      • Coordinated Control of Multiple Unified Power Flow Controllers for Stability Enhancement of the Multi-Machine Power System

        This paper presents an application of UPFC (unified power flow controller), a kind of FACTS (flexible AC transmission systems) devices, to stability enhancement in a multi-machine power system. A novel control scheme using wide-area information from generators in the system is proposed for two parts of UPFC, shunt and series connected components, which mainly affect bus voltage and line active power flow around the installation site respectively. The proposed control scheme is based on a control law for shifting a time derivative value of the kinetic energy function in the negative direction. Numerical simulations are carried out on a 27-machine meshed test system where several UPFCs are installed, and the impacts of the two controlled components of UPFC and number of UPFCs on stability enhancement are investigated. The results show that both of the UPFC components controlled by the proposed method are effective for damping enhancement and the shunt connected component is particularly effective for the first-swing stability improvement. It is also made clear that the proposed control scheme can control multiple UPFCs without mutual interference.

        The feasibility of percutaneous transhepatic gallbladder aspiration for acute cholecystitis after self-expandable metallic stent placement for malignant biliary obstruction: a 10-year retrospective analysis in a single center

        Background/Aims: Patients with acute cholecystitis (AC) after metallic stent (MS) placement for malignant biliary obstruction (MBO)have a high surgical risk. We performed percutaneous transhepatic gallbladder aspiration (PTGBA) as the first treatment for AC. Weaimed to identify the risk factors for AC after MS placement and the poor response factors of PTGBA. Methods: We enrolled 401 patients who underwent MS placement for MBO between April 2011 and March 2020. The incidence of ACwas 10.7%. Of these 43 patients, 37 underwent PTGBA as the first treatment. The patients’ responses to PTGBA were divided into goodand poor response groups. Results: There were 20 patients in good response group and 17 patients in poor response group. Risk factors for cholecystitis after MSplacement included cystic duct obstruction (p<0.001) and covered MS (p<0.001). Cystic duct obstruction (p=0.003) and uncovered MS(p=0.011) demonstrated significantly poor responses to PTGBA. Cystic duct obstruction is a risk factor for cholecystitis and poor re-sponse factor for PTGBA, whereas covered MS is a risk factor for cholecystitis and an uncovered MS is a poor response factor of PTG-BA for cholecystitis. Conclusions: The onset and poor response factors of AC after MS placement were different between covered and uncovered MS. PTG-BA can be a viable option for AC after MS placement, especially in patients with covered MS.

      • Current Status of the Diagnosis and Management of Pancreatic Neuroendocrine Tumors in Japan

        The epidemiology of pancreatic neuroendocrine neoplasms (PNENs) in Asia has been clarified through epidemiological studies, including one conducted in Japan, and subsequently another in South Korea. As endoscopic ultrasonography (EUS) has become more widely accessible, endoscopic ultrasound-fine needle aspiration (EUS-FNA) has been performed in pancreatic tumors for which the clinical course was only monitored previously. This has enabled accurate diagnosis of pancreatic tumors based on the 2010 WHO classification; as a result, the number of patients with an accurate diagnosis has increased. Although surgery has been the standard therapy for PNENs, new treatment options have become available in Japan for the treatment of advanced or inoperable PNENs; of particular note is the recent introduction of molecular target drugs (such as everolimus and sunitinib) and streptozocin. Treatment for progressive PNENs needs to be selected for each patient with consideration of the performance status, degree of tumor differentiation, tumor mass, and proliferation rate. Somatostatin receptor (SSTR)-2 is expressed in many patients with neuroendocrine tumor. Somatostatin receptor scintigraphy (SRS), which can visualize SSTR-2 expression, has been approved in Japan. The SRS will be a useful diagnostic tool for locating neuroendocrine neoplasms, detecting distant metastasis, and evaluating therapy outcomes. In this manuscript, we review the latest diagnostic methods and treatments for PNENs.
