extracorporeal membrane oxygenation; HIV; Pneumocystis jirovecii; respiratory failure.
2016 Acute and Critical Care Vol.31 No.2
The shortage of available organ donors is a significant problem and various efforts have been made to avoid the loss of organ donors. Among these, extracorporeal membrane oxygenation (ECMO) has been introduced to help support and manage potential donors. Many traumatic brain injury patients have healthy organs that might be eligible for donation for transplantation. However, the condition of a donor with a fatal brain injury may rapidly deteriorate prior to brain death determination; this frequently results in the loss of eligible donors. Here, we report the use of venoarterial ECMO to support a potential donor with a fatal brain injury before brain death determination, and thereby preserve donor organs. The patient successfully donated his liver and kidneys after brain death determination.

This paper tries to analyze the illustrations of Choo Choo by Virginia Lee Burton. In this work, the author attracts children's interests using language and pictures. In language, the author makes visual effects with the arrangement and sizes of letter and uses various onomatopoeic expressions such as choo choo, ding dong, sssswish, whoooo, ch, ah choo rendering the mood real to children. In pictorial technique, the author uses colors, shapes and personifications, to create various effects. Except the pictures colored in yellow, green, red and blue in the first and last chapters, the rest is all in black-and-white, where the black color represents the soot of the coal smudged in the body of the engine. The symbol of a locomotive could be the white steam rising from the flue; in this work, the shape, length, and size of the steam imply the changes of the situation and the difference in the duties of the characters. Mountains, farmyard, hills, switchyard, railroad drawn in curves symbolizes the maternal love and the author's love toward children. The pictures in a large scale on the big screen represent the dynamic wakes of the train in the American continent. The dynamic movement of the train is well-rendered in an S-shaped perspective while the pictures drawn in an oblique line render the impression that something has quickly passed. In these ways, Virginia Lee Burton creates a very impressive work.
Vol.1 No.-
