        Long-Term Outcomes of FIM Motor Items Predicted From Acute Stage NIHSS of Patients With Middle Cerebral Artery Infarct

        Objective To outline the association between the National Institutes of Health Stroke Scale (NIHSS) in the acute stage and the Functional Independence Measure (FIM) of motor items several months later. Methods Seventy-nine infarct cases with middle-cerebral-artery region transferred to long-term rehabilitation facilities were analyzed. Patients were allocated to either the model-development group or the confirmatory group at a 2:1 ratio. Independent variables were based on the NIHSS during the acute care and on demographic factors such as age and modified Rankin Scale (mRS) before onset. Multivariate logistic analyses were performed to predict the independence of each FIM motor item. These models were evaluated in the confirmatory group.Results Multivariate logistic analyses in the model-development group (n=53) indicated that at least one NIHSS item was statistically significantly associated with the functional independence of a single FIM motor item. Of the NIHSS items, the affected lower extremity item was the most widely associated with 11 of the FIM motor items, except for eating and shower transfer. The affected upper extremity function was the second widely involved factor associated with 7 of the FIM motor items including eating, grooming, bathing, toileting, bed transfer, toilet transfer, and shower transfer. Age and mRS were also statistically significant contributing factors. The obtained predictive models were assessed in the confirmatory group (n=26); these were successful except for the stairs climb item.Conclusion In combination with age and pre-stroke status, the NIHSS items (especially the affected extremity items) may be useful for the prediction of long-term outcome in terms of activities in daily living.

        Comparison of the diagnostic performance of panoramic and occlusal radiographs in detecting submandibular sialoliths

        Purpose: The aim of this study was to assess and compare the diagnostic performance of panoramic and occlusal radiographs in detecting submandibular sialoliths. Materials and Methods: A total of 40 patients (20 cases and 20 controls) were included in this retrospective study. Cases were defined as subjects with a submandibular sialolith confirmed by computed tomography (CT), whereas controls did not have any submandibular calcifications. Three observers with different expertise levels assessed panoramic and occlusal radiographs of all subjects for the presence of sialoliths. Intraobserver and interobserver agreement were assessed using the kappa test. Sensitivity, specificity, accuracy, positive and negative predictive values, and the diagnostic odds ratio of panoramic and occlusal radiographs in screening for submandibular sialoliths were calculated for each observer. Results: The sensitivity and specificity values for occlusal and panoramic radiographs all ranged from 80% to 100%. The lowest values of sensitivity and specificity observed among the observers were 82.6% and 80%, respectively (P=0.001). Intraobserver and interobserver agreement were higher for occlusal radiographs than for panoramic radiographs, although panoramic radiographs demonstrated a higher overall accuracy. Conclusion: Both panoramic and occlusal radiographic techniques displayed satisfactory diagnostic performance and should be considered before using a CT scan to detect submandibular sialoliths.

      • Self-Tuning Control for Ratational Speed of Slider-Crank Mechanism

        This paper proposes a self_tuning control method based on generalized minimum variance control for rotational speed control of a slider_crank mechanism. Dynamic characteristics of the slider_crank mechanism significantly changes depending on the wheel rotational angle. Thus, to achieve a good control performance, it is necessary for the plant parameters to be identified at the each section divided into six by the wheel angle. This kind of control based on the local modeling is useful not only for such a slikder_crank mechanism but also for highly_accurate positioning control systems with significant nonlinearity. The validity of the proposed method is verified through simulations and experiments.

        Lactobacillus acidophilus와 Streptococcus thermophilus에 의한 ultrafiltration處理 牛乳의 醱酵促進을 위해 利用된 無機物과 L-cysteine의 효과를 要約하면 다음과 같다. 1. L. acidophilus 單獨 培養時 酸生成은 全乳, UF-retentate 및 濃縮乳중 全乳에서 가장 높게 나타났으며, L. acidophilus와 S. thermophilus를 混合 培養할때의 젖산 濃度 증가는 UF-retentate에서 제일 높게 나타났다. 생존 젖산 균수는 L. acidophilus 단독 培養時에는 거의 유사하나, 混合 培養時에는 UF-retentate에서 제일 많았다. 2. L. acidophilus와 S. thermophilus 混合 培養時의 酸生成이 L. acidophilus 單獨培養時보다 더 높은 수준을 나타내었다. 생존 젖산 균수에 있어서는 L. acidophilus 單獨 培養時 더 높았으나 UF-retentate에서는 單獨 및 混合 培養時 生存 젖산 菌數가 유사하였다. 3. L. acidophilus 單獨 培養時 全乳, UF-retentate 및 濃縮乳에 無機物을 添加하면 酸生成이 促進되며, 微生物 數가 증가하였고, L. acidophilus와 S. thermophilus를 複合 培養시에는 濃縮乳에서만 徵生物 數가 증가하였다. 4. L. acidophilus 單獨 培養時 UF-retentate에 L-cysteine을 添加했을때 酸生成이 促進된다. The effects of minerals and L-cysteine on the fermentation of ultrafiltrated milk by Lactobacillus acidophilus and Streptococcus thermophilus are summerized as follows; 1. L. acidophilus inoculated solely produced more acid in whole milk than in UF-retentate and evaporated milk, mixed culture of L. acidophilus and S. thermophilus produced acid most in UF-retentate. The viable cells of lactic acid bacteria of the sole culture of L. acidophilus were similar in whole milk, UF-retentate and evaporated milk, whereas viable cells of the mixed cultures of L. acidophilus and S. thermophilus were highest in UF-retentate. 2. The mixed culture of L. acidophilus and S. thermophilus produced more acid than the sole culture of L. acidophilus. The viable cells of L. acidophilus inoculated independently usually showed the higher level, but those of the mixed and the sole cultures were similar in UFretentate. 3. Minerals added to whole milk, UF-retentate and evaporated milk, increased the acid production and viable cells of L. acidophilus. In the case of fermentation by the mixed culture of L. acidophilus and S. thermophilus, minerals only increased the viable cells in evaporated milk. 4. L-cysteine stimulated acid production in UF-retentate formented by the sole culture of L. acidophilus.

        Comparison between different cone-beam computed tomography devices in the detection of mechanically simulated peri-implant bone defects

        Purpose: This study compared 2 cone-beam computed tomography (CBCT) systems in the detection of mechanically simulated peri-implant buccal bone defects in dry human mandibles. Materials and Methods: Twenty-four implants were placed in 7 dry human mandibles. Peri-implant bone defects were created in the buccal plates of 16 implants using spherical burs. All mandibles were scanned using 2 CBCT systems with their commonly used acquisition protocols: i-CAT Gendex CB-500 (Imaging Sciences, Hatfield, PA, USA; field of view [FOV], 8 cm×8 cm; voxel size, 0.125 mm; 120 kVp; 5 mA; 23 s) and Orthopantomograph OP300 (Intrumentarium, Tuusula, Finland; FOV, 6 cm×8 cm; voxel size, 0.085 mm; 90 kVp; 6.3 mA; 13 s). Two oral and maxillofacial radiologists assessed the CBCT images for the presence of a defect and measured the depth of the bone defects. Diagnostic performance was compared in terms of the area under the curve (AUC), accuracy, sensitivity, specificity, and intraclass correlation coefficient. Results: High intraobserver and interobserver agreement was found (P<0.05). The OP300 showed slightly better diagnostic performance and higher detection rates than the CB-500 (AUC, 0.56±0.03), with a mean accuracy of 75.0%, sensitivity of 81.2%, and specificity of 62.5%. Higher contrast was observed with the CB-500, whereas the OP300 formed more artifacts. Conclusion: Within the limitations of this study, the present results suggest that the choice of CBCT systems with their respective commonly used acquisition protocols does not significantly affect diagnostic performance in detecting and measuring buccal peri-implant bone loss.

      • Maximization of sodium storage capacity of pure carbon material used in sodium-ion batteries

        <P>Generally, carbon anode materials used in sodium-ion batteries do not exhibit good electrochemical performance because of low coulombic efficiency (CE). This paper presents a strategy to overcome this limitation by causing a co-intercalation reaction in a newly designed material. Here, Na was doped inside carbons and desodiation was caused by cleaning the doped Na. Consequently, the CE consistently exceeded 100%. Furthermore, new spaces were created when the doped Na was released continuously from the carbons, thereby allowing more Na to be stored in these spaces. This consistently increased the reversible capacity during cycling. Even though the designed material was a nanomaterial with a large specific surface area, the CE in the first cycle was 85%. Because of the co-intercalation reaction, a solid-electrolyte interphase (SEI) layer might not be formed depending on the anode surface structure and continuous long-term stable cycling was possible even without an SEI layer. Thus, a useful material for sodium-ion batteries can be designed using only carbons and without next-generation materials.</P>

      • Influence of Growth and Yield by Diffrence of Seeding Accuracy and Depth on Green Soybeans Cultivation used Up-cut Type Ridging and Mulching Seeder

        A cultivation outline of green soybeans could not set mechanical seeding technics to have difference of mulched soil temperature at any seeding season. Especially, the early cropping type of green soybeans seeding that used mulcting method to get high emergence rate on low temperature season have been high labor load to seed by manual operation. Therefore, the purpose of this study was to develop high efficiency and precision mechanical seeding technique which have ridging and mulching same time. We developed prototype seeding machine combined with up-cut rotary, mulching seeder and the molding implement. After that, we made control device to move diching device for up and down direction. We investigated the seeding accuracy and seeding depth of the prototype seeder, and evaluated the growth and yield, respectively. As a result, it is showed that the seeding accuracy is as same as manual seeding. Although the seeding depth was shallow compared to the conventional manual seeding, it can increase seeding depth than conventional seeding. However, the measured seeding depths were shallower than the set values. This is reason why was influenced of soil compression, posture of the seeding part and covered soil after seeding. The plant length at harvest time might become longer as the seeding depth got deeper. So, we considered that it is possible to do multiple simultaneous work used up-cut ridging and mulching seeder.

        Removing Boundaries, Losing Connections: Electoral Consequences of Local Government Reform in Japan

        In this article we examine the role of local politicians in affecting national-level election outcomes by focusing on the drastic municipal mergers in Japan that took place in the early 2000s. Specifically, we argue that the political party that relies most extensively on local politicians’ efforts for electoral mobilization and monitoring will suffer an electoral slump when municipalities are merged and the number of municipal politicians is swiftly reduced. We empirically show that municipalities with a history of mergers exhibit significantly lower voter turnout and obtain a smaller vote share for the ruling Liberal Democratic Party in national elections when compared to other municipalities without an experience of mergers. This result indicates that municipal politicians are indispensable human resources for LDP candidates running for the national parliament.
