Background: Changwon, the hub city of Southeastern Maritime Industrial Region, 72,811 employees at 941 companies woking, is in a high risk of contact dermatitis. However, there have been no reports available on the TRUE test in Changwon. Objectives: The purpose of this study was to analyze the type, frequency and evaluated the specific relationship with each allergen in Changwon. Methods: During a 5-year period, 197 patients were examined with TRUE test. A univariate analysis of multiple variables including age, gender, occupation was conducted and a comparison with a previous multicenter study was also analyzed. Results: A total of 197 TRUE tested cases (male 79, female 118) were compiled and analyzed. The highest age distribution was 3rd decade in males and 4th decade in females. Patients`` employment background showed that housewives and industrial -workers were the most frequently expected jobs consisting 41.6% and 26.9% respectively. Overall, 71.5% of patients had at least one positive reaction, and the most common allergens were nickel 58(41.1%), cobalt 24(17%), thiomersal 20 (14.1%), and P-Phenylenediamine 19(13.5%), in order of frequency. While nickel and cobalt showed increased reacitivities, formaldehyde, fragrance mix and colophony showed a decreased tendency when compared with those of other previous studies. Conclusion: These results provide the basis for the presumption of the route of allergen exposure and prevention of contact dermatitis in Changwon.
Treatment of infantile hemangiomas with topical timolol agent - single center experience

Background: Systemic propranolol is treatment of choice of large and complicated infantile hemangiomas (IHs), but there are safety concerns. Topical uses of the nonselective beta-blocker timolol can also effective treatment of IHs by inhibit the growth and promote regression. Objectives: The purpose of this study was to evaluate efficacy and safety of topical timolol for managing IHs through a Korean single center experience. Methods: A total of 25 patients with IHs were applied with timolol 0.5% ophthalmic solution twice a day without any other therapeutic modalities. Outcomes were defined in terms of decrease of thickness and reduction of red color in a semiquantitative manner: almost complete reduction (>80%), paler and thinner (50-80%), slightly paler and thinner (<50%), no change, and further growth. Results: The duration of timolol therapy ranged from 8 to 60 weeks (mean: 22.7 weeks). Among 25 patients, 4 patients were shown almost complete reduction, 14 patients were shown paler and thinner, 3 patients were shown slightly paler and thinner, 2 patients were shown no change, and 2 patients were shown further growth. Twenty-one (84%) of the 25 patients reported symptomatic improvement associated with growth arrest of IH and a reduction in thickness and redness. During topical timolol treatment, any systemic absorption adverse events were not observed. Conclusion: In this study, topical timolol is safe and efficacious topical therapy for IHs.
A case of blastic plasmacytoid dendritic cell neoplasm presenting as leukemia

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare hematologic malignancy that is derived from plasmacytoid dendritic cells. This disease typically presents with cutaneous manifestations and have an aggressive clinical course involving lymph nodes, peripheral blood and bone marrow leading to leukaemic transformation. In our case, the patient is a 56 year-old man presenting with multiple erythematous to violaceous plaques, nodules and mass suspicious of leukemia cutis predominantly distributed on the trunk and upper extremity. At the onset of the disease, the patient only had one erythematous nodule on the left forearm which are refractory to antibiotics medication. Histopathological examination of the left upper arm lesion demonstrated nodular dense neoplastic infiltration in the dermis sparing the epidermis with a Grenz zone. On immunohistochemistry, the specimen were positive for CD4, CD56, and CD123. Thus, the patient was consecutively diagnosed with BPDCN. 1 week after biopsy done, the patient complain of gingival hypertrophy and more widespread skin lesions. Through immediate staging investigations, the patient was revealed to have blood and BM involvement. He has been treated with multi-agent chemotherapy. We report a case of blastic plasmacytoid dendritic cell neoplasm with leukaemic dissemination at presentation.
