      • S-73 Endoscopic subepithelial tumor resection using Band ligation-assisted Over-the-sc ope Clip Method

        Background: The purpose of this study is to demonstrate that endoscopic lumpectomy using over-the-scope clip (OTSC) device enables safe and complete removal along with definite diagnosis of small SET lesion.?Methods: A total of four patients were removed SET lesions using OTSC device in March, 2015. Every lesion was performed endoscopic resection after performing EUS. First, we suctioned both SET lesion and overlying mucosa into the endoscopic cap and tied below the tumor by rubber band to get the entire tumor. Then, an OTSC was mounted onto the tip of endoscope with an application cap, and the band-ligated tumor was once again suctioned into the cap. We released clip under the rubber band to prevent perforation. After that, we cut the gap between the ligation band and the clip by endoscopic snare.?Results: Three SET lesions were located in stomach. Every lesion was less than 2cm in size, and was originated from proper muscle layer as observed through endoscopic ultrasonography. En bloc resection was performed in all cases. Histologically, two stomach lesions turned out to be low grade GIST, and the other was leiomyoma.There were no significant complications, such as delayed bleeding or perforation. Conclusions: GIST or carcinoid suspected small epithelial tumors cannot be surgically resected, but can be successfully removed through endoscopic muscular layer resection using band ligation-assisted OTSC.?Keywords: Subepithelial tumor, Endoscopic muscular layer resection, Over-the-scope clip

        <B>Graphic Abstract</B> <P>An efficient and practical protocol in atmospheric and room-temperature conditions for the direct surface modification of PDMS is achieved using X-ray irradiation.
