        Objective: Human adenovirus type 55 (HAdV-55) is an emerging pathogen causing outbreak of pneumonia in Korean military since 2014. Since the vaccines are developed yet, nonpharmacological interventions (NPIs) including environmental disinfection may play a key role in controlling the outbreak. To evaluate effect of environmental disinfection, we conducted a randomized controlled trial in the Korea Army Training Center. Method: The study population included 60 companies of five regiments. The companies are divided into environmental disinfection company (intervention arm) and control company (control arm). The environmental disinfection company performed disinfection of environmental surfaces using 70% alcohol-based disinfectant and 2,000ppm sodium hypochlorite. The disinfection procedures were performed at the beginning and end of training. The control companies maintained routine hygiene procedures. For primary outcome, the number of patients with fever or acute upper respiratory infection per week is evaluated. Results: Study period included March 11, 2019 to June 9, 2019 and pre-intervention period included January 7, 2019 to April 14, 2019. The average numbers of weekly patient visit were 42.51 ± 9.21 persons in the intervention arm and 45.12 ± 9.38 persons (P-value = 0.281) in the control arm. Compared to the pre-intervention period, the average numbers of weekly patient visit during the intervention period were weakly decreased without statistical significance [2.1 ± 10.4 persons in the intervention arm and 7.45 ± 13.28 persons in the pre-intervention (P-value = 0.091)]. Conclusion: Although there is no statistical significance, fever or acute upper respiratory infections was reduced by intervention (environmental disinfection). Further defined controlled investigations for the effect of NPIs in controlling outbreaks of acute upper respiratory tract infection including HAdV-55 is needed.

      • SARS-CoV-2 감염으로 인한 폐렴의 위험인자: 후향적 코호트 연구

        Objective; We retrospectively analyzed 310 patients with SARS-CoV-2 infection who were admitted to the Armed Forces Daegu Hospital to report the clinical characteristics of COVID-19 and the risk factors for pneumonia caused by SARS-CoV-2 infection. Method; This was a single center, retrospective cohort study. Clinical information of 312 COVID-19 patient was obtained by electronic medical record review. Clinical differences between the pneumonia group and without pneumonia group were analyzed. Results; Altogether, 312 COVID-19 patients were admitted to the AFDH from March 5 to April 19 and 310 were retrospectively analyzed. Compared with the without pneumonia group, the pneumonia group showed statistically significant differences in age. BMI, smoking history, underlying disease Conclusion; Risk factors for pneumonia in these patients were old age, underlying disease, smoking history, and obesity.

      • 코로나-19 집단발병 사례의 임상적 특징 분석

        Background; Following the first confirmed diagnosed case on January 19, 2020, a cluster outbreak within a religious group occurred in Daegu city, South Korea, in February 2020. As of February 29, 2020, the number of patients related to the religious group had increased to 1,557 (53.1% of total cases), immediately followed by large-scale community spread that overwhelmed the city's medical surge capacity. Method; We collected clinical medical records, nursing records, laboratory findings, and radiology results for 44 patients (23 in the religious group, 21 from community transmission) with coronavirus disease (COVID-19) transferred to Armed Forces Daejeon Hospital between February 25 and March 25, 2020. Results; The median patient age was 53 years, and 20 patients were men. The primary outcome, de-isolation and discharge, was confirmed in 32 patients. The median time to de-isolation was 10 days, and there was no difference between the community transmission and religious groups. The most common symptom was fever (20.5% on admission and 56.8% during hospitalization), followed by cough (70.4%), sputum (56.8%), and myalgia (40.9%). Ten patients experienced anosmia or hyposmia. The median incubation period was 5 days. On admission, 28 patients showed abnormal radiologic findings. The most common pattern on computed tomography scans was ground-glass opacity (67.5%). Neutropenia, lymphocytopenia, and thrombocytopenia were present in 13.6%, 45.4%, 13.6% of patients, respectively, on admission. Conclusion; Among 44 patients with COVID-19, 41 showed mild respiratory tract infection symptoms. Of these, three required oxygen therapy and recovered and 32 were discharged within a median of 24 days without any complications.

      • 한국군 코로나19 감염자의 초기 역학적 특성

        Introduction; South Korea is one of the exemplary countries that have successfully responded to COVID-19 pandemic with the South Korean military having only a small number of confirmed patients. This study aimed to provide basic data for establishing a strategy in response to COVID-19 in the future by analyzing the epidemiological and clinical characteristics of patients in the Republic of Korea Armed Forces military. Method; The materials were collected through electronic medical records and telephone interviews, while descriptive analysis was performed for the statistical analysis of clinical and epidemiological characteristics. Result; From February 20 to September 2, 2020, 110 patients were confirmed cases of SARS-CoV-2 infection in the ROK military. The number of confirmed cases due to cluster infection was 63.6%, of which 62.9% were enlisted. The prevalence of COVID-19 in the ROK military was 17.7 per 100,000 persons. The median age was 22 years, and 94.5% of patients were men. The primary infection rate was 59.4%, followed by 34.2%, and 5.4%. The common symptoms were fever and cough during the entire hospitalization period. Moreover, radiological diagnosis of pneumonia was confirmed in 17.3% of patients. The incubation period was five days, and there was no deceased case among the confirmed cases; hence, the case fatality rate was 0%. Conclusion; The timing and region of the outbreak in patients in the military was affected by the pattern and status of confirmed cases in the entire country. Enlisted were more vulnerable and affected mainly by cluster infection. The military response to COVID-19 has been effective so far, while the military can prevent the spread of COVID-19 by implementing a stronger quarantine system to preserve combat power.
