      • F-128 Prevalence and acute exacerbation rate of Asthma-COPD overlap syndrome in Korean COPD cohort

        Purpose: We evaluated the proportion of asthma-chronic obstructive pulmonary disease (COPD) overlap syndrome (ACOS) among Korean COPD patients defined by recently suggested Spanish and modified Spanish (or ERJ) criteria, and investigated the impact of ACOS on acute exacerbation. Method: Among COPD patients enrolled in Korean COPD Subtype Study (KOCOSS), ACOS was defined as follows: (1)Spanish criteria - at least 1 major criterion (bronchodilator test (BDR)>400mL and 15% or past history of asthma) or at least 2 minor criteria(blood eosinophil>5%, IgE>100IU/mL, or BDR>200mL and 12%) (2)ERJ criteria - all 3 major criteria(persistent airflow limitation with 40 years of ages or older, smoking more than 10 PY or equivalent air pollution exposure, and BDR>400mL) and at least 1 minor criterion (history of atopy or allergic rhinitis, BDR>200mL, blood eosinophil ≥ 300cell/μL). During follow-up, moderate-to- severe exacerbations were evaluated. Result: Prevalences of ACOS according to the Spanish and ERJ criteria were 50.2%(694/1383) and 3.2%(44/1383), respectively. ACOS according to both criteria had no statistically significant impact on time to 1st exacerbation (hazard ratio(HR,1.42; 95%CI, 0.79-2.55 and HR, 0.27; 95%CI, 0.04-1.96; respectively) and the rates of moderate-to-severe exacerbation (incidence rate ratio (IRR),1.26; 95% CI, 0.82-1.92 and IRR 0.50; 95% CI, 0.19-1.29). Conclusion: There were substantial discrepancy in the proportion of ACOS among KOCOSS COPD patients between Spanish criteria and modified Spanish criteria. ACOS did not affect the risk of exacerbation.

        Background: Blood biomarkers including eosinophil count, neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and eosinophil-basophil ratio (EBR) have been suggested to be prognostic factors of chronic obstructive pulmonary disease (COPD). Additionally, the roles of eosinophil-platelet interactions were also suggested in vitro or animal models. We extensively evaluated the impacts of complete blood count (CBC) profiles including eosinophil-platelet interaction on clinical outcomes of stable COPD in Korean cohorts. Methods: The COPD patients enrolled in Korean COPD Subtype Study (KOCOSS) or Seoul National University Airway Registry between 2011 and 2018 were analyzed. Based on the CBC profiles at the time of enrollment, their association with the rate of acute exacerbation (AE) within one year and the decline rate of forced expiratory volume in 1 second (FEV1) were tested. Results: Among 2,221 COPD patients, 1,202 patients were analyzed. Multivariant analysis showed that baseline FEV1 is the most significant factor for both AE rate and FEV1 decline rate. High EBR is associated with high moderate-to-severe AE rates, and both EBR and NLR were related to severe AE rates in addition to FEV1. While eosinophil or eosinophil-platelet interaction terms were not significant factors for outcomes, the patients with low eosinophil count and EBR were a tendency of faster FEV1 decline. Conclusion: EBR and NLR are independent prognostic markers for the AE and FEV1 decline rate of COPD, while eosinophil- platelet interaction had a limited role in predicting clinical outcomes.
