Hidrocanthoma simplex(HAS) is a rare benign intraepidermal neoplasm arising from eccrine duct. Clinically, it appears as brownish or verrucous plaque on the lower extremities. Histologically, HAS can exhibit intraepidermal cell nests which are called “Borst-Jadassohn phenomenon”. It could be observed in the HAS, clonal seborrheic keratosis (SK), Bowen’s disease and actinic keratosis. An 86-year-old female presented with several decades history of huge brown, verrucous plaque on the right buttock. At initial biopsy, intraepidermal clonal expansion of polygonal basaloid cells were observed within acanthotic epidermis. She was diagnosed as clonal SK, and was removed by CO2 laser ablation. After 3 months, she returned with polypoid grouped papules on the previously treated area. Histologic examination showed intraepidermal proliferation of small to medium-sized polygonal basaloid cells as well as large cells with abundant clear cytoplasm. Atypical cells with large, hyperchromatic nuclei were observed. There was a component of eccrine syringofibroadenoma which is characterized by thin, anastomosing cords of epithelial networks with duct formation extending from the base of the tumor. Tumor cells were stained with EMA and PAS. Final diagnosis was HAS combined with eccrine poroma, eccrine syringofibroadenoma and focal malignant changes. Herein we report a case showing natural history of eccrine tumor from HAS to malignant changes over time.
Evaluation of YouTube videos about isotretinoin as treatment of acne vulgaris

Background: YouTube is the popular video-sharing websites in Korea. But, there is no proper regulation on incorrect or biased information. Acne is common in adolescent familiar with YouTube. Thus, misleading information about isotretinoin in YouTube videos could have a bad impact on clinical setting. Objectives: We evaluate the contents quality of information about isotretinoin in YouTube videos. Methods: The following keywords were searched on YouTube Korea on July 1st, 2020: ‘isotretinoin’, ‘roaccutane,’ and ‘isotinone’. DISCERN tool was used to evaluate the reliability and quality of information and 8 items were used to evaluate scientific accuracy and comprehensiveness: mechanism, indications, dose regimen, contraindications, side effects, blood tests, drug interactions, and pregnancy issue. Results: Excluding duplicate or inappropriate videos, total 164 videos were analyzed. Mean overall quality score was 2.24 in DISCERN tool (5-point scale from 1 to 5). Mean score in 8-issue-criteria (3-point scale from 0 to 2) was 0.61. Especially, indications, blood tests, and drug interactions were poorly addressed in videos. Health care providers offer better quality of video than non-health care providers. The quality score was not correlated with the popularity of video. Conclusion: YouTube videos covering isotretinoin show overall poor quality. Incorrect information could be propagated in YouTube. So, dermatologists should try to provide more accurate information about isotretinoin.
A case of myiasis in a patient with chronic lymphedema of lower limbs

Myiasis is the infestation of dipterous larvae that feed on vital tissue of human. It is common in endemic areas but is rare in Korea. Myiasis is classified as site involved or clinical features. Wound myiasis is a kind of cutaneous myiasis and generally associated with poor hygiene, and low socio-economic status. A 66-year-old man presented with erythematous patches with severe swelling and cobble-stone like skin surface changes on the right leg. He had a cerebral palsy and he usually sleeps in sitting position because it is hard to lie supine due to his physical disability. He frequently hit his legs to corner of sidewalk. He was diagnosed as cellulitis with chronic lymphedema. He was treated with antibiotics and dressing was done with compressive band, but he was lost to follow-up. When he re-visited our clinic after a month, cellulitis was aggravated and lots of larvae were observed on his right leg. Larvae were eliminated manually. The patient was treated with antibiotics, vitis vinifera extract (Entelon®) and compression dressing. Also, we encouraged him to sleep in supine position. The cellulitis and lymphedema were improved slowly. Skin infection is more common in a patient with chronic lymphedema. He had severe lymphedema which shows features of cobble-stone like nodules known as elephantiasis nostra verrucosa and his hygiene was poor. Herein we report a case of wound myiasis in patient with chronic lymphedema and cellulitis.
실제 임상에서 중등증 및 중증의 한국인 건선 환자를 대상으로 한 Risankizumab의 치료 효과 및 안정성에 대한 연구

Background: Risankizumab is a humanized immunoglobulin G1 monoclonal antibody that selectively binds to the p19 subunit of interleukin-23. Risankizumab has demonstrated rapid and excellent therapeutic effects in several clinical trials. Although a growing number of studies have reported data on the real-world efficacy and safety of risankizumab for the treatment of psoriasis, no such study has been conducted in Korea. Objective: We evaluated the real-world efficacy and safety of risankizumab for the treatment of moderate-to-severe plaque psoriasis in Korean patients. Methods: This was a retrospective single-center study. A total of 33 patients treated with risankizumab, for at least 16 weeks, were enrolled. Based on electronic medical records, the clinical characteristics, psoriasis area and severity index (PASI) score, body surface area, and adverse events were investigated. Results: The mean PASI score was significantly reduced at 4 weeks of risankizumab treatment (3.27±2.15) and gradually reduced at week 16 (1.06±0.97) and week 52 (0.24±0.63) (p<0.05). At week 16, all patients achieved a PASI 75 response, and 66.7% and 27.3% of patients achieved PASI 90 and PASI 100 responses, respectively. Obese patients (body mass index, BMI≥25 kg/m<sup>2</sup>) showed a lower PASI 90 response than non-obese patients (BMI<25 kg/m<sup>2</sup>) at week 16. Older patients (age≥65 years) showed significantly higher PASI scores than younger patients (age<65 years) at week 16. Mild to moderate adverse events were reported in four patients; however, no patient discontinued treatment. Conclusion: Risankizumab was very effective in a real-world clinical practice with a favorable safety profile in Korean patients with moderate-to-severe psoriasis.
