Comparison of Image Quality using Various Quantitative Evaluation Methods between Sequence and Spiral scan during Brain CT

두부 CT검사에는 고식적 방식과 나선형 방식이 있다.두부 CT검사 시 고식적 방식이 나선형 방식에 비해 화질이 더 우수하여 더 많이 사용되어왔다. 그러나 이러한 우수한 화질이라는 평가는 주로 정성적으로 평가한 것이 대부분이었으며 이는 객관성이 떨어진다는 제한점이 있다. 이에 본 연구는 다량한 정량적 평가를 하기 위하여 고식적 방식과 나선형 방식, 즉 2가지 검사방법을 동시에 시행한 적이 있는 환자 20명을 대상으로 CT Number, 영상 잡음, 대조도 대 잡음비 등 다양한 정량적 평가를 하였다. 또한 저대조도, 영역백질, 회백(질)의 구분을 위하여 ACR 팬텀의 모듈 2를 관전류초를 변화시키며 히스토그램 및 그래프를 이용해 두부 CT 영상 화질을 비교하였다. 후향적 환자 연구 시 CT Number, 영상 잡음, 대조도 대 잡음비 각각의 결과는 고식적 방식과 나선형 방식 순서로 31.73 ± 3.96 vs 31.92 ± 3.25, 3.53 ± 0.63 vs 4.1 ± 0.54, 275.82 ± 43.49 vs 239.76 ± 31.88이었다. 팬텀 연구의 경우 고식적 스캔 방식이 나선형 스캔 방식에 비해 보고자 하는 CT Number를 잘 표현했고 그래프화하였을 때 높은 첨도를 가지고 있었다. 결과적으로 고식적 방식이 나선형 방식에 비하여 우수한 화질을 가졌고, 히스토그램 기반으로 분석한 수치와 그래프를 이용한 방법은 영상 평가의 도구로 사용 시 유용할 수 있을 것이라 판단된다. There are two scan methods such as sequence and spiral scan in CT examination.I n Brain CT, the sequence scan mode has been utilized more than the spiral mode due to having better image quality.H owever, this evaluation of image quality was mainly subjective assessment and had some limitation.S o, we tried to analyze the image quality of brain CT between the sequence and spiral mode with 20 patients data and an ACR phantom by using various quantitative evaluation such as CT number, image noise, contrast to noise ratio (CNR), and histogram-based analysis.E ach result of CT number, image noise, CNR between two modes (Sequence vs Spiral) were as follows: 31.73 ± 3.96 vs 31.92 ± 3.25, 3.53 ± 0.63 vs 4.1 ± 0.54, 275.82 ± 43.49 vs 239.76 ± 31.88. Overall quantitative image quality of the sequence mode were better than the spiral mode in the study of patient images.A lso, histogram analysis by using the ACR phantom represented higher figures of targeted CT number in the sequence mode than the spiral mode. Additionally, all graphs of phantom images with the sequence scan had high profiles.A s a results, the sequence scan mode had better quantitative image quality and histogram based evaluation was proven by very useful method to distinguish the low contrast resolution between different two vague portions.

육미지황탕(六味地黃湯)및 팔미지황탕(八味地黃湯)의 약침(藥鍼)이 신장기능(腎臟機能)에 미치는 영향(影響)

The aim of this experiment was to investigate the effects of Aqua-Acupuncture of Yukmijiwangtang(六味地黃湯) and Palmijihwang-tang(八味地黃湯) water extracts applied at the meridian points BL 23(賢兪) and GV 4(命門) to test the renal function in normal rats. The results obtained were as follows; 1. Among the effects of Aqua-Acupuncture of Yukmijihwangtang water extract at the merdian point BL 23 group, there were significant changes in water balance and urine volume over a 1 week period; Both decreasing and decreasing trends were exihibited. Urinary excretion of sodium and free water clearance changed significantly over a 2 week period; Both decreasing and increasing trends were exihibited, Urinary excretion of potassium, chloride and creatinine, plasma renin activity, plasma levels of aldosterone and atrial natriuretic peptide showed no significant differences compared to the contral group. 2. Among the effects of Aqua-Acupuncture of Palmljihwangtang water extract at the merdian point BL 23 group, there were significant changes in water balance and free water clearance over a 2 week period; a decreasing trend. Urinary excretion of creatinine changed significantly over a 2 week period; an increasing trend. Urinary excretion of chloride changed significantly over 1 week period; an increasing trend. The plasma levels of atrial natriuretic peptide changed significantly over 1 and 2 week period; an increasing trend. Urine volume, urinary excretion of sodium and pottasium, plasma renin activity and plasma level of aldosterone showed no significant differences compared to the control group. 3. Among the effects of Aqua-Acupuncture of Yukmijihwangtang water extract at the meridian point GV 4 group, there was significant decrease in water balance over a 2 week period; there was significant decrease in urine volwne and urinary excretion of sodium and creatinine over a 1 week period, followed by an increasing trend after 2 weeks. Urinary excretion of free water clearance demonstrated significant changes over both 1 and 2 week period; both increasing and decreasing trends were exihibited. Urinary excretion of chloride and plasma levels of aldosterone increased significantly over 1 and 2 week period. Plasma levels of atrial natriuretic peptide also decreased significantly. Plasma renin activity showed no significant differences compared to the control group. 4. Among the effects of Aqua-Acupuncture of Palmijihwangtang water extract at the meridian point GV 4 group, water balance and urinary excretion of chloride, plasma levels of aldosterone decreased significantly over both 1 and 2 week period. Urine volume and urinary excretion of pottasium decreased significantly. Urinary excretion of creatinine and urinary excretion of sodium changed significantly over both 1 and 2 week period. Urinary excretion of free water clearance, plasma renin activity and plasma level of atrial natriuretic peptide showed no significant differences compared to the control group. Seeing these results, I come to know that the effects Aqua-Acupuncture of Yukmijihwangtang and Palmijihwangtang water extracts at the meridian point BL 23 and GV 4 have affected the renal function differently. Seeing the results that BL 23 is a meridian point for Aqua-Acupuncture directly related to the kidney, I think, we can use Aqua-Acupuncture of Yuk-mijihwangtang and Palmijihwangtang water extracts to prevent and to treat the diseases related to kidney.

약침요법(藥鍼療法)의 소개(紹介) 및 문제점(問題點) 고찰(考察)

Aqua-acupuncture therapy is a new acupuncture-therapy that cures diseases by needling with direct medication of oriental or western medicine at acupuncture points which are related to a disease, pressure pain points or positive reaction points according to meridian theory, followed by controlling one's body function and relieving the pathogenic state. Recently, the method is widely applied clinically owing to its handiness and usefulness, but it is pointed that it has some problems. Therefore, the present studies have been carried out to introduce history of the method, materia-medica which is ordinarily used, acupuncture points to be selected, handling method, mechanism of action, range of application and some points to be careful. Moreover, through the experiences that we have made by applying the method clinically, we suggest that the following study should be carried out : First, clinical application of needling with direct medication should be made by using materia-medica which is already proved safe. Second, it is essential to study the side-effects accompanying fever, pains, mass and hemolysis after the method is applied. Third, the study on the mechanism of action of the method should be accomplished continuously.

Objectives : The purpose of this study was to investigate the location of LI20 and ST3 by facial measurement according to Sasang Costitution, Sex and Age. Methods : To collect the photograph of face, we used the face measurement apparatus, and then analysed whether ST3 was located out of the nasolabial sulcus or not. Results : ST3 was located at the lateral to the nasolabial sulcus in 94.9%; 92.1% of Soyangin, 97.7% of Taeumin, 94.4% of Soeumin 92.3% of male, and 92.3% of female according to Sasang Constitution and sex. According to age distribution, the highest percentage(100%) was appeared at $16{\sim}20$ age, and the lowest(91.8%) was appeared at $70{\sim}74$ age. Conclusion : This study suggests that actual measurement would be nessesary with classical literature to determine the location of acupuncture point, and we need more study in other race.

An effective method to combinate the meridian point could be said, in this study, as follows: 1. A combination method of the meridian points of connecting meridian is the basic treatment combination point in disease of Meridian. 2. A combination method of the meridian points to mother and child is adapted in the treatment of bowels chiefly, but of Meridian additionally. 3. A combination method of the meridian points in anterior and posterior strengthen the function of the entrails as a mutual response. 4. A combination method of the meridian points in superficial and internal match the Meridian of Yin and Yang, specially the acupuncture of the back. 5. A combination method of the meridian points in upper and lower is to match arms and legs. 6. A combination method of the meridian points in fight and left is to treatment the disease of human bodies by matching the right and left sides effectively. 7. A combination method of the meridian points in far and near is to treatment the near and far side simultaneously. 8. A combination method of the meridian points according to some symptoms treatment the chief side of disease by finding a bad condition of human beings correctly. As above said, the combination method of the meridian points could accept a reactive way fundamentally in accordance with the cause and effect of disease, furthermore match more than two ways.
