두경부 조직검사 시행 시 불안감 완화 방안으로 음악요법 적용 후 고객 만족도 평가

목적 : 두경부 조직검사를 받게 되는 환자들은 의식이 명료한 상태에서 진행되어 검사 중 불안이나 통증에 노출되어 있다. 또한 조직검사 시 환자의 움직임은 환자 안전에 영향을 미칠 수 있기 때문에 환자는 부동 자세를 취해야 하므로 공포감 또는 불안감이 증가된다. 이에 검사 중 불안감 완화 방안으로 음악요법을 적용하여 그 유용성을 평가하고자 하였다. 대상 및 방법 : 연구대상은 2021년 4월5일부터 4월30일 까지 본원에서 두경부 조직검사를 시행한 환자 28명으로, 대상자 선정기준은 1) 설문지를 이해할 능력이 있으며 연구 참여에 수락한 자, 2) 15세 이상 65세 미만의 환자, 3) 청각이나 언어장애가 없으며 의사소통이 가능한자, 4) 음악 청취가 가능한 자로 하였다. 검사 시생 전 The Hospital Anxiety-Depression Scale(Zigmond and Snaith method)을 이용하였으며, 본 도구는 자기기입형 설문 총 14문항으로 홀수 번째 문항은 불안 정도, 짝수 번째 문항은 우울 정도를 확인하는 질문으로 구성되어 있다. 문항마다 0점에서 3점으로 채점되며, 총점의 범위는 0점에서 21점까지로 8점이상이면 불안군 혹은 우울군으로 판정한다. 이중 불안 문항인 홀수 번째 문항을 이용하여 불안 정도를 평가하였으며, 음악요법 적용 후 불안감 완화에 대한 만족도를 3점척도로 평가하였다. 설문에 대한 신뢰도는 크론바흐(Cronbach’s) 알파계수를 이용하였다. 결과 : 28명 중 조직검사 시행 전 The Hospital Anxiety-Depression Scale 홀수문항의 합이 8점 이상인 불안군은 10명으로 전체의 35%이었으며, 평균은 6.48점이었다. 음악요법 시행 후 만족도는 25명(89%), 보통은 3명(11%), 불만족은 0명(0%)이었다. 결론 : 환자의 심리적 상태를 안정화 시켜 불안감 완화를 위한 음악요법의 결과, 대상 환자의 90%에서 만족하는 결과를 얻어, 음악요법이 조직검사 시행 시 불안감 완화에 효과가 있는 것으로 생각된다. 향후 더 많은 데이터와 상황별, 음악장르별 특성을 고려한 연구들이 필요하리라 사료된다. Pirpose : The purpose of this research is to focus on the patients who take the biopsy test for the head and neck. While the group of patients are taking this test, , they are exposed to the status of nerve and pain due to the patients have any movement while they had a biopsy test will affect to the safety of patients. So patient stand immovable posture however, this posture will accelerate the panic and nervous. To avoid these issues, Therefore, the main purpose of this study is to evaluate the usefulness of anxiety during the test by applying music therapy usefully to avoid thus problem. Materials and Methods : The subjects of this study were 28 patients who underwent a head and neck biopsy at our hospital from April 5 to 30, 2021, and the selection criteria are as follows. 1) Those who have the ability to understand the questionnaire and have accepted the study, 2) 15 patients over 65 years of age, 3) those who have no hearing or speech impairment and can communicate, 4) patients who can listen to music. The pre-test hospital anxiety-depression scale (Zigmond and Snaith method) was used, the toll consisted of a total of 14 self-written questionnaires, and the odd numbered questionnaire consisted of questions to determine the degree of anxiety and even numbers. There are numbered question levels. Each item is 0 to 3 points, and the total score is 0 to 21, and if it is 8 or more, it is judged as an anxiety group or a depressed group. Among them, the degree of anxiety was evaluated using odd-numbered questions and anxiety questions, and the satisfaction with the relief of anxiety after the application of music therapy was evaluated on a 3-point scale. Cronbach’s alpha coefficient was used for the reliability of the questionnaire. Results : Among 28 patients, the anxiety group scored 8 points or more on the hospital anxiety-depression scale before biopsy was 10, 35% of the total, and the average was 6.48. Satisfaction after music therapy was 25 (89%) satisfied, 3 (11%) average, and 0(0%) dissatisfied. Conclusion : As a result of music therapy, which stabilizes the patient’s psychological state and relieves anxiety, 90% of the target patients obtained satisfactory results, and music therapy is considered effective in relieving anxiety during biopsy. In the future, more data and research will be needed taking into account the characteristics of each situation and music genre.
