Purpose: We conducted a study to evaluate the relationship between body mass index (BMI) and the risk of breast cancer (BC) and outcome in a population of 14,684 women aged 55 to 69 years eligible to participate in the Mammography Screening Program (MSP) in the Province of Modena, Italy. Methods: The study population was drawn from women who underwent mammography screening between 2004 and 2006 in the Province of Modena. Women were subdivided into obese, overweight, and normal-weight categories according to BMI and followed until July 31, 2010, to evaluate the BC incidence. The clinicopathological characteristics of BC were also evaluated in different groups of patients classified according to BMI. After BC diagnosis, patients were followed for a median period of 65 (range, 2–104) months. Second events (recurrences and second tumors) were recorded, and the 5-year event-free survival (EFS) was calculated. Results: After a period of 73 months, 366 cases of BC were diagnosed. Compared with normal-weight women, obese women had a significantly higher incidence of BC (relative risk [RR], 1.32; p=0.040) (RR=1), larger tumors (27% of tumors were larger than T2 size), and more nodal involvement (38.5% of tumors were node-positive). Furthermore, a significantly higher rate of total events was seen in obese women compared with overweight and normal-weight patients, respectively (17.9% vs. 11.4% vs. 10.8%, p=0.032). The 5-year EFS was 89.0%, 89.0%, and 80.0% for normal-weight, overweight, and obese patients, respectively. Conclusion: We observed a significantly higher risk of BC in obese women among those eligible to participate in the MSP in the Province of Modena. Finally, obese women had more second events and poorer EFS compared to nono bese women.
A new method to predict the critical incidence angle for buildings under near-fault motions

It is well known that the incidence angle of seismic excitation has an influence on the structural response of buildings, and this effect can be more significant in the case of near-fault signals. However, current seismic codes do not include detailed requirements regarding the direction of application of the seismic action and they have only recently introduced specific provisions about near-fault earthquakes. Thus, engineers have the task of evaluating all the relevant directions or the most critical conditions case by case, in order to avoid underestimating structural demand. To facilitate the identification of the most critical incidence angle, this paper presents a procedure which makes use of a two-degree of freedom model for representing a building. The proposed procedure makes it possible to avoid the extensive computational effort of multiple dynamic analyses with varying angles of incidence of ground motion excitation, which is required if a spatial multi-degree of freedom model is used for representing a building. The procedure is validated through the analysis of two case studies consisting of an eight- and a six-storey reinforced concrete frame building, selected as representative of existing structures located in Italy. A set of 124 near-fault ground motion records oriented along 8 incidence angles, varying from 0 to 180 degrees, with increments of 22.5 degrees, is used to excite the structures. Comparisons between the results obtained with detailed models of the two structures and the proposed procedure are used to show the accuracy of the latter in the prediction of the most critical angle of seismic incidence.

Extended lymphadenectomy in locally advanced rectal cancers: a systematic review

Purpose: The surgical treatment of advanced low rectal cancer remains controversial. Extended lymphadenectomy (EL) is the preferred option in the East, especially in Japan, while neoadjuvant radiotherapy is the treatment of choice in the West. This review was undertaken to review available evidence supporting each of the therapies. Methods: All studies looking at EL were included in this review. A comprehensive search was conducted as per PRISMA guidelines. Primary outcome was defined as 5-year overall survival, with secondary outcomes including 3-year overall survival, 3- and 5-year disease-free survival, length of operation, and number of complications. Results: Thirty-one studies met the inclusion criteria. There was no significant publication bias. There was statistically significant difference in 5-year survival for patient who underwent EL (odds ratio, 1.34; 95 confidence interval, 0.09–0.5; P=0.006). There were no differences noted in secondary outcomes except for length of the operations. Conclusion: There is evidence supporting EL in rectal cancer; however, it is difficult to interpret and not easily transferable to a Western population. Further research is necessary on this important topic.

The development of a prosthetic hand able to replicate as much as possible the grasping and sensory features of the natural hand represents an ambitious project for scientists. State of the art technology is still far to provide engineers with components with similar performance of their natural models, and active prosthetic hands can be only a pale replication of the missing natural limb. <BR>This paper presents the current research efforts towards the development of a self-adaptative and anthropomorphic prosthetic hand. In particular, the paper is focused on the problem of replicating the natural sensory system of the hand with an artificial proprioceptive and exteroceptive sensory system.

<P>Rechargeable lithium-sulfur (Li-S) batteries have drawn significant attention as next-generation energy storage systems. Sulfur-copolymers are promising alternative cathode materials to elemental sulfur in Li-S batteries as they provide high reversible capacity. However, the redox mechanisms of these materials are not well understood owing to the difficulty in characterizing amorphous structures and identifying individual ionic species. Here, we use solid-state NMR techniques together with electrochemistry experiments and quantum calculations to investigate the structural evolution of the prototype S-copolymer cathodes, sulfur-diisopropenylbenzene copolymers (poly(S-co-DIB)), during cycling. We demonstrate that polysulfides with different chain lengths can be distinguished by <SUP>13</SUP>C and <SUP>7</SUP>Li NMR spectroscopy, revealing that the structure of the copolymers can be tuned in terms of polysulfide chain lengths and resulting reaction pathways during electrochemical cycling. Our results show that the improved cyclability of these cathodes originates from the role of organic moieties acting as anchors that fixate polysulfides to the polymeric network during cycling, thus preventing their diffusion into the electrolyte. We provide a new methodological concept for the mechanistic studies to track the intermediate species and phase transition in Li-S batteries.</P> [FIG OMISSION]</BR>
