        사암침법의 정신과적 활용에 대한 질적 사례연구 -한 의료기관에 대한 현장연구-

        Objectives: To investigate validity and effectiveness of Saam five element acupuncture treatment in psychological problems. Methods: We selected a Korean medicine doctor known to apply Saam five element acupuncture treatment to the psychiatry field. An investigator visited the doctor’s clinic to observe the cases and interview patients. Duration of each interview was 30 minutes to one hour. A semi-structured questionnaire was used for in-depth interviews. All interviews were recorded and converted into texts. Converted texts were then summarized and analyzed qualitatively. Results and Discussion: Treatment process of Saam five element acupuncture designed for psychiatric use is divided into four stages: qi transformation (氣化), acupuncture treatment, deep breathing, and acupressure. In the first stage, patients are asked to express their feelings and emotions in a physical sense; to imagine their feelings have colors, weight, and physical form, and to describe those characteristics. Next, the doctor evaluates each patient’s qi according to the six qi (六氣), and then determines protocol of Saam five element acupuncture treatment. This treatment is focused on suffering of the patient and specific symptoms or emotions. The treatment duration is brief and changes in symptoms are frequently checked during treatment sessions. Without discussing details of stressful events, the doctor enables patients to express physical aspects of their feelings. He also provides them with psychoeducational resources. As a result, patients experienced immediate changes in body sensation, emotions and thinking, decreased response to triggers, and increased self-control after treatment. Conclusions: The Saam five element acupuncture treatment has the potential to be used as a treatment for psychiatric disorders. Treatment can be effective in managing psychiatric disorders when applied by primary Korean medicine clinics.

        화병의 한의학적 치료에 대한 연구동향

        Objectives: To determine the general characteristics of clinical studies about Hwabyung and assess their limitations and alternatives. Methods: Clinical studies that examined the effects of traditional Korean medicine intervention on Hwabyung were included in this study. A systematic search of English, Chinese, Japanese, and Korean databases was performed. The characteristics of included articles were described and those articles were assessed by Risk of Bias (RoB) tool or Risk of Bias for Nonrandomized Studies (RoBANS) tool. Results: Sixteen articles were selected from 1,826 articles. Most clinical studies about Hwabyung were published in Korea. The number of conducted trials was insufficient. The prevailing study design was randomized controlled trial. Traditional Korean medicine intervention used in the trials were acu-puncture, herbal medicine, counselling, meditation, emotional freedom technique (EFT), music ther-apy, art therapy, and multi intervention program. Herbal medicine study used placebo as control while non-pharmacological intervention study mostly used no treatment as control. Most of the trials were supported by the government. Therefore, financial conflict of interest might not exist for results. We judged that some studies had a high risk of bias. In general, most of the studies with a high risk of bias were non-pharmacological intervention studies, and the risk of bias was mainly due to lack of blinding. Conclusions: More clinical studies of Hwabyung are needed. There are some issues about a suitable comparison and effective blinding strategy for non-pharmacological study. Improving methodo-logical quality is required.

        화병(火病) 유사 병증의 한의학적 치료에 대한 임상연구 동향분석 - CNKI를 중심으로

        Objectives: To analyze whether a concept similar to Korean ‘Hwabyung’ exists in China. We investigated the status of clinical studies conducted in China for relevant diseases and trends of the traditional Chinese medicinal (TCM) treatment adopted in clinical studies. Methods: To explore the concept of a condition similar to Korean Hwabyung in China, we searched for the existence of concept and pattern that were parallel or similar to those of Hwabyung in TCM textbooks and diagnostic guidelines. We searched and analyzed clinical studies of TCM treatment for depression syndrome similar to Hwabyung from CNKI using terms 'depression' and 'qi stagnation transforming into fire'. Using extracted data, characteristics of clinical research, herbal medicine, and acupuncture treatment used in the clinical research and their effects were systematically reviewed. Results: Symptoms of 'qi stagnation transforming into fire' were most similar to those of Hwabyung. Nine articles were selected from a total 258 articles. Most of them used DSM-IV or CCMD-3 for depression diagnosis. They applied 'diagnostic and efficacy guidelines for TCM diseases and syndromes' for pattern diagnosis of 'qi stagnation transforming into fire'. Danzhixiaoyao-san and Jiaweixiaoyao-san were found to be effective when they were used alone or in combination with antidepressants. Acupuncture treatment also showed remarkable effect on LR3, HT7, LI4, PC6, GV20 when it was used alone or in combination with antidepressants. However, careful interpretation is required because a small number of studies are included. Conclusions: 'Qi stagnation transforming into fire' seemed to have symptoms similar to Hwabyung. However, further research is needed to determine its diseases and pattern types compared to Hwabyung. According to included studies of 'depression with Qi stagnation transforming into fire', herbal medicine and acupuncture treatment tended to be effective in relieving depressive symptoms. However, more discussion is required for future application of herbal medicine and acupuncture for treating Hwabyung.

        성인 중증 지적장애에서의 문제행동에 대한 억간산가미(抑肝散加味) 치험 1례 보고

        This case report presents a 31-year-old male intellectual disability patient with problem behavior such as food refusal and temper tantrums. He was treated with the Korean traditional herbal medicine (Yokukan-san-gami) continuously for 56 days. The effects of the treatment were measured by the frequency of his problem behavior and the score of the Korean version of Aberrant Behavior Checklist (ABC). After treatment, his challenging behavior became reduced, and his ABC score had been decreased by 24.6%. The findings from this case suggest that the Korean traditional herbal medicine (Yokukan-san) could be effective for challenging behavior in patients with intellectual disabilities.

        ‘기공’의 국외 임상연구 최신동향: 스코핑 고찰을 중심으로 한 예비연구

        Objectives: The objective of this study was to perform a scoping review to describe clinical study trends on Qigong in order to identify further directions of Qigong in Korean medicine. Methods: Under the Arksey and O’Malley methodological framework, PubMed was searched to identify articles published from January 1, 2019 to June 28, 2021. A total of 224 articles were retrieved. Results were systematically filtered by two independent reviewers based on inclusion/exclusion criteria. Publication information, disease, intervention and research results of a total of 153 articles were extracted and analyzed. Results: Asia had the largest number of Qigong studies (82 studies, 65%). Most research studies were conducted in the academic field of Medicine (n=109, 86.5%), including Complementary and Alternative Medicine (n=35), Medicine (miscellaneous) (n=15), and Oncology (n=15). Based on ICD-10 classification, Mental and Behavioral Disorder (n=25, 19.8%) was the most frequently analyzed decease, followed by Neoplasm (n=24) and Disease of the Nervous system (n=12). Almost half of all studies were Systematic Reviews. RCTs only accounted for 25.4%. Interventions were very diverse and inconsistent. Sixty (47.6%) studies analyzed Qigong as a single intervention. In 66 cases, Qigong was analyzed as part of a large category such as Mind-Body intervention. Most studies designed a Donggong (動功) program. The age of the population was relatively high as 47.1% of all studies were conducted on middle aged or older adults. Conclusions: These findings suggest that further standardized research on Qigong, especially Junggong (靜功), needs to be conducted by developing research protocols and practice programs to verify effects of Qigong and utilize Qigong as a medical intervention in Korean Medicine.

        한국 노인이 겪는 우울감 및 자살사고에 대한 질적 연구 분석

        Objectives: To analyze the reports of qualitative research about depressed mood and suicidal ideation of Korean Seniors. Methods: Eleven published qualitative research studies were selected for analysis. We extracted common topics and factors of each stage, Selected factors were paralleled and synthesized to make a flow chart of the phenomena. Results: Depressed mood arousals of Korean seniors are categorized by causation factors, which include economic, psychosomatic, and social factors, and positive-negative coping strategies, which include cognitive-behavioral factors. Suicidal experiences are categorized as ideation, suicidal attempt, and after-suicide stage. In the latter, participants reported three types of lifestyles by their enthusiasm for 'being alive'. Conclusions: Through the qualitative synthesis of research, we could analyze and categorize major factors and coping strategies of participants who exhibited a depressed mood or suicidal ideation.

        산후우울증과 관련된 질적 연구에 대한 보고 분석

        Objective: The purpose of this paper is to help the development of qualitative research in the field of Korean medicine and policy support for postpartum depression. Methods: Qualitative studies related to postpartum depression retrieved from Pubmed, CENTRAL, EMBASE, NEJM, and CINHAL were evaluated independently by two researchers using the integrated criterion, COREQ. In addition, each subject of study was qualitatively synthesized. Results: A total of 1,374 papers were searched and 13 papers were finally selected. The results of the study using COREQ lacked information about the researchers. The results of the study were integrated into three themes: “Feeling Failure”, “Struggling for self-identity and role of mother” and “Seeing Help and Recovery”. Conclusion: Postpartum depression can be accessed through various methods such as Korean medicine counseling, acupuncture and herbal medicine, and further research and manual preparation is required.

        M&L 심리치료가 불안장애를 가진 취약계층 청소년의 건강한 성장에 미치는 영향 - 단일증례의 양적, 질적 분석

        Objectives: This study investigates the impact of M&L psychotherapy on an adolescent with anxiety disorders transitioning into adulthood, using a retrospective mixed-method approach. Methods: An adolescent with Diagnostic Statistical Manual of Mental Disorders (DSM)-5-diagnosed anxiety disorders underwent combined treatment with M&L psychotherapy and complex Korean medicine therapy over a period of approximately two years. Patient records tracked the diagnosis, treatments, and progress. In-depth interviews were also conducted. The study used a convergent parallel mixed-method approach, integrating quantitative analysis from psychological assessments, including the State-Trait Anxiety Inventory (STAI-X), the Core Seven Emotions Inventory-short form (CSEI-s), and the Minnesota Multiphasic Personality Inventory-2 (MMPI-2), with qualitative analyses of traditional Korean medicine counseling session notes, interviews, and other qualitative psychological assessments. Results: State anxiety decreased from 40 to 36, and trait anxiety decreased from 42 to 34. MMPI-2 L scale scores decreased slightly to 64T, whereas S scale scores increased to 62T, and FRS scores decreased to 70T from an initial score of 78T. Qualitative analysis revealed an increase in the MMPI-2 Es scale, identified as “subjectivity.” Joy (喜) emerged as the highest emotional profile score in the CSEI-S. Scores for anger (怒), thought (思), fear (恐), and fright (驚) decreased, indicating improved negative emotions. Qualitatively, expressions of determination to “move forward” and “strengthen the mind” were noted, suggesting enhanced “lower danjeon (下丹田).” Conclusions: Integrating traditional Korean medicine treatment with M&L psychotherapy for a vulnerable adolescent with anxiety disorders demonstrated efficacy. Further research is warranted to substantiate the effectiveness of M&L psychotherapy and advocate for its wider adoption in mainstream practice.
