목적: 완관절 척측 통증의 원인 중 하나인 척측 수근 신건 탈구는 유병률이 낮고 그 수술적 치료 결과가 보고된 바가 많지 않다. 본 연구에서는 11예의 척측 수근 신건 탈구 증례를 대상으로 시행한 수술적 치료 방법 및 치료 결과를 보고하고자 한다. 대상 및 방법: 2005년 3월부터 2015년 2월까지 강남세브란스병원에서 수술적 치료를 진행한 11예의 환자를 대상으로 후향적 연구를 시행하였으며, 자기공명영상 검사 및 동적 초음파를 사용하여 진단하였다. 손상의 분류는 Inoue의 분류 방식을 사용하였으며, type A 2예, type B 1예, type C 5예가 있었다. oInue의 분류에 해당되지 않는 3예가 있었으며, 원위 요척 관절로 탈구 2예, 척측 잠 김탈구로 1예가 있었다. 수술적 치료로 type A와 B는 재배치 후 신전 지지대를 봉합한 후에 골섬유막을 고정하였고, type C는 재배치 후 골섬유막을 고정하였다. 분류되지 않는 3예는 모두 척측 수근 신건 재배치 후 신전 지지대를 이용하여 재건술을 시행하였으며, 모든 예에서 anchor suture를 사용하여 골섬유막을 고정하였다. 수술 전후 임상적 결과는 관절 운동 범위와 악력을 비교하였고, visual analogue scale (VAS) score, quick disabilities of the arm, shoulder and hand (Q-DASH) score, Mayo wrist score를 통하여 환자의 기능적 회복을 비교하였다. 결과: 수술 받을 당시 나이의 중앙값은 32세였으며 수술 후 평균 11.2개월간 추시 관찰하였다. 동반손상으로는 삼각섬유인대 복합체 손상 5예, 원위부 요척 관절 불안정성 2예가 있었으며, 탈구된 신건 중 3예에서 종 파열 소견을 보였다. 운동 범위는 건측과 비교하였을 때 평균 84.7%에서 최종 추시에서 92.4%로 호전되었고 VAS score, Q-DASH score, Mayo wrist score 모두 호전을 보였으며(평균 6.0점에서 1.1점, 평균 40.9점에서 12.4점, 평균 75.9점에서 86.4점) 모두 통계적으로 유의한 차이를 보였다. 결론: 척측 수근 신건 탈구는 드문 질환이나 완관절의 척측 통증 및 운동 장애를 유발하는 원인이 될 수 있다. 이학적 검사와 자기공명영상 검사 및 동적 초음파 등을 이용하여 정확한 진단을 내리고 진단 후 수술적 치료를 통해 각 분류에 따른 적절한 방법으로 수술 시 임상적으로 좋은 결과를 얻을 수 있을 것으로 생각된다. Purpose: Extensor carpi ulnaris (ECU) subluxation has a low incidence rate, to date, there has only been a few studies evaluating the operative treatment for type of injury. The purpose of this study was to retrospectively analyze 11 patients with ECU subluxation who underwent operative treatment. Materials and Methods: Between March 2005 and February 2015, 11 patients received operative treatment. Magnetic resonance imaging and dynamic ultrasound were used to make the diagnosis. ECU subluxation patterns were classified by the Inoue’s classification system. There were two cases of type A, one case of type B, five cases of type C, and three cases unfit for Inoue’s classification. We also found two cases of radial subluxation and one case of ulnar locked subluxation. In type A and B cases, ECU tendons were relocated then sheaths repair was performed, and the extensor retinaculum reconstruction was performed. In type C cases, the fibro-osseous sheaths were fixed. In the three unclassified cases, extensor retinaculum reconstruction was performed. In all cases, fibro-osseous sheaths were fixed using the anchor suture technique. We compared the clinical results based on the following: motion range of the wrist joint; grip strength; visual analogue scale (VAS) score; quick disabilities of the arm, shoulder and hand (Q-DASH) score; and Mayo wrist score. Results: The median age of patients at the time of the operation was 32 years, and the average follow-up period was 11.2 months. There were five cases of triangular fibrocartilage complex tear, two cases of distal radioulnar joint instability, three cases of ECU split tear as accompanying injury. There were significant differences in the clinical results. The average motion range of the wrist increased compared with the preoperative value (84.7% to 92.4% compared to contralateral normal side). The postoperative VAS score, Q-DASH score and Mayo wrist score showed better results compared with the respective preoperative values (6.0 to 1.1, 40.9 to 12.4, 75.9 to 86.4). Conclusion: ECU subluxation is a rare occurrence. Dynamic ultrasound is useful in diagnosing ECU tendon subluxation. Satisfactory results can be obtained with the use of a proper technique, which depends on the type of subluxation.
