파킨슨병 환자의 유형에 따른 양팔협응 운동과제 수행의 차이

본 연구에서는 양팔을 사용한 협응패턴의 수행에 있어서 인체 활동에서의 중추적인 역할을 하는 것으로 여겨지는 대뇌기저핵(basal ganglia)과 관련된 질환인 파킨슨병의 효과를 좀 더 명확하고 구체적으로 규명하기 위해 항파킨슨 약물을 복용하지 않은 초기(de novo) 파킨슨질환 환자군(n=10)과 약물을 5년 이상 복용한 중증 파킨슨질환 환자군(n=10), 그리고 정상 대조군(n=10)을 대상으로 하여 파킨슨병환자의 유형에 따라 협응동작 과제의 수행 정확성과 안정성 등의 상위수준의 매개변인과 동작주기와 거리조절력, 그리고 동작의 연속성 등의 하위수준의 매개변인에 어떠한 차이가 있는지 조사하였다. 연구결과, 약물미복용 파킨슨환자집단은 양팔협응의 정확성, 일관성, 동작의 연속성에서 정상집단과 유사한 수행을 보인 반면, 약물복용 파킨슨환자집단은 나머지 두 집단에 비해 낮은 수행을 나타냈으며 과제의 복잡성이 더욱 증가된 반대위상 협응조건에서 운동장애 증상이 더욱 뚜렷이 나타났다. 이는 양팔협응 동작과 관련된 수행의 상위수준에서의 제어력 감소가 파킨슨병으로 인해 본태적으로 발생한 것이 아니라 병의 진행이나 약물에 의해 유발된 이상운동증(dyskinesia) 등과 같은 운동합병증에 의해 발생된다는 사실을 시사해 준다. 한편, 약물 미복용집단은 하위영역인 동작속도와 거리조절력에서는 정상인 집단보다 저조한 수행력을 보여 주었다. 이러한 결과는 약물미복용 파킨슨질환 집단의 경우 양팔협응 과제와 관련된 각 사지의 움직임에 있어서 정교하고 세부적인 운동계획과 실행에서는 다소 어려움을 나타내는 것으로 파악할 수 있다. It has been suggested that the basal ganglia are involved in the performance of upper-limb bimanual coordination movements. The purpose of this study was to examine the influence of basal ganglia impairments, as manifested in Parkinson`s disease (PD), on the capability of performing bimanual coordination in detail. Ten PD patients in advanced stages and 10 early stages of untreated PD patients, and 10 age-matched normal controls were instructed to perform bimanual coordination movements at 3 different speeds (0.75, 1.25, 1.78Hz). The results demonstrated that only advanced PD patients exhibited a reduced synchronized coordination during the anti-phase mode, particularly at the fastest speed (i.e., 1.75 Hz), compared with other two subject groups. This finding suggests that the observed bimanual coordination dysfunction in PD may be developed over the disease progression or a side effect of dopaminergic medication. The present study also examined the parameterization of individual limb movements such as speed, amplitude, and smoothness of each limb movement, independent of the coordinative requirements of in-phase and anti-phase patterns. The results showed that both PD patient groups showed impairments in scaling movement speed and amplitude of limb, and these deficits were more pronounced as a function of movement frequency. This indicates that difficulties in parameterizing the individual limb movements during bimanual coordination could be an early sign of the pathophysiology of the disease.

본 연구에서는 양팔을 사용한 협응운동의 수행에 있어서 중추적인 역할을 하는 것으로 여겨지는 소뇌에 이상이 있는 환자들(n=10)을 대상으로 협응동작 패턴의 수행 정확성과 안정성 등의 상위수준의 매개변인과 동작주기와 거리조절력, 그리고 동작의 연속성 등의 하위수준의 매개변인을 측정하여 비슷한 연령대의 정상인과 비교하였다. 연구결과, 소뇌질환 환자들은 양손을 움직이는 하위수준에서 거리측정 장애를 보여 정상인집단에 비해 거리조절 능력이 떨어지는 것으로 나타났고, 동작의 끊김이나 주저함이 없이 연속적으로 운동을 수행하는 능력에도 저하가 나타났다. 이러한 결과는 뇌질환으로 인해 운동수행 시 필요로 하는 적절한 양의 근력이나 힘을 생성해내는데 어려움이 발생하여 일어난 결과라고 할 수 있으며, 또한 신체 내외의 감각정보를 활용하여 최적의 동작을 계획하고 발현시키는 제어체계에 이상이 생겨 발생한 것으로 생각된다. 하지만 양손을 움직이는 상위수준인 동일위상과 반대위상 협응패턴의 수행에서는 정상인들과 큰 차이를 보이지 않았다. 이는 단일관절을 이용한 양팔 협응동작의 수행에 있어서 전체적인 협응구조와 그 안정성 등의 상위수준의 제어에는 뇌신경 구조체 중에서 소뇌가 크게 관여하고 있지 않은 것으로 보이며, 오히려 대뇌심부의 기저핵이 관련 동작의 수행에 깊은 연관이 있는 것으로 해석된다. Although much less literature data are available as compared with what is known about the role of the basal ganglia in the bimanual movements, the cerebellum has also been known as a critical neural structure involved in coordination of limb movements. The purpose of this study was to examine the influence of cerebellar disease on the capability of performing bimanual coordination in a more specific manner. Ten cerebellar patients and 10 age-matched normal contro inwere instructed to perform one-limb and bimanual coordination movements (in-phase and anti-phase) at 3 different speeds (0.7mal1.2mal1.78Hz). The results demonstrated that 1) at the limb-specific level, cerebellar patients made larger movement amplitudes and moved more slowly than normal contro inin both coordination modes. In addition, they showed lower harmonicity values at slower movement frequencnnerthan normal contro i, suggneting artendency to hneitate or ancerr their movements on the target when making movement reversals to the next. 2) At the global level, the coordinative characteristics of in-phase and anti-phase modes were intact regardless of speed demands in cerebellar patients. This result suggests that the cerebellum may not be critical for bimanual coordination, providing evidence against a general role of cerebellum for coordination tasks.
