General Well-being and Coping Strategies in Adult Eosinophilic Esophagitis Patients

Background/AimsGrowing evidence suggests a negative effect of eosinophilic esophagitis (EoE) on patients’ general health-related quality of life (HRQOL). However, the relevance and use of coping strategies and its relation to (disease specific) HRQOL as well as its determinants have not been studied well. MethodsAdult EoE patients were invited to complete standardized measures on general HRQOL (Short Form-36 Health Survey [SF-36]) and coping strategies (Utrechtse Coping Lijst [UCL]). Scores were compared to general population norms. The disease specific Adult Eosinophilic Esophagitis Quality of Life (EoE-QOL-A) measure was used to assess EoE-HRQOL. Socio-demographic-and clinical factors were also evaluated. ResultsIn total, 147 adult EoE patients (61% males), age 43 (interquartile range, 29-52) years were analyzed. Mental health-scores (SF-36) were significantly lower in EoE patients, whereas physical health-scores (SF-36) were similar in EoE patients (vs the general population; P = 0.010 and P = 0.240), respectively. The subdomain “disease anxiety” (EoE-QOL-A) was mostly affected, determinants were; female gender, younger age, severe clinical disease activity, higher number of food bolus extraction, and more recent EoE-diagnosis. Less effective coping styles (ie, passive/palliative reaction) were associated with a significant impact on each individual EoE-HRQOL-subdomain as well as lower scores of the Mental Health Component Scale in male EoE patients. Passive reaction in female EoE-patients correlated with impairment of the EoE-HRQOL-domains “emotional impact” and “disease anxiety.” Active problem solving was significantly related to better perception of mental HRQOL (SF-36) in both males and females. ConclusionsEoE has a significant negative impact on mental HRQOL, with less effective coping strategies––specifically in males, being a relevant determinant. Thus, a pro-active approach towards coping mechanisms is needed in order to enhance HRQOL and manage patients’ burden of EoE.

The Effect of Cracks on Chloride Penetration into Concrete

<P>This study is focused on examining the effect of critical crack width in combination with crack depth on chloride penetration into concrete. Because concrete structures have to meet a minimum service-life, critical crack width has become an important parameter. Specimens with different crack width / crack length have been subjected to rapid chloride migration testing (RCM). The results of this study show a critical crack width of about 0.012 mm. Cracks smaller than this critical crack width are considered not to have a significant influence on the rate of chloride transport inwards, while chloride penetration does proceed faster above this critical crack width.</P>
Current Self-Balancing Mechanism in ZVS Full-Bridge Converters

Full-bridge converters are widely used in isolated dc-dc conversion and wireless power transfer systems. A full-bridge converter can operate in zero-voltageswitching (ZVS) mode with the assistance of an inductive branch that connects the two switch nodes in parallel with the load. The inductive branch provides a ZVS current and has a very low resistance. A tiny volt-second imbalance between the switch nodes may cause a large current bias on the branch. However, it can be demonstrated experimentally that the current on the branch can be almost self-balanced without any dc blocking capacitor or balancing control. This paper quantitatively studies the current self-balancing mechanism and shows that the dead-time switch-node voltage can give a strong negative feedback effect that pushes the current bias toward zero. Experimental verification was carried out on a gallium-nitride device-based converter.

In this paper, we introduce a charge integrator as a read-out system for piezoelectric cantilever sensors. The charge integrator is used to monitor the surface charge induced by piezoelectric materials as a function of mechanical stress. This technique improves the response of piezoelectric cantilever sensors in low frequency measurements which makes them suitable for chemical and biological detection. Furthermore, it is capable of real-time, portable and allelectrical detection. The detection of alkanethiols on the Au-coated piezoelectric cantilevers is demonstrated to verify the concept.

An algorithm has been developed to determine changes in surface topography on asperity level. The software stitches small but detailed images together to create one large image. If such an image is made before and after an experiment, their difference shows a direct 3D view of the changes in micro-geometry, rather than a change in surface parameters. The algorithm is described in detail and illustrated using artificial as well as real surfaces.
