방사선 치료 중인 암환자의 주관적 행복감 향상을 위한 탐색적 연구

본 연구는 방사선 치료 중인 암환자의 주관적 행복감을 향상시키기 위한 서비스를 탐색하기 위한 것이다. 백 한 명의 방사선 치료중인 암환자를 대상으로 설문조사하여 주관적 행복감에 영향을 줄 수 있는 자기의식과 증상 불편감을 파악하여, SPSS 20.0 프로그램을 이용하여 독립표본검정, 일원분산분석, 피어슨 상관분석을 시행하였다. 방사선 치료중인 암환자의 불편감은 피로감, 외모변화, 현기증, 식욕저하, 수면장애의 순으로 높게 나타났으며, 암환자의 주관적 행복감은 증상불편감이 낮을수록, 그리고 개인의 내면적인 의식이 높을수록 향상됨을 나타냈다. 따라서 방사선 치료기간 동안 주관적 행복감을 향상시키기 위해서는 증상불편감은 감소시키고, 개인의 의식수준을 향상시킬 수 있는 신체적 심리적 정서적 측면을 고려한 통합적 서비스가 필요하다. 이를 위해서는 기존의 서비스 프로그램의 보완과 환자의 편의를 고려한 치료병원과 지역암센터나 보건기관을 연계할 수 있는 시스템이 준비되어야 할 것이다. This study is to find a service that improves the subjective happiness of cancer patients that receive radiation therapy. One hundred and one cancer patients undergoing radiation therapy were enrolled in the survey to verify the self-consciousness and symptom distress that could affect the subjective happiness. Independent t-test, one-way analysis of variance, and Pearson correlation were conducted with the SPSS 20.0 program. Symptom distress of cancer patients undergoing radiation therapy showed highest in the order of fatigue, appearance changes, dizziness, loss of appetite, and sleep disorders. And lower symptom distress, higher private self-consciousness were significantly correlated with higher subjective happiness among patients receiving radiation therapy for cancer. Study findings reflect the necessity of an integrative approach combining physical, psychological and emotional interventions during a radiotherapy period. For this cause, it would be necessary to supplement previously existing service programs and organize a patient-friendly system that could affiliate regional cancer centers or Public health centers with the hospitals in which the patients are being treated.

This study was performed to understand the characteristics and the meaning of the illness experience of breast cancer survivors as basic data for the development of an intervention program. Methods: The participants were 25 breast cancer survivors who had completed treatment at a tertiary hospital in Seoul. Data were collected through in-depth and unstructured audio-recorded interviews by the investigator. The participants were asked to describe their illness experience. The data were analyzed according to Giorgi`s method for phenomenological analysis. Results: The interview data were organized by theme into 6 categories that emerged from the analysis. The themes were acceptance of the illness, active coping with reality, gaining strength through the support of surrounding people, struggling to overcome a negative mindset, self-reflection, and the pursuit of a meaningful new life. Conclusion: We recommend the development of a survivorship program based on self-reflection, which can engender self-transcendence and spiritual well-being.

Purpose: The study was conducted to find out clinical characteristics for coronary artery disease patients with diabetes mellitus. Methods: We retrospectively reviewed the electronic medical records which included the data of 6,792 patients, who had been diagnosed coronary artery disease (CAD) such as angina or acute myocardial infarction and admitted to a university hospital in Seoul from January, 2005 to November, 2010. Results: Of the 6,792 patients, 43% had been diagnosed diabetes as comorbidity. The CAD patients with diabetes had lower left ventricular ejection fraction, stayed longer at hospital, and spent on more time from the first symptom to hospital visit than those without diabetes. In addition, they were more likely to have multi vessel coronary artery disease. Conclusion: The CAD patients with diabetes lay on the various factors which can make more worsen condition. Hence, we need to pay attention to specialized nursing care and patient education for the CAD patients with diabetes.

One of the greatest challenge that the world's societies are confronted with is the growing ratio of aged to total population. Aging population has multiple impacts on individual, and family lives and on the society as a whole. With aging, man experiences more health problems together with many social and psychological problems that are unique to an old age and functional to cultural variations. It should be an cooperative venture among individuals, families, professionals and society as a whole to help alleviate the problems of aged in each society. Nurses meet aged more frequently in recent years in their practice settings: Good gerontological nursing care should take account these multiple aspects of aging. Education of nursing student in gerontological nursing will not only enhance the quality of the care of the aged, but also is the most important part contributing for the development of the field of gerontology. Currently gerontological nursing is included in undergraduate nursing curricula in all 16 baccalaureate nursing programs that were surveyed. Six schools of nursing have established separate subject on gerontological nursing. For the rest of schools, gerontological nursing content are integrated into traditional nursing subjects in various degrees. The result of our survey and literature review as the background, we propose that gerontological nursing curriculum integrated in undergraduate nursing curricular in baccalaureate schools of nursing in Korea. The proposed principles of curriculum building in gerontological nursing is as follows : 1) The course of study in gerontological nursing should be developed in combination of a separate course and integrated courses. 2) For the effectiveness of study, the subject of gerontology as a discipline including the general characteristics of aging is developed in a separate course, and implication of aging in various practice fields will be integrated into current courses of study as applicable. 3) A parallel gerontological nursing clinical practicum should be developed as an integral part of current fields of practice. Students learn about normal aging by experiencing well elderly in the family and community setting in sophomore year to foster positive attitude toward aged and agings. In junior and senior years, the practicum include the care of ill and well elderly in institutions of care and as an integral part of field experience in community nursing.
