        Daily Pad Usage Versus the International Consultation on Incontinence Questionnaire Short Form for Continence Assessment Following Radical Prostatectomy

        Purpose: Continence assessment is an essential component of follow-up after radical prostatectomy (RP). Several methods exist to assess the severity of urinary incontinence (UI). Our study examined the relationship and degree of agreement between International Consultation on Incontinence Questionnaire Short Form (ICIQ-SF) scores and the number of pads used in a 24-hour period in the assessment of UI following RP. Methods: Continence was prospectively assessed in 746 men from a Spanish urology clinic 12 months after RP using the ICIQ-SF and pad usage. The relationship between ICIQ-SF scores and pad usage was assessed using Spearman rank correlation coefficients. The Jonckheere-Terpstra trend test was used to determine whether the ICIQ-SF score and the component question scores increased with increasing pad usage. The Bonferroni-corrected pairwise Wilcoxon rank-sum test was used to determine which pairs of pad usage levels differed. The weighted kappa was used to evaluate the agreement between pad usage levels and ICIQ-SF questions. Results: The continence rate was 82% using the “no pad usage” definition of continence versus 78% using the definition of an ICIQ-SF score of 0 (P<0.001). Strong positive correlations were observed between the number of pads and the ICIQ-SF total and component question scores (rs>0.85, P<0.001). The ICIQ-SF total and component question scores increased significantly with increasing pad usage (P<0.001). The ICIQ-SF scores (P<0.018) for all pairs of pad usage levels (0, 1, 2, or 3 or more) differed significantly. The agreement between the ICIQ-SF leakage amount question and pad usage was very good (rs=0.861, P<0.001). Conclusions: At 12 months post-RP, 24-hour pad usage was closely correlated with ICIQ-SF, although the continence rate differed depending on the definition used. Higher levels of pad usage were associated with higher questionnaire scores, more leakage, and poor quality of life (interference with everyday life).

      • Psychosocial Factors and Injury Risk in Taekwondo: An Exploratory Prospective Cohort Study

        The purpose of this study was to examine the psychological profile of Taekwondo athletes competing at the 2011 Australian National Championships and to explore the potential relationship between psychosocial variables and injury risk. Forty-five athletes aged 16 years and older consented to complete a pre-tournament survey comprising demographic information and a battery of questionnaires designed to quantify four separate psychosocial factors (i.e., life-changing events, competition anxiety, coping skills, and social support). Older athletes had significantly lower competition anxiety (p = 0.035) and greater satisfaction with social support (p = 0.019) relative to younger athletes. Female athletes reported significantly fewer positive life events (p = 0.039) compared to their male counterparts. These findings indicate that younger or inexperienced athletes and females may be at greater risk from psychosocial stressors. Targeted programs to address these stressors may be beneficial; however, further research is needed to elucidate the relationships between injury risk and psychosocial factors in Taekwondo athletes.

        No retreat from the heat: temperature-related risk of violent assault is increased by being inside

        Studies finding an association between increasing temperature and violent crime have largely overlooked the potential effect modification of crime location. This study analysed 13 years of reported violent crime to investigate if there is a relationship between daily ambient temperature and crime type (domestic, non-domestic and sexual assault), and how this differs by whether the crime occurred inside or out. We found that predicted daily counts of domestic violence increased with temperature, and this association was greater for locations inside. Non-domestic assaults also increased with higher temperature, however when compared by location the incidents that occurred outside rose to around 30°C then plateaued while inside the risk continued to rise. Sexual assault peaked and then declined at around 30°C in inside and outside locations. The findings support the development of prevention and preparedness strategies by considering how the complex drivers behind the temperature–aggression association are modified by the location where it occurred.

        A novel human cadaver model to investigate a retrourethral transobturator male sling procedure

        Purpose: To develop a cadaver model for the assessment of a male transobturator male sling (retrourethral transobturator sling [RTS], AdVance™; Boston Scientific, USA) to investigate its effect on a simulated abdominal and retrograde leak point pressures (ALPP, RLPP) and the urethral pressure profile (UPP). Materials and Methods: Three fresh frozen human male cadaver specimens were obtained. A suprapubic tube was inserted into the bladder and connected to a digital manometer to measure bladder pressure. Manual suprapubic pressure was then applied to generate an increase in intraabdominal pressure and measure a simulated ALPP. Subsequent measurements of RLPP and UPP were recorded. All measurements were undertaken prior to and following insertion of a RTS. Results: The placement of the RTS consistently increased the simulated ALPP for all three cadaver specimens when compared to baseline measures. No leaks occurred at simulated ALPP's of 170 cm H2O for specimen 1, 160 cm H2O for specimen 2, and 170 cm H2O for specimen 3. There was minimal or no change in the RLPP's and UPP's following insertion of the RTS when compared to respective baseline. Conclusions: A model using fresh unfixed cadavers that incorporates a simulated measurement of ALPP is feasible for male stress urinary incontinence surgical intervention investigations.
