        Efficacy of a Second Brain Biopsy for Intracranial Lesions after Initial Negativity

        Background and Purpose The rationale for performing a second brain biopsy after initial negativity is not well evaluated in the literature. This study was designed to 1) assess the efficacy of a second brain biopsy when the first biopsy was nondiagnostic, 2) identify possible factors associated with an increased diagnostic rate in the second biopsy, and 3) analyze additional morbidity induced by the second biopsy. Methods We performed a retrospective cohort study from 2009 to 2019, during which 1,919 patients underwent a brain biopsy, including 30 who were biopsied twice (1.6%). The specific histological diagnosis rate, diagnosis-associated factors, and complication rate were assessed for the 30 twice-biopsied patients. Results The second biopsy allowed a specific histological diagnosis in 86.7% of the patients who had initially undergone a nondiagnostic brain biopsy [odds ratio (OR)=7.5, 95% confidence interval (CI)=3.0–18.7, p<0.001]. The multivariate analysis showed that only prebiopsy corticosteroid administration (OR=2.6, 95% CI=1.1–6.0, p=0.01) was an important factor in predicting a nondiagnostic biopsy. None of the patients developed a symptomatic complication after the first biopsy, while two (6.0%) patients experienced a transient complication after the second biopsy (p=0.49). Conclusions Performing a second brain biopsy in patients who have an initial nondiagnostic biopsy is effective in most cases. We advocate that a second biopsy be systematically considered in the diagnosis algorithm of these patients after it has been verified that molecular testing cannot help to obtain a diagnosis. Corticosteroid administration can lead to nondiagnostic biopsies and should be avoided when possible during the prebiopsy period.

        Phenolic-Rich Extract from Almond (Prunus dulcis ) Hulls Improves Lipid Metabolism in Triton WR-1339 and High-Fat Diet-Induced Hyperlipidemic Mice and Prevents Lipoprotein Oxidation: A Comparison with Fenofibrate and Butylated Hydroxyanisole

        Hyperlipidemia and oxidative stress are risk factors for atherosclerosis. In this study, we investigated the hypolipidemic and anti-lipoprotein oxidation activities of polyphenol-rich extracts from almond hulls using Triton WR-1339 and high-fat diet-induced hyperlipemic mice as experimental models. We demonstrated that the almond hull extract significantly reduced total cholesterol, triglycerides and low-density lipoprotein-related plasma cholesterol (LDL-C) in the two experimental models of hyperlipidemia, but significantly increased high-density lipoprotein-related plasma cholesterol (HDL-C). Another beneficial effect of the extract was its ability to reduce the atherogenic index and LDL-C/HDL-C ratio. However, the extract exhibited effective antiradical activity against 2,2-diphenyl-1-picrylhydrazyl and significantly protected lipoprotein-rich plasma from mice against oxidation induced by copper ion. The extract contains 342.63±3.44 ㎎/g total phenolics, 144.67±6.83 ㎎/g tannins, and 20.66±0.92 ㎎/g flavonoids. These finding indicate that almond hulls contain polar products able to lower plasma lipid concentrations and which might be beneficial for the treatment of hyperlipidemia and prevention of atherosclerosis.

        In the maritime industry, most perceptions, frameworks and methodologies of dealing with hazards are for their risk assessment rather than their risk management. This tendency discloses the reality that within the maritime sectors in areas like shipping, logistics, oil and gas there is a lack of coherent Quantitative Risk Management (QRM) methodology from which to understand the risk-based decisions especially for appropriate risk management such as in seaports’ terminals. Therefore, in this paper initially, during priority assessment of the identified hazards, Fuzzy Set Theory was applied to handle imprecision of the uncertain risk-based statistics to get an accurate result. In the next stage, Fuzzy Fault Tree and Fuzzy Event Tree methods were used to achieve the sequence of quantitative risk analysis. In the final step, a Fuzzy Technique for Order of Preference by Similarity to Ideal Solution tool was used for the implementation of the mitigation phase to complete and conclude the proposed QRM cycle.

      • A Proposed Method for Ship Safety Enhancement After Flooding

        Bulk carrier ship type is one of the three dominant types of merchant vessels along with container ships and tankers. Recently, in terms of tonnage, bulk carriers constitute about the world's fleet third. The stuff materials indigence such as grains, fertilizers, ores, etc. Since the turn of the millennium, the number has increased considerably. Recently, a growing variance in bulk cargo which has transported by sea, represents now a large part of international commercial exchange shipped by sea. Every year there are huge number tons of cargoes like steel, coals, livestock feed, copper and minerals that are transported by sea in bulk. While most of those shipments are made without accidents, a number of serious injuries have occurred which have resulted not only in the ship's loss, but also in lives loss. In this paper, there is an existing bulk carrier vessel, a computational case study, is made to investigate the different loading conditions effects and bulk cargoes’ types on ship’s stability, in case of damaged conditions, in addition to the effect on the ship’s hull longitudinal strength. A proposed method is suggested to improve the level of safety of the ship past flooding by the usage of air bag (cushion). The candidate vessel consists of two cargo holds. In this paper there are three investigated damage scenarios, the first one is checking the stability in case of damage of cargo hold No.1. The second one is checking the stability in case of damage of cargo hold No.2 and the third scenario is checking the stability in case of flooding of both of the two cargo holds. There’s a modelling software, Auto Ship, is used here to model the vessel, simulate the different scenarios and to run the stability code to check the stability criteria. Finally, after performing the above mentioned three damage scenarios, the result was that the ship will still float in case of scenario one or two but in case of the third scenario, it will sink. So, this study has suggested a way to keep the vessel floating in case of the third scenario until reaching the nearest port without sinking. This proposed method is by using air cushions to open directly in case of existing alarm in the bulk carrier work to fill all the space inside the cargo hold instead of filling with the water.

        Using XFEM Technique to Predict the Effect of Default on the Damage of Steel Pipe Reduced-Connection Under Bending and Pressure Loading

        Over the years of service and by effect of their typical geometries, pressurized tubular structures assembled by a connection of an elbow or a reduced Tee or other, are much more stressed compared to straight tubes at significant normal and tangential stresses. With the presence of faults, these connections are subsequently largely responsible for their damage. Damage to an X60 Grade 8 ”x4” steel tube Reduced Tee connection element under oriented loading and internal pressure with or without the presence of a defect was investigated in this work. Because of these advantages and in order to present the propagation of a crack, we opted for the use of the XFEM technique (extend finite element method). Numerical prediction using the ABAQUS compute code set out to assess dependent parameters such as location and type of fault as well as mode and accumulated load level. Indeed, the loading mode clearly showed a large difference between the response down to the level of damage. These results are presented by curves which present both the response of a structure in force-displacement and the crack propagation (initiation and critical length). The default location in the structure at the connection joint is oriented according to the loading mode. The different situations analyzed affect all possible cases that the element of the reduced Tee may undergo. The type of defect is related by its location according to the thickness in the form of a metal tear. This damage analysis shows that the parameters studied in the structure clearly condition the level and mode of failure as well as their response to damage.

      • Prevalence of IDH1/2 Mutations in Different Subtypes of Glioma in the North-East Population of Morocco

        Background: Genetic alterations in gliomas have increasing importance for classification purposes. Thus, we are especially interested in studying IDH mutations which may feature potential roles in diagnosis, prognosis and response to treatment. Our aim was to investigate IDH mutations in diffuse glioma patients diagnosed in university hospital centre of Fez in Morocco. Materials and Methods: IDH1 codon 132 and IDH2 codon 172 were direct-sequenced in 117 diffuse glioma samples diagnosed and treated in University Hospital Hassan II between 2010 and 2014. Results: The R132H IDH1 mutation was identified in 43/117 tumor samples and R172K IDH2 mutation was detected in only one anaplastic oligodendroglioma. IDH mutations were observed in 63.2% of astrocytomas, 73.3% of diffuse oligodendrogliomas and 12.90% of glioblastomas. Conclusions: Our results confirmed other studies published earlier for other populations with some small discrepancies.
