      • Design Robust Artificial Intelligence Model-base Variable Structure Controller with Application to Dynamic Uncertainties OCTAM VI Continuum Robot

        Design a robust artificial intelligent nonlinear controller for second order nonlinear uncertain dynamical systems is one of the most important challenging works. This paper focuses on the design of a robust chattering free mathematical model-base artificial intelligence (fuzzy inference system) variable structure controller (MFVSC) for highly nonlinear dynamic continuum robot manipulator, in presence of uncertainties. In order to provide high performance nonlinear methodology, variable structure controller is selected. Pure variable structure controller can be used to control of partly known nonlinear dynamic parameters of continuum robot manipulator. In order to reduce/eliminate the chattering, this research is used the artificial intelligence (fuzzy logic) theory. The results demonstrate that the model base fuzzy variable structure controller with switching function is a model-based controllers which works well in certain and partly uncertain system. Lyapunov stability is proved in mathematical model-based fuzzy variable structure controller with switching (sign) function. This controller has acceptable performance in presence of uncertainty (e.g., overshoot=1%, rise time=0.9 second, steady state error = 1.6e-8 and RMS error=4.8e-8).

        Abnormal frequency of the memory B cell subsets and plasmablasts in patients with congenital severe hemophilia A: correlation with “Inhibitor” formation

        Background Development of antibodies against infused Factor VIII (FVIII) or "inhibitors" represents a major challenge following FVIII replacement therapy in patients with hemophilia A (HA). Recent studies have shown that certain cellular compartments of the immune system contribute to the production of such antibodies. Herein, we determined the frequency of class-switched CD19+ IgD−CD27+/non-class-switched CD19+ IgD+CD27+ memory B cell subsets and CD19+ CD27hiCD38hi plasmablasts in patients with severe HA and their association with the development of inhibitors in these patients. Methods This cross-sectional case–control study enrolled 32 patients with severe HA, including 8 with and 24 without inhibitors, and 24 healthy individuals. The frequencies of the memory B cell subsets and plasmablasts were determined using flow cytometry. Results The frequency of CD19+ IgD+CD27+ non-class-switched memory B cells was significantly lower in patients with HA (including both patients with and without inhibitors) than in healthy controls. The percentages of both CD19+ IgD−CD27+ class-switched and CD19+ IgD+CD27+ non-class-switched memory B cells did not differ significantly between patients with and without inhibitors. HA patients with inhibitors had significantly higher proportions of CD19+ CD27hiCD38hi plasmablasts than the control group as well as the inhibitor (-) ones. No significant correlation was observed between the inhibitor levels with the percentages of memory B cell subsets and plasmablasts. Conclusion This study is the first to demonstrate a dysregulated proportion of CD19+ IgD+CD27+ non-class-switched memory B cells and CD19+ CD27hiCD38hi plasmablasts in patients with severe HA. Therefore, strategies targeting memory B-cell/plasmablast differentiation may have promising outcomes in the management of inhibitor formation in patients with severe HA.

        A simple mathematical model for static analysis of tall buildings with two outrigger-belt truss systems

        In this paper a simple mathematical model for approximate static analysis of combined system of framed tube, shear core and two outrigger-belt truss structures subjected to lateral loads is presented. In the proposed methodology, framed tube is modeled as a cantilevered beam with a box section and interaction between shear core and outrigger-belt truss system with framed tube is modeled using torsional springs placed at location of outrigger-belt truss; these torsional springs act in a direction opposite to rotation generated by lateral loads. The effect of shear lag on axial deformation in flange is quadratic and in web it is a cubic function of geometry. Here the total energy of the combined system is minimized with respect to lateral deflection and rotation in plane section. Solution of the resulting equilibrium equations yields the unknown coefficients of shear lag along with the stress and displacement distributions. The results of a numerical example, 50 storey building subjected to three different types of lateral loading obtained from SAP2000 are compared to those of the proposed method and the differences are found to be reasonable. The proposed method can be used during the preliminary design stages of a tall building and can provide a better understanding of the effects of various parameters on the overall structural behavior.

        Measurement of Clavicle Fracture Shortening Using Computed Tomography and Chest Radiography

        Background: Nonoperative management of midshaft clavicle fractures has resulted in widely disparate outcomes and there is growing evidence that clavicle shortening poses the risk of unsatisfactory functional outcomes due to shoulder weakness and nonunion. Unfortunately, the literature does not clearly demonstrate the superiority of one particular method for measuring clavicle shortening. The purpose of this study was to compare the accuracy of clavicle shortening measurements based on plain radiographs with those based on computed tomography (CT) reconstructed images of the clavicle. Methods: A total of 51 patients with midshaft clavicle fractures who underwent both a chest CT scan and standardized anteroposterior chest radiography on the day of admission were included in this study. Both an orthopedic surgeon and a musculoskeletal radiologist measured clavicle shortening for all included patients. We then determined the accuracy and intraclass correlation coefficients for the imaging modalities. Bland-Altman plots were created to analyze agreement between the modalities and a paired t-test was used to determine any significant difference between measurements. Results: For injured clavicles, radiographic measurements significantly overestimated the clavicular length by a mean of 8.2 mm (standard deviation [SD], ± 10.2; confidence interval [CI], 95%) compared to CT-based measurements (p < 0.001). The intraclass correlation was 0.96 for both plain radiograph- and CT-based measurements (p = 0.17). Conclusions: We found that plain radiograph-based measurements of midshaft clavicle shortening are precise, but inaccurate. When clavicle shortening is considered in the decision to pursue operative management, we do not recommend the use of plain radiograph-based measurements.

        Prevalence of complications associated with polymer-based alloplastic materials in nasal dorsal augmentation: a systematic review and meta-analysis

        Background: Various techniques with different grafts and implants have been proposed to establish a smooth and symmetric nasal dorsum with adequate function. Broadly, two categories of materials have been used in this regard: alloplastic implant materials and autograft materials. The aim of these meta-analyses is to explore the incidence of complications after dorsum augmentation surgery using alloplastic materials. Materials and methods: After duplication removal 491 papers remained that title and abstract were assessed for eligibility. Regarding the study type, 27 observational studies were included, 21 retrospective and 6 prospective case series. A total of 3803 cases were enrolled in this systematic review and meta-analysis. Result: Twenty-seven articles reported on complications and outcomes of dorsal augmentation rhinoplasty with synthetic materials. In a random-effects model, the weighted mean percentage was 2.75% (95% CI 1.61 to 4.17%). the weighted mean percentage were 1.91% (95% CI 0.77 to 3.54%), 0.72% (95% CI 0.316 to 1.31%), and 0.78% (95% CI 0.43 to 1.24%) respectively. Conclusion: The widely used alloplasts were expanded polytetrafluoroethylene (ePTFE), high-density polyethylene, and silicone. The total rates for complications, infection, deviation, irregularity, hematoma, extrusion, and overcorrection were 2.75%, 1.91%, 0.72%, 0.70%, 0.78%, and 0.49%, respectively. The revision rate, based on the random effects model, was 6.40% with 95%CI (3.84 to 9.57). Trial registration: This meta-analysis was registered at the International Prospective Register of Systematic Reviews (PROSPERO, registration number CRD42 02020 9644).

        Reopening of dental clinics during SARS-CoV-2 pandemic: an evidence-based review of literature for clinical interventions

        Background: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes serious acute respiratory diseases including pneumonia and bronchitis with approximately 2.3% fatality occurrence. Main body: This study argues the main concepts that need to be considered for the gradual reopening of dental offices include treatment planning approaches, fundamental elements needed to prevent transmission of SARS-CoV-2 virus in dental healthcare settings, personal protection equipment (PPE) for dental health care providers, environmental measures, adjunctive measures, and rapid point of care tests in dental offices. Conclusion: This article seeks to provide an overview of existing scientific evidence to suggest a guideline for reopening dental offices.

        Using Multi Criteria Decision Making for Selection the Telecom Operators` Strategy for OTT Services

        While mobile operators are competing closely, new players of OTT (Over-The-Top) services such as Skype, WhatsApp, Instagram providing communication and media services in the context of mobile communications have created a new space in the ecosystem of mobile services. These companies make use of the internet capacity of mobile operators instead of creating a dedicated network. However, on the one hand, mobile operators are in need of huge investment in order to take advantage of extensive telecommunication networks, and their income, on the other hand, has been significantly reduced. Therefore the success of mobile operators need to adopt a right strategy against OTT services. In this paper, strategies using Strengths, Weaknesses, Opportunities and Threats (SWOT) analysis were developed by evaluating the external and internal environment factors of Mobile Telecommunication Company of Iran(MCI) as well as using library and field studies into company experts. Then, the strategies were prioritized using Analytical Network Process (ANP) as an efficient techniques of Multi Criteria Decision Making (MCDM) which deals with dependencies between criteria. MCI should be focus on the strategy of “cooperation and partnership with OTT companies” and then toward the strategy of “creation of innovative service and penetration into new markets.”

        Progressive Collapse of Exterior Reinforced Concrete Beam–Column Sub-assemblages

        Many experimental studies have evaluated the in-plane behavior of reinforced concrete frames in order to understand mechanisms that resist progressive collapse. The effects of transverse beams, frames and slabs often are neglected due to their probable complexities. In the present study, an experimental and numerical assessment is performed to investigate the effects of transverse beams on the collapse behavior of reinforced concrete frames. Tests were undertaken on a 3/10-scale reinforced concrete sub-assemblage, consisting of a double-span beam and two end columns within the frame plane connected to a transverse frame at the middle joint. The specimen was placed under a monotonic vertical load to simulate the progressive collapse of the frame. Alternative load paths, mechanism of formation and development of cracks and major resistance mechanisms were compared with a two-dimensional scaled specimen without a transverse beam. The results demonstrate a general enhancement in resistance mechanisms with a considerable emphasis on the flexural capacity of the transverse beam. Additionally, the role of the transverse beam in restraining the rotation of the middle joint was evident, which in turn leads to more ductile behavior. A macro-model was also developed to further investigate progressive collapse in three dimensions. Along with the validated numerical model, a parametric study was undertaken to investigate the effects of the removed column location and beam section details on the progressive collapse behavior.
