Background: Neutrophil extracellular traps (NETs) are structures composed of DNA and granular proteins, which rapidly trap and kill pathogens. The release of NETs has been identifi ed as a novel immune response in innate immunity. But their role in pneumonia sepsis is unclear. We studied the clinical value of NETs of plasma for the development of prognosis for adult patients admitted to ICU for pneumonia Methods: We investigated the presence of NETs in the blood from15 patients with community- acquired pneumonia who have been admitted to the intensive care. We isolated neutrophils from community-acquired pneumonia patients on day 1 (PD1) and day 3 (PD3). Then we determined the ROS generation, NETs formation, surface expression of granule markers (CD63, CD66b, CD35; which are specifi c markers for azurophil granule, specifi c granule and secretory vesicle, respectively) and examined the morphology of neutrophils Results: Out of 15 patients with pneumonia, 7 (33.3%) were male; whose median age was 70.9 years median acute physiology and chronic health evaluation (APACHE) II was 26.5 and median SOFA 11 at admission. In the follow-up, 11(73.3%) patient survived at 28 days NETs were identifi ed in from all patients. When neutrophils were stimulated with PMA, PD1 neutrophils showed increased NETs formation, higher surface expression of CD63/CD66b whereas reactive Oxygen Species (ROS) generation was rather attenuated. Survival group showed lower ROS generation and higher NETs formation thannonsurvival group Conclusions: NETs in serum may re ect prognosis of pneumonia sepsis. Degree of ROS generation and NET formation could be used predictor of prognosis of pneumonia sepsis
