        구개상의 두께에 따른 한국어 자음의 발음 변화에 관한 컴퓨터 분석 - 치조음, 경구개음-

        This study was carried out to investigate the sound distortion following the alternation of the palatal plate thickness. For this study, 2 healthy male subjects (24-year-old) were selected. Born in Seoul, they both spoke Seoul dialect. First, their sounds of /na(나)/, /da(다)/, /1a(라)/, /ja(자)/, /cha(차)/, /ta(타)/, without inserting plates were recorded, and then the sounds with palatal plates of different thickness were recorded, successively. The plate was fabricated in 3 types, each palatal thickness being 1.0mm, 2.5mm, dentoalveolar portion 2.5mm, other residual portion was 1.0mm, successively. Each type plates named B, C, D-type, in succession. Series of analysis were administered through Computer(16 bit) to analyze the sound distortions. These experiments were analyzed by the LPC (without weighting, pre-weighting, post-weighting) of the consonants, vowels portion, formant frequency of the vowels and word duration of the consonants. The findings led to the following conclusions: 1. There was no correlation of the distortion rate on the 2 informants. 2. Generally, vowels were not affected by the palatal plate thickness in the formant analysis, however, more distortion was detected in the LPC analysis, especially C, D-type plates. 3. Consonants distortion was more evident in the C, D-type plate. 4. The second formant was most disturbed and reduced in the all consonants with insertion of the palatal plate, especially C, D-type plate. 5. Word duration was shortened in the plate inserted(except /ja/, /cha/), especially C, D-type. 6. It was found that dentoalveolar, hard palatal sounds were severely distorted in plate inserted, and they were mainly affected by the dentoalveolar portion thickness. 7. There was correlation between palatal thickness and consonants quality.

        고정성 보철치료에서 골유착성 임프란트의 경사도변화에 따른 변위와 응력에 관한 유한요소적 연구

        The purpose of this study was to analyse the deflection and stress distribution at the supporting bone and it's superstructure by the alteration of angulation between implant and it's implant abutment. For this study, the free-end saddle case of mandibular first and second molar missing would be planned to restore with fixed prosthesis. So the mandibular second premolar was prepared for abutment, and the cylinder type osseointegrated implant was placed at the site of mandibular second molar for abutment. The finite element stress analysis was applied for this study. 13 two-dimensional FEM models were created, a standard model at $0^{\circ}$ and 12 models created by changing the angulation between implant and implant abutment as increasing the angulation mesially and distally with $5^{\circ}$ unittill $30^{\circ}$. The preprocessing decording, solving and postprocessing procedures were done by using FEM analysis software PATRAN and SUN-SPARC2GX. The deflections and von Mises stresses were calculated under concentrated load (load 1) and distributed load(load 2) at the reference points. The results were as follows : 1. Observing at standard model, the amount of total deflection at the distobuccal cusp-tip of pontic under concentrated load was largest of all, and that at the apex of implant was least of all, and the amount of total deflection at the buccal cusp-tip of second premolar under distributed load was largest of all, and that at the apex of implant was least of all. 2. Increasing the angulation mesially or distally, the amounts of total deflection were increased or decreased according to the reference points. But the order according to the amount of total deflection was not changed except apex of second premolar and central fossa of implant abutment under concentrated load during distal inclination. 3. Observing at standard model, the von Mises stress at the distal joint of pontic under concentrated load was largest of all, and that at the apex of implant was least of all. The von Mises stress at the distal margin of second premolar under distributed load was largest of all, and that at the apex of Implant was least of ail. 4. Increasing the angulation of implant mesially, the von Mises stresses at the mesial crest of implant were increased under concentrated load and distributed load, but those were increased remarkably under distributed load and so that at $30^{\circ}$ mesial inclination was largest of all. 5. Increasing the angulation of implant distally, the von Mises stresses at the distal crest of implant were increased remarkably under concentrated load and distributed load, and so those at $30^{\circ}$ distal inclination were largest of all.

        니켈-크롬 합금 보철물 주위 치은 열구내에서 발견된 니켈 내성 균주에 관한 분자생물학적 연구

        As a material of metal-ceramic prosthesis, nickel as a form of Ni-Cr alloy has been used for many dental prostheses in many cases. However, several problems in use of the alloy have been revealed (ex ; tissue stimulation, skin allergy, hypersensitivity cytotoxicity and carcinogenecity). Little is known about nickel with respect to the relationship between Ni-prosthesis and gaining of Niresistance in oral microorganisms. The present study was undertaken to check whether use of Ni-prosthesis leads to occurrence of Ni-resistant microorganisms. So this study may suggest the possible relationships between the oral microorganisms and nickel-resistance in oral environment. Bacteria were isolated from the gingival crevicular fluid on the patients wearing Ni-Cr prosthesis. The isolated bacteria were tested fir their Ni-resistance in nickel containing media at different concentration from 3mM to 110mM. E. coli HB101 was used as control. The Ni-resistant bacteria were isolated and biochemically identified. The Ni-resistant bacteria were tested several biochemical, molecular-biological tests. Performed tests were : measuring the growth curve, antibiotic test, growth ability test in liquid media, isolation of the chromosome and plasmid, digestion of DNA by restriction enzyme, electrophoresis of chromosome and plasmid DNA, identification of Ni-resistant genes by the DNA hybridization. The results were as follows 1) The bacteria isolated from gingival crevicular fluid on the patients wearing Ni-Cr alloy prosthesis showed nickel-resistance. 2) The isolated microorganisms grew at nickel containing media of high concentrations (60mM-110mM). 3) Based on the biochemical tests, the isolated microorganisms were identified as Enterococcus faecalis(13 cases), Klebsiella pneumoniae(1 case) and Enterobacter gergoviae(1 case). 4) Enterococcus faecalis expressed not only nickel resistance but also the multi-drug resistance to several antibiotics ; chloramphenicol, kanamicin, streptomycin, lincomycin, clindamycin, However, all strain showed the sensitivity against the tetracycline. 5) DNA hybridization result suggests that there is no homology between the previously known gene of nickel resistance in Klebsiella pneumoniae and chromosomal DNA of Enterococcus faecalis.

        금속하부구조물이 도재의 색조에 미치는 영향에 대한 분광측색분석

        Statement of problem Metal-ceramic restorations have been used extensively by dental clinicians for nearly 40 years. Strength an functional ability of metal-ceramic restorations are proved to be satisfying, However esthetics and biocompatibility of metal alloy which is used in metal-ceramic restoration is not ideal. Using pure gold as an alternative, have advantage of esthetics, biocompatibility over conventional metal alloy. But there had been little article which studied on the color effect of pure gold on fual porcelain color. Purpose The purpose of this study was to spectrophotometrically evaluate the difference between color of metal alloy(Au-Pt, Ni-Cr) and pure gold, during color masking procedure with opaque porcelain and to analyze the differences, Material and Methods Three types of metal - base metal(Ni-Cr), high gold alloy(Au-Pt), pure gold(GES) - specimen were fabricated 1cm in diameter. Four steps were established - after finishing, after pre-coditioning, after application of first opaque porcelain(0.08mm in thickness), after application of second opaque porcelain(0.15mm in thickness)- and tested color with spectrophotometer every each steps and analyzed with $CIEL^*a^*b^*$ color order system. One-Way ANOVA test was used to and out if there were significant differences between groups tested and Shaffe multiple comparison was used to identify where the differences were. Results 1. After finishing and pre-conditioning, pure gold(GES) group showed most high values in $L^*,a^*,b^*$. 2. After application of first opaque porcelain(0.08mm in thickness), after application of second opaque porcelain(0.15mm in thickness), pure gold(GES) group showed the least difference in $L^*,a^*,b^*$ values and the lowest ${\Delta}E$ value(${\Delta}E$=0.63). 3. After application of first opaque porcelain and after application of second opaque porcelain differences that were significant (P<0.05) between groups were found only in $a^*$ values. 4. Base metal alloy group showed the lowest $a^*$ value in test after application of first opaque porcelain and the highest value in test after application of first opaque porcelain Conclusion Pure gold group and high gold group showed higher $a^*$ values than base metal group when tested after 0.08mm thickness of opaque porcelain was applied and pure gold group showed much similar $L^*,a^*,b^*$ values between 0.08mm thickness and 0.15mm thickness of opaque porcelain. This meant that pure gold was more easily masked by opaque porcelain than the other two groups.

        정상 한국인의 하악 전방운동시 시상과로각과 절치로각에 관한 연구

        Mandibular movements are guided mainly by three determinants, namely the two posterior controls (the Temporomandibular joints) and the anterior control (the incisal guidance). The aim of this study was to evaluate the incisal guide angle in effort to reconcile a patient's condylar guide angle and incisu guide angle, to develop criteria for incisal guidance in clinical practice. 48 subjects (male 33, female 15) with intact intercuspation, without past history and symptoms of stomatognathic system, were selected for this study. All of the subjects had not anterior prostheses. The sagittal condylar guide angles and incisal guide angles were measured and estimated statistically by cephalogram and articulator (Whip-Mix 8500A). The results of this study were as follows: 1. Average of condylar guide angle was 43.33 degree by cephalogram, and was 35.18 degree by articulator. 95% confidence interval was from 40.43 to 46.23 degrees in cephalogram and was from 32.98 to 37.38 degrees in articulator. 2. Average of incisal guide angle was 51.51 degree by cephalogram, and was 44.11 degree by articulator. 95% confidence interval was from 49.12 to 54.95 degrees in cephalogram and was from 40.67 to 47.56 douses in articulator. 3. Difference between condylar and incisal guide angle was 8.18 degree by cephalogram, and was 8.94 degree by articulator. 95% confidence interval was from 4.61 to 11.74 degrees in cephalogram and was from 4.90 to 12.98 degrees in articulator. 4. In case of the incisal guide angle steeper than condylar guide angle, subjects were 69% (33 of 48) in cephatogram and 75% (36 of 48) in articulator. 5. By the multiple regression equation, condylar guide angle was more influenced by the anterior teeth. 6. When the mandible protrudes from the intercuspal position to the edge to edge position the incisal linear movement was 4.18mm (S.D.:1.30mm) and the condylar linear movement was 4.38mm (S.D.:1.26mm).

        지르코니아 전부도재관 지대치 삭제시 축면 경사각과 만곡 반경에 대한 조사

        연구 목적: 지르코니아 보철물을 위한 치아 형성이 실제 임상에서 어떻게 이루어지는지에 대해 조사하고 향후 치아 형성에 대한 교육 자료 및 추가 연구의 기초 자료로 활용하고자 한다. 연구 재료 및 방법: 지르코니아 치관 제작을 위해 milling center에 보내진 모형을 조사하여 Lava CAD/CAM System으로 스캔 한 후 'ImageJ' 프로그램 상에서 측정하여, 이를 비교 분석하였다. 각 치아마다 근원심면과 협설면에서의 축면 경사각을 측정하였다. 또한, 견치를 제외한 전치에서 절단연 0.4 mm 하방 부위의 bucco-lingual diameter 측정하였다. 결과 및 결론: 실험군 내에서 지르코니아 전부도재관을 위한 지대치의 축면 경사각은 $18.67^{\circ}$로 문헌에서 추천하는 각도 보다 크게 나타났다. 지르코니아 완전도재관의 지대치 형성 시 축면 경사각은 협설과 근원심면, 악궁 내 위치에 따라 차이가 있었다. Peak 0.4의 평균값은 1.18 mm로 밀링버의 최소직경을 고려한 요구조건인 0.8 mm 보다 큰 값으로 나타났다. 그러나 선각 부위 rounding 에 관한 추가적인 연구가 필요할 것으로 생각된다. Purpose: This article attempted to examine how teeth for restoration is made in a clinical practice and utilize it as future educational material of teeth formation and basic data for additional research. Materials and methods: This experiment investigated the models sent to milling center for production of zirconia crowns. After scanned with Lava CAD/CAM System (3M ESPE, Seefeld, Germany), they are measured on 'ImageJ (version 1.32j, National Institutes of Health, USA)' program and compared and analyzed. Convergence angle from mesio-distal surfaces and bucco-lingual surfaces of each teeth are measured. Also, bucco-lingual diameter of the region lowered as much as 0.4 mm from incisal edge in anterior teeth except canines.(This measure is defined as the Peak 0.4) The analysis of data between each group was conducted by Windows SPSS statistic program, and was proved significant on 95% confidence level by independent t-test, one-way ANOVA and multiple analysis (Sheff${\'{e}}$ test). Results: The mean value of convergence angle was $18.67^{\circ}$ It is ranked as molar ($26.70^{\circ}$) > premolar ($16.87^{\circ}$) > anterior teeth ($14.81^{\circ}$) in the order of mesio-distal convergence angle; anterior teeth ($22.32^{\circ}$) > molar ($20.93^{\circ}$) > premolar ($15.41^{\circ}$) in the order of bucco-lingual convergence angle. The mean value of Peak 0.4 was 1.18 mm. Conclusion: Convergence angle of abutment of zirconia all ceramic crown has difference depending on the location in the arch. Due to the nature of production of zirconia all ceramic crown, convergence angle of abutment and line angle finishing degree can have an effect on internal suitability of restoration.

        치과용 석고계 매몰재의 특성에 관한 연구

        The properties of a investment material can be described by the consistency at the slurry state, the setting time, the compressive strength and the thermal expansion during the casting. In this study the effect of the production parameters which are included the ratio of quartz and cristobalite, the content of binder, the water powder ratio and the content and concentration of additives on the properties of the gypsum-bonded investments has been investigated with help of the consistency test, the vicat needle test, the compressive strength test, the thermal expansion test, x-ray diffraction and DTA thermal differential test. The experimental results showed that the constitution of a investment with W/P ratio of 0.34, 30% of gypsum, 0.8% aluminium sulfate, 2% magnesium sulfate, 0.6% sodium phosphate was adapted for the properties of the KDA Spec. No. 13 type I investment. The important experimental results are summarized as follows. 1. The consistency of the investment decreased with increasing amount of aluminium sulfate and decreasing amount of sodium phosphate. An addition of magnesium sulfate up to 2% an increase of the consistency was shown. But 3% magnesium sulfate in investment showed a decrease of consistency. The consistency did not vary significantly with a variation of the content of gypsum and cristobalite and the W/P ratio. 2. Aluminium sulfata and the magnesium sulfate promoted the hardening and the aluminium phoshpate delayed the hardening. The setting time increased with amount of gypsum. The effect of the matrix on the setting time was insignificant. With the W/P ratio of 0.34 the setting time was 14 min. 3. The compressive strength decreased with the amount of aluminium sulfate up to 0.25% and increased with the amount of aluminium sulfate greater than 3%. The compressive strength decreased as decreasing the amount of magnesium sulfate and gypsum and as increasing the W/P ratio. The effect fo the refractory on the compressive strength was also not significant. With the W/P ratio of 0.34 the compressive strength was $34Kg/mm^2$. 4. The 1st thermal expansion was found at the temperature near $250^{\circ}C$ and the steady state or the contraction stage was found at the temperature between $250^{\circ}C$ and $500^{\circ}C$. After this stage the 2nd thermal expansion took place at the temperature near $500^{\circ}C$. The amount of thermal expansion increased with decreasing the content of magnesium sulfate, aluminium sulfate and gypsum and the W/P ratio. And the amount of thermal expansion increased as the content of sodium phosphate ad cristobalite. With the W/P ratio of 0.34 the amount of total expansion was 1.2%.

        지속적인 재평가를 통해 최적화된 임시 보철을 최종 보철로 이행한 전악 CAD/CAM 고정성 수복 증례

        보철적 의사 결정(decision-making)은 다양한 요소가 작용하므로 복잡하며, 치과의사가 분석한 자료를 바탕으로 환자와의 소통을 통해 이루어진다. 다양한 치료 옵션과 그에 따른 결과의 경우의 수가 많아짐에 따라 이러한 의사 결정 과정의 지속적인 재평가가 요구된다. 본 증례는 교합평면의 부조화를 보이는 환자의 교합평면을 재형성하는 치료 방법으로 고정성 보철 수복을 선택하고, 진단 납형 제작을 통한 치료계획의 수립으로 교합평면을 재설정하였다. 충분한 기간 동안 사용하면서 지속적인 재평가를 시행하여 얻은 임시 보철물을 CAD/CAM (Computer-aided design/computer-aided manufacturing)을 이용하여 지르코니아 최종 보철물로 이행하였다. 치료 종결 후 3개월 간의 주기적 내원을 통해 관찰하였을 때 만족스러운 결과를 얻었다. Prosthetic decision-making is complex because of various factors, and involves a combination of the individual dentist's interpretation of the objective clinical data and his or her interaction with the patient. Increasing therapeutic options and emerging outcome data demand the constant re-evaluation of our decision-making process. In this case, fixed prosthetic restorations were selected as a treatment method to reconstruct the occlusal plane of a patient with disharmonious occlusal plane. And the occlusal plane was re-established by establishing a treatment plan through diagnostic wax-up. Provisional restorations obtained by continuous re-evaluation for a sufficient period of time was replicated to definitive restorations using CAD/CAM technology. The results were satisfactory when they were observed through clinical follow-up for 3 months after the end of treatment.
