigmentary disorders mixed in one patient. Therefore, it is very important for dermatologist to recognize different types of pigmentation and their response to treatment. By explaining which pigment will be removed easily and which pigment will last long, tremendous rapport with patients can be formed. Also, dermatologists should be prepared for unexpected postinflammatory hyperpigmentation and explain it to patients before treatment. Melasma is a common pigmentary disorder with therapeutic challenges. Due to postinflammatory hyperpigmentation, treatment of melasma with laser was contraindication, including Q-switched Nd-YAG laser. However, using low fluence with genesis technique made it possible to apply Q-switched Nd-YAG laser to melasma treatment. Its mechanism is based on the subcellular selective photothermolysis that low fluence Q-switched Nd-YAG laser treatment influences only melanosomes without destroying the melanocytes. Although laser toning is a treatment without downtime and major side effects, mottled hypopigmentation develop in some patients after repetition of the treatment. It can be very problematic in some patients because it does not recover very easily even after cessation of laser toning. To overcome this side effect, pulse-in-pulse IPL has been developed for treatment of melasma. IPL is very useful in treating freckles and lentigines. However, very subtle epidermal melasma may become evident after treatment with IPL. Since patients with melasma sometimes aggravate, IPL with conventional mode should be avoided in these patients. Pulse-in-pulse IPL decreased the risk of these side effects by subdividing pulse duration. Although there is no dramatic pigment removal effect, PIP IPL can be useful in melasma or postinflammatory hyperpigmentation. In this session, I will summarize common problems dermatologist face while treating pigmentary disorders and ways to overcome these problems.

      • Symposium 4-2-1 (SYP 4-2-1) : Case presentation: Partial unilateral lentiginosis treated with low-fluence repeated Q-switched Nd:YAG laser

        Partial unilateral lentiginosis (PUL) is a rare pigmentation disorder characterized by multiple lentigines that are grouped within an area of normal skin, often in a segmented pattern, that first appear at birth or in childhood (1). As a consequence of its early appearance, PUL often compromises one or more dermatomes, and, with time, the lesions usually become darker and more extensive. Histologically, PUL exhibits lentiginous epidermal melanocyte proliferation and rete ridge elongation, similar to the simple lentigines. Only a few reports have studied laser treatment of PUL and optimal treatment guidelines have not yet been fully established.

        에크린 혈관종성 과오종 2예

        Eccrine angiomatous hamartoma is a rare benign tumor similar to hemangioma in appearance. Unlike hemangioma, it has numerous eccrine structures in addition to numerous capillary channels. Occasionally it accompanies hyperhidrosis and pain. The sweat apparatuses in the lesions showed antigenic activities by immunohistochemical stains, particularly with anti-S100 protein and anti-CEA, the two common antigens associated with eccrine sweat glands. The antigenic activities of anti-S100 protein and anti-CEA in sweat gland structure of 2 presenting cases, one with sweating and the other without sweating, were compared to each other. As a result, we found no differences between the two cases. Although an aggressive treatment is not required due to its benign nature, total excision can be considered in cases with severe symptoms. We present two cases of eccrine angiomatous hamartoma developed on right knee joint areas in a 2-year-old male and a 10 month-old male, respectively.

        비전형적 피부섬유종의 임상적 특징과 더모스코피 소견에 관한 연구

        Background: Dermatofibroma is a common benign skin neoplasm that is usually easy to recognize; however, in some cases, it is difficult to differentiate it from atypical dermatofibroma. Atypical dermatofibroma is a rare variant of a more aggressive nature and requires complete excision due to its tendency to recur. Objective: To describe the clinical and dermoscopic features of dermatofibromas with special emphasis on the characteristics of atypical subtypes and to differentiate them from typical subtypes on the basis of results. Methods: We retrospectively searched the databases of patients of Korea University Ansan Hospital from January 2010 to December 2019 who underwent skin biopsies or excision surgeries and had pathological reports of dermatofibroma. Analyses of the clinical characteristics (age, sex, number of lesions, anatomical site) and dermoscopic patterns were performed. Results: Dermoscopic patterns were analyzed in 148 cases of dermatofibroma; peripheral delicate pigment networks and central white scar-like patches were the most common patterns found in 45 cases (30.4%). In eight cases of atypical dermatofibroma, three cases (37.5%) showed peripheral homogenous areas and central white networks, followed by multiple white scar-like patches in 2 cases (25.0%), peripheral delicate pigment networks, and central white scar-like patches in 2 cases (25.0%). Dermoscopy revealed a statistically significant difference in the peripheral homogenous area and the central white network pattern between the two groups. Conclusion: In this study, we analyzed the dermoscopic findings of 148 patients with dermatofibroma and found that the peripheral homogenous area and central white network patterns were significantly more in atypical dermatofibroma.         Basal cell carcinoma is a common cancer and has been reported to account for approximately 80% of non-melanoma skin cancers. Superficial basal cell carcinoma occurs mainly in the trunk or limbs and is characterized by clinical features of scaly erythema with or without epidermal atrophy. These clinical features require differentiation from those of other benign and malignant skin diseases. Recently, specific dermoscopic findings of basal cell carcinoma have been helpful for diagnosis. Common dermoscopic findings of superficial basal cell carcinoma include superficial telangiectasia and maple leaf-like areas. Herein, we report a case of superficial basal cell carcinoma that was not diagnosed by punch biopsy but by dermoscopy. (Korean J Dermatol 2020;58(6):419∼422)

        초음파 자극기 Part II; 초음파 자극기 사용의 부작용에 대한 고찰

        An ultrasonic device for medical skin care is used to gently exfoliate the skin of dead skin cells and impurities. Ultrasound waves help loosen and remove oil, dirt and cellular debris from pores. Ponophoresis or sonophoresis is the use of ultrasound to increase percutaneous absorption of drugs, serums, lotions, creams and masques to a depth where it is more effective and beneficial. For improved results and safer procedures, we need to know the contraindications and adverse effects of ultrasonic treatments. Therefore, we summarized and reviewed the issue of safety including contraindications, cautions, and adverse events.

        콜히친으로 치료한 성인에서의 재발성 특발성 호중구성 한선염 1예

        Idiopathic neutrophilic eccrine hidradenitis is usually observed in healthy children with tender erythematous nodules on the soles or palms. We report a case of a 26-year-old man with a painful erythematous swollen patch on both hands and feet. His symptom recurred once monthly over 2 years; thus, he visited Orthopedics. However, no bony abnormality was identified. We performed a skin biopsy on an erythematous indurated patch on his right sole. Histopathological findings indicated neutrophilic eccrine hidradenitis. After 2 weeks of treatment with oral colchicine, his symptom resolved, and no relapses were observed during follow-up. Only a few case reports describe idiopathic neutrophilic eccrine hidradenitis in adults, which is not associated with malignant hematological disease, solid cancer, and chemotherapy. Conventional treatment comprises bed-rest and the administration of topical steroids. Colchicine can be a good treatment option for adults with severe or recurrent idiopathic neutrophilic eccrine hidradenitis. (Korean J Dermatol 2018;56(5):322∼324)
