골다공증이 있는 상완골 골두의 파일럿 홀 (Pilot Hole)과 흡수성 나사못의 크기에 따른 토크 (Torque)와뽑힘 강도 (Pullout Strength) 간의 관계

목적: 본 연구의 목적은 골다공증이 있는 회전근개 파열 환자에서 상완골 골두에 표준 크기 (5.0 mm) 보다 작은 송곳 (awl) (3.7 mm 나사못용)을 사용해서 5.0 mm 나사못을 삽입하는 것이 표준 크기의 송곳과 6.5 mm 나사못을 삽입하는 것에 비해 토크 (torque)나 뽑힘 강도(pullout strength)에서 차이가 있는지를 알아보고자 하였다. 대상 및 방법: 방부 처리된 12명, 24개의 짝지은 사체 견관절을 A군와 B군, 두 군으로 나누어 관심 부위의 골밀도를 측정한 후에 A군에서는 5.0 mm 나사못 삽입을 위해 3.7 mm 송곳 (awl)을 이용하는 경우를 A1군, 5.0 mm 나사못 삽입을 위해 5.0 mm 송곳을 이용하는 경우를 A2군으로 하였다. B군에서는 5.0 mm 송곳을 이용하여 5.0 mm 나사못을 삽입하는 경우를 B1군, 6.5 mm 나사못을 삽입하는 경우를 B2군으로 하여 나사못 삽입 시의 토크와 뽑힘 강도를 측정하였다. 결과: 골밀도는 A군과 B군 간이나 군내에 차이가 없었다. 토크는 A1군이 A2군보다 각각 20.6 cN·m와 13.2 cN·m로 유의한 차이를 보였고 (p<0.001), B1군과 B2군에서도 각각 12.1 cN·m와 20.8 cN·m로 유의한 차이를 보였다 (p<0.001). 그러나 증가된 정도의 차이는 유의하지 않았다. A1군과 A2군의 뽑힘 강도는 각각 204.2 N과 152.9 N으로 유의한 차이를 보였고 (p<0.001), B1군과 B2군에서도 149.5 N과 210.9 N으로 유의한 차이를 보였다 (p<0.001). 그러나 증가된 정도의 차이는 유의하지 않았다. 결론: 송곳 크기보다 큰 치수의 나사못을 사용하면, 같은 크기의 송곳과 나사못을 사용하는 경우 토크나 뽑힘 강도가 유의하게 증가하였다. 그러나 증가된 정도는 큰 송곳과 작은 송곳 간에 차이가 유의하지 않았다. 따라서 대결절의 footprint 내에 나사못과 나사못 사이의 거리가 충분하지 않은 경우에 5.0 mm 송곳을 이용하여 6.5 mm 나사못을 삽입하는 대신 3.7 mm 송곳을 이용하여 5.0 mm 나사못을 삽입해도 증가되는 뽑힘 강도는 차이가 없을 것으로 생각한다. Purpose: The object of this study was to investigate the difference in torque and pullout strength between the standard anchor insertion (5.0 mm) with a small awl (3.7 mm) and larger anchor insertion (6.5 mm), with a standard awl (5.0 mm) in osteoporotic humeral head. Materials and Methods: The embalmed 24 paired cadaveric shoulders were assigned to either Group A or B. After measuring the bone mineral density (BMD) of the ROI (region of interest) in the humeral head, 5.0 mm suture anchors were inserted using a 3.7 mm awl in Group A1, and the same 5.0 mm anchors were inserted using a 5.0 mm awl in Group A2. The 5.0 mm anchors were inserted using a 5.0 mm awl in Group B1, and 6.5 mm anchors were inserted using a 5.0 mm awl in Group B2. We measured the torques at the time of the anchor insertion and pullout strengths. Results: There was no significant difference in the BMD between the groups. The torque of A1 (20.6 cN·m) was significantly higher than that of A2 (13.2 cN·m), and the torque of B2 (20.8 cN·m) was significantly higher than that of B1(12.1 cN·m). However, the difference in the increased torque between group A and B was not significant. The pullout strength of A1 (204.2 N) was significantly higher than that of A2 (152.9 N), and the pullout strength of B2 (210.9 N) was significantly higher than that of B1 (149.5 N). However, the difference in the increased pullout strength between Group A and B was not significant. Conclusion: In severe osteoporosis, the use of a larger suture anchor with a standard awl increased the torque and pullout strength significantly, in comparison to the use of the same sized suture anchor and awl. If there is an inadequate interval between the anchors on the greater tuberosity, the use of a 3.7 mm awl and 5.0 mm anchor will be beneficial compared to that of a 5.0 mm awl and 6.5 mm anchor, considering that an increase in the pullout strength does not depend on the awl size.
관절경하 AO 무두 압박 나사를 이용한 견갑골 전방 관절와 분쇄 골절의 치료

목적: 견갑골 전방 관절와 분쇄 골절에 대해 관절경하 정복 및 AO 무두 압박 나사를 이용하여 골편을 고정한 증례를 보고하고자 한다. 대상 및 방법: 31세 남자로 내원 2주 전 추락 후 발생한 좌측 견관절의 전방 관절와 분쇄 골절에 대하여 관절경하에서 골절을 정복한 후, 표준 전상방 삽입구를 통해 1.1 mm K 강선을 가이드핀으로 삽입하고 AO 무두 압박 나사로 골편을 고정하였다. 결과: 수술 후 12개월째 견관절의 운동 범위는 정상 범위로 회복 되었으며, 골절은 유합을 얻었고 일상 생활에 불편감은 없었다. 결론: AO 무두 압박 나사는 나사의 길이가 다양하여 작은 관절와 골편에 사용하기 적합하고, 가이드 핀의 길이가 상대적으로 짧으나 1.1 mm K 강선으로 대체가 가능하며, 나사 돌리개 (screw driver)나 천공기 (drill bit) 등의 동반 기구의 길이가 길어 비교적 깊은 위치의 견갑과 골절의 고정에 사용하기 편리하여 관절경적 고정술 시 유용하게 이용할 수 있을 것으로 생각된다. Purpose: We present a case of anterior glenoid rim comminuted fracture that was treated with arthroscopic reduction and an AO headless compression screw (HCS) fixation. Materials and Methods: A 31-year old man complained of left shoulder pain after falling down on stairs. The anterior glenoid comminuted fragments were arthroscopically reduced. Fixation with an AO HCS was done after placement of 1.1 mm Kirschner wire as a guide pin through a standard cannulated anterosuperior portal. Results: Twelve months after the operation, union of the fracture was achieved and the range of motion was fully recovered. He did not complain of any discomfort during his activities of daily living. Conclusion: An AO HCS had various screw sizes and this was good for fixation of a small glenoid fracture and a long drill bit and screw driver were useful for fixation of deep seated glenoid fracture. A short guide wire could be replaced by a 1.1 mm K-wire. An AO HCS was useful for fixation of an anterior glenoid rim comminuted fracture.
